EE was removing a damaged guard from the truck dump hopper. EE was carrying the broken guard off the truck dump hopper and in the process the guard pinched the tip of EE's middle finger, requiring four stitches. EE returned to work EE's next shift without restrictions.

EMPLOYEE WAS CLOSING A DOOR ON TRAILER. ANOTHER EMPLOYEE WAS GETTING OUT OF PICKUP TRUCK TO CLOSE THE TAILGATE, THINKING HE PUT THE TRUCK INTO PARK. TRUCK WENT INTO REVERSE AND THE TAILGATE HI T THE FIRST EMPLOYEE IN THE BACK OF LEGS AND HE HIT THE BACK OF THE TRAILER WITH THE FRONT OF HIS LEGS.

EMPLOYEE WAS USING AN ACETYLENE/OXYGEN TORCH TO TRIM A SMALL PIECE OF METAL OFF A CONVEYOR FRAMEWORK. A HOT SPARK BLEW BACK AND ENTERED HIS RIGHT EAR.
