Struck by flying object

Employee was disconnecting a 4"" water line from the water truck when the water line flew back and made contact with the employee's mouth, cutting EE's lip and chipping a tooth.

Over-exertion in pulling or pushing objects

Employee was pulling electrical wire in an awkward position when EE pulled a muscle in EE's left shoulder.

Fall from machine

Employee was climbing up steps of a rented loader, missed the top step, and fell to the ground injuring left hip, left knee, and left elbow.

The employee sat in the passenger seat of the supervisor's truck while a CB was being installed. It was at this time the employee experienced a possible personal medical event and fell out of the truck, striking their head. This caused a small laceration/abrasion to the right forehead which involved swelling.

Employee was fabricating a rock chute on ground. He had rolled chute over to access other side. Chute then rolled back on his right foot breaking his first 3 toes just behind the steel toe guard in his boot.

TWO BEARING CAPS HAD BEEN REMOVED FROM THE CRUSHER/JAW. THEY WERE PLACED ON A PALLET. THE MINER WAS ASSIGNED TO CLEAN THE CAPS IN PREPARATION TO PUT IN NEW BEARINGS AND GREASE. AS HE WAS TURNING ONE OF THE CAPS OVER TO CLEAN IT, IT SLIPPED WHILE HE WAS HOLDING IT OVER STRIKING HIS RIGHT HAND BETWEEN THE TWO CAPS.

EE WAS HELPING MECHANICS WORK ON JAW CRUSHER WHILE MACHINE WAS INOPERABLE. EE WAS USING A PRY BAR ON A LARGE BEARING WHILE THE MECHANICS WERE TRYING TO REMOVE THE SAME BEARING. THE BEARING CAP MOVED AND THE PRY BAR EE WAS USING SLIPPED, CUTTING HIS RIGHT HAND BETWEEN HIS THUMB AND FOREFINGER.

EE WAS TYING OFF A LADDER THAT HAD BEEN PLACED INSIDE A PUG MILL HOPPER. WHILE TYING OFF THE LADDER, EE PRESSED HIS RIGHT SIDE AGAINST THE HOPPER WALLS. WHEN HE DID THIS, HE FELT A ""POPPING"" IN HIS RIGHT SIDE.

EE was removing a large stone from the haul road from the pit area which was too large to lift, by moving rolling it out of the roadway and sprang wrist on left hand.

EMPLOYEE WAS ATTEMPTING TO PERFORM REPAIRS ON WATER PUMP AT PUMP HOUSE WHEN HE INADVERTENTLY STEPPED ON A LOOSE BOARD WITH A NAIL IN IT.

EMPLOYEE WAS USING PRY BAR TO RE-POSITION A MASK PIN IN DRILL AND USING HIS OWN BODY WEIGHT TO PRY THIS PIN BACK INTO PLACE WHEN RIB FRACTURED.

EE'S INJURY WAS RE-DIAGNOSED SEVERAL DAYS AFTER THE ORIGINAL ACCIDENT REPORT AS TRAUMATIC CARPAL TUNNEL SYNDROME. HE WAS REFERRED TO A PHYSICIAN IN PINSON, ALABAMA FOR A NEW TYPE OF CHIROPRATI C TREATMENT INVOLVING WRIST AND HAND EXERCISES AND COLD LASER TREATMENT OF THE MEDIAN NERVE TO REDUCE NERVE SWELLING. THESE TREATMENTS LASTED FROM APPROXIMATELY SEPT 2001 UNTIL JANUARY 2002.
