Struck by falling object

EE sustained a contusion to the right forearm and minor swelling when the drill rod fell from above striking EE after the yoke cotter pin broke connecting the mainline and the rod while laying the rod on the rod rack.

Over-exertion in lifting objects

Employee twisted at the waist while lifting a HDPE pipe fusing machine and felt a dull burning pain in lower back. Employee was evaluated by a medical professional on June 6th and diagnosed with a lower back strain. The employee was placed on restricted duty from June 3rd to June 7th.

Employee was removing obstruction from a spring loaded backflow preventer valve when the screwdriver he was using to open the plate slipped and caught his middle finger on his left hand causing a laceration

Employee was reposistioning the flyght pump, when he grabbed the discharge hose to set the pump verticle, hose came loose and employee was sprayed in the chest and face.

Employee was moving sandbags by SB-6 when he slipped and fell causing his left leg to bend underneath his body. NOTE: On Sept. 16, 2009 employee felt discomfort in his knee, went to physician and was removed from duty.

Employee was trying to cut a hole in a plastic handle using a knife on a leatherman tool and the blade folded up cutting his right index finger. Employee was taken to Casa Grande Hospital emergency room.

Employees were removing the window AC unit. While lowering the unit on the table, his left fifth finger was caught under the unit, causing a small laceration.

Employee was walking along on the Mezzanine Level above the offices when he tripped on floor grating and fell. He attempted to break his fall with his arm, and subsequently broke his right arm.

Employee states while attempting to catch falling pvc, his left thumb became entrapped between bail and plate. Sustained laceration of left thumb.

EE WAS TRYING TO REMOVE A WINDOW FROM A WINDOW FRAME WITH A SCREW DRIVER. HE WAS PRYING OUT ON THE CORNER TO BREAK THE WINDOW FREE FROM THE WINDOW FRAME. A PIECE OF THE WINDOW BROKE OFF. HE TR IED AGAIN TO FREE THE WINDOW BY REACHING THROUGH THE HOLE OF THE WINDOW AND PULLING OUTWARD. A BIGGER PIECE OF WINDOW BROKE OFF AND FELL ONTO HIS RIGHT WRIST CAUSING A LACERATION REQUIRING SU
