Employee was going around a corner on a shuttle car. Their hand was on the handle on the canopy. They rubbed the corner of the rib with the canopy. They caught a finger between the hand hold and the rib causing a laceration that required stitches.

Employee had dropped the buggy cable that was hung. When employee walked around the corner, the shuttle car operator started the car. The slack that was on the ground reeled up hitting the employee causing employee to fall and hurt right shoulder.

Employee was going to the feeder to dump a load of coal. EE ran over a rock in the belt entry causing EE to bounce and jar their back.

Employee was riding in the back of a man trip, hit a bump in the travel way causing EE to hit EE's head on the mine roof, which caused EE's hard hat to be pushed down against EE's nose. This resulted in a fracture and a deviated septum. EE had surgery on 3/2/2023 to repair the septum, which was EE's first day of missing work.

Employee was riding in the back of the mantrip on the way out of the mines. There was a low spot just in by 5C head. EE's cap light caught a plate that was against the mine roof causing EE's head to be pushed back into the back of the mantrip causing a laceration to the back of the head. This required 5 staples.

Employee was loading dust into the rock duster with the forklift. The bag dust was over the pod. The forks slipped and hit employee in the side causing a cracked rib. This is a contracted employee.

Employee was hanging the miner cable so the scoop could go under it. the scoop tire caught the side of EE's foot causing a slight fracture.

Employee was unloading supplies from a flat car with a scoop. They had the bucket bowed on the flat car with the front tires off the ground. The bucket slid off the car which caused the scoop to hit the floor and caused employee to hit their head on canopy resulting in a slight fracture to their spine in their neck. No surgery is required.

Employee had went around the block to let the other supply tractor pass. As EE tried to pull out into the roadway, the tractor high centered on some gob which raised the rear wheels off the ground. EE was using their foot to push some rocks under the wheel to try to get traction. The rear of the tractor swiveled down pinning EE's leg under the tractor causing EE to twist knee.

EE was riding in the front seat of the mantrip going to the section. There was pogo sticks on the front of the man trip. The trip passed through a flypad and the flypad caught the pogo stick and forced them into EE's chest causing a laceration and a broken rib.

Employee was pinned by machine bucket against rockwall causing injury to left shoulder and neck.

Got off 12 ton motor on bottom of slope to hook up supply car when the employee got foot caught between 12 ton and mine floor.

Employee was driving a stinger and ran over a crib block. The steering wheel spun and the steering knob struck the right hand. This resulted in a fracture to the 5th(pinkie)finger.

He was on a scoop taking down old 5b head. He ran over a block hurt upper back. He finish shift. He continued to work everyday regular duties until 11-9-15. His first day missed was on 11-10-15.

According to MSHA Inspector, Employee operating shuttle car hit hole, hit head on canopy. This 7000-1 is being filed in order to terminate a citation written during a Part 50 Audit. This is filed under protest. Excel Mining disagrees that a job related injury occurred.

Employee was operating a battery powered 4 wheeler. While traveling the track entry the 4 wheeler slid off the track causing employee's neck and lower back to be jarred. Employee was seen by local physician who removed him from work pending MRI results.

Employee was in the process of cleaning a tail piece. As he was shoveling under the tail piece the bottom belt came in contact with the shovel, jerking the shovel causing pain in his right arm. He ceased work and was taken to a local hospital where X-Rays were taken. He was taken off work until a follow up on 8-2-2012.

Employee had a timber that was knocked. He got it out of roadway, and laid it on the rib. His buggy cable pulled it back out in roadway, and when he came back from miner with a load of coal. He ran over timber hitting hard hat on buggy canopy hurting his neck.

While in the process of using a scoop to clean in an outby area, the scoop deck struck rib causing a piece of material to come loose and fall onto employee's back, resulting in contusions. An Xray was performed revealing no broken bones.

Employee completed loading shuttle car from the #6 right, car left to return to feeder and traveled approximately 30 feet up a grade in 5 right. For unknown reason the shuttle car rolled backwards pinning him between shuttle car and the rib, resulting in fracture of the right leg and left ankle.

While checking the radiator of a bobcat loader employee's right ring and middle fingers came in contact with fan blades resulting laceration and fracture to both fingers.

The trucks bed started sliding around to the left and he tried to correct it. His R/front tire struck the berm on his right side causing the bed of the truck to turn on around and his L/front tire struck the berm. This caused truck to turn striking the left berm below him with his left front tire causing the cab of the truck to overturn.

While operating a 4 wheeler, the front tire struck a concrete block causing the steering knob to strike employee's right thumb, causing a fracture.

While traveling on a stinger personnel carrier, employee's hard hat struck a roof bolt, causing his hard hat to fall off and his head to hit the roof bolt plate, resulting in a laceration.

Injured employee was entering a tool shed on his rubber tired mantrip when he struck the wall of the area injuring his right arm.

Injured employee was in the process of cleaning water sprays on a continuous miner that was positioned outby a check curtain in the number 4 entry. A battery power scoop started to travel through the check curtain, the bump from the scoop battery propelled the injured employee into the cutter boom of the continuous miner, resulting in a puncture wound to his left thigh.

General Laborer and Scoop Operator in process of removing a rock from top of continuous miner using scoop and 3/8 chain, chain was hooked to scoop's ram, general laborer hooked chain to rock, told scoop operator to pull up scoop to give him more slack on chain, inadvertenly operator pulled ram in, causing chain to tighten, causing lacerations to laborer's index and middle fingers.

While in the process of operating a 10 ton track motor, hauling supplies, dust from top of track motor blew into employee's left eye, causing irritation.

Injured employee, while driving through a curtain, felt some debris enter his right eye. It was later found to be a paint chip and had to be removed.

Injured employee, while on a rubber tired man trip, came off the track jarring his right wrist and hand. X-rays showed two breaks, one in the middle finger, one in the ring finger of his right hand.

Employee was operating a Joy 10 shuttle car. While in the process of making a left hand turn toward feeder, the operator's compartment rubbed the left rib, causing some coal to be loosened from the rib, striking the employee's left knee. Employee stated that when rib roll started to come inside operator compartment, he twisted his back. This accident is still under investigation.

EE was operating battery powered scoop. While in the process of changing batteries EE placed a crib block under scoop bucket & raised the front tires off mine floor. Scoop bucket slid off the crib block, causing scoop to jar EE. He was seen by local ER physician 3/3/09 & released to return to work. He visited his family doctor 3/9/09 & was removed from work.

Employee was operating a 2 man rubber tire johnson ride that was traveling thru a small water hole when the ride stalled. Employee dismounted ride and placed his body in front of the johnson ride to hook a chain to the front axle when the ride started moving forward causing the employee to be struck by the ride resulting in fractured ribs.

Injured employee was operating a 10 ton locomotive on the outby end of a supply train. Employee became panicked while descending a slight grade and jumped from the moving equipment. When landing, the employee suffered contusions.

Injured was a passenger on a johnson rubber tire vehicle that was in the process of parking next to a power center when his right knee struck an electrical plug. This resulted in a laceration which required medical treatment. He was given a full release from the ER physician to return to work on 7/1/08. He in return visited his family doctor and was removed from work.

EE was loading supplies with a front-end loader. As he was moving forward the front wheel came off of the machine. He continued to work the reainder of his shift. Later that day he visited a local hospital's emergency room where the attending dr diagnosed his condition as degenerative disc diease and contusions. Mgts feels due to dr' opiona. this is best classified as occ illness.

EE was riding a rubber-tire mantrip, when the man trip hit a hole in the roadway. This resulted in the EE's left foot to come in contact with the front tire of the machine, causing a contusion to his left ankle.

Injured employee was operating Johnson ride when the front tire struck a piece of Omega block on the mine floor. This caused the steering wheel to turn suddenly and the steering knob hit his right hand resulting in a fracture.

Injured EE was operating a battery powered 6 ton man trip that was approaching 5A track switch, when a second battery powered 6 ton man trip struck the operator compartment of the injured EE's man trip, resulting in fractures to his left foot.

EE was operating a tired mantrip. EE drove over a piece of draw rock causing the steering wheel to be jerked. This jerking of the wheel caused it to spin around striking EE on the right hand resulting in a fracture to his right hand.

While attempting to get a supply motor back onto the rail after it had de-railed, employee was struck on the left foot by the track motor resulting in 4 broken bones.

EE was operation a shuttle car in an area where the continuous miner cable was supported from the mine roof. He turned a curve the mine cable was jerked loose from the mine roof, falling onto the operation deck of the shuttle car. The cable struck him on the neck and shoulders. He was transported to a local E.R. where he was told not to return to work.

EE was holding an air lock door open until a scoop trammed past. When the rear tire of the scoop rolled past the door it struck the metal door pushing it upward. This upward movement of the door pinned his hand against the top causing 3 of his fingers to be fractured & 2 received lacerations. He was taken to a local hospital, examined & transferred to another hospital for surgery.

Injured employee was positioned at the end of the line curtain in no. 3 heading when he stepped into the path of a shuttle car that was in the process of backing up, pinning him between rib and shuttle car, resulting in a contusion and laceeration to his left leg.

Injured EE was operating a battery power scoop that was pushing on the high line sled assisting another battery power scoop that was pulling the section power center to maneuver a turn, when the injured EE scoop front tire ran over a 1 1/4" water line. This resulted in the water line striking his right eye, resulting in a fracture to his eye socket.

Injured employee was operating battery powered scoop when the front tire struck a pothole in the mine floor causing his head to strike canopy of machine. He was seen in local ER & issued full release to return to work. Later visited family dr for follow up where MRI was ordered. After reviewing the results of the MRI the family dr. ordered regimen of physical therapy.

While cleaning a break, employee injured his neck & shoulder when the front tire of scoop he was operating ran over a steel jack.

Employee was injured when a battery powered scoop came into contact with his foot. EE later complained of lower back pain & and independent medical exam was requested. On 7/17/07 a medical dr., stated it was reasonable to relate the back pain to the incident.

Employee was operating a supply motor attempting to pull a derailed supply car back onto the tracks. The chain attached to the supply motor and supply car broke, flew back into the operator's compartment striking his forehead causing a laceration requiring several stiches to close.
