Caught in, under or between a moving and a stationary object

EE was lowering a slab of sandstone to the ground when he pinched his left middle finger between ground and the stone slab. DR. Diagnosis: laceration & contusion left middle finger. EE lost 3 work days.

Over-exertion in lifting objects

Employee was lifting a 4"" thick slab of sandstone up onto its edge to be split when he sustained pain in his lower back. Physician diagnosis: strain lumbar spine.

EE was lifting a block of stone into position to be split when he sustained a sharp pain in his left shoulder. Physician diagnosis: left shoulder injury, no use of left shoulder for 7-10 days.

Employee was lifting a slab of sandstone using a 36"" crow bar. The crow bar slipped causing the stone to strike his right leg. He declined medical treatment at that time. On 9-26-05 he requested medical treatment for same injury. Physician diagnosis: infected hematoma, employee not to work until further instruction from physician.

Struck by flying object

While splitting a piece of stone with a 4 lb. hammer and stone chisel, a metal fragment broke off the chisel puncturing employee's left wrist. Metal fragment was removed by a physician. Employee was placed on light duty for three workdays and returned to full duty on 9-20-05.

Employee sustained a fracture to his right ring finger when a block of wood standing on end atop a large block of stone was blown over by the wind, striking his finger. Injury not reported until next morning 8-24-05, at which time he received medical treatment. He returned to regular duty 8-24-05 at his own request.

EMPLOYEE WAS SPLITTING A PIECE OF STONE WITH A 4 LB HAMMER AND STONE CHISEL WHEN HE WAS STRUCK IN THE RIGHT EYE BY A STONE FRAGMENT WHICH ENTERED OVER THE TOP EDGE OF GLASSES. HE ADMITTED HIS SAFETY GLASSES HAD SLID DOWN HIS NOSE DUE TO PERSPIRATION, EXPOSING HIS EYES TO INJURY. DR DIAGNOSIS: TRAUMATIC CONJUNCTIVITIS, PERSCRIPTION MEDICATION GIVEN

EE WITH ANOTHER EE WAS ATTEMPTING TO LOAD A LARGE PIECE OF MOSS ROCK ONTO A FLAT BED TRUCK BY HAND. WHEN EE SUSTAINED A SHARP PAIN TO LOWER BACK. EE WAS TREATED BY PHYSICIAN ON 3/14/05. NO WORK FOR ONE WEEK. APPT ON 3/21/05

EMPLOYEE WAS CARRYING A ROLL OF PALLET BANDING MATERIAL ON HIS SHOULDER WHEN HE SLIPPED ON SNOW & ICE COVERING THE GROUND. AS HE FELL FORWARD HE PLACED BOTH HANDS IN FRONT OF HIS BODY TO BREAK THE FALL, AT WHICH TIME THE ROLL OF BANDING CAME OFF HIS SHOULDER LANDING ON HIS RT. HAND & THUMB.

EE WAS USING A METAL PRY BAR TO LIFT AND POSITION A SLAB OF SANDSTONE FOR SPLITTING WHEN THE PRY BAR SLIPPED OUT FROM UNDER THE STONE. THE STONE FELL ON HIS LEFT FOOT. HE WAS WEARING FULL METATARSAL SAFETY SHOES. DOCTOR REPORTED NO BROKEN BONES BUT A CONTUSION OF THE LEFT FOOT.

Ee was lifting a piece of stone weighing approx. 80 lbs onto a pallet when he felt a sharp pain in the muscles of his right ribcage area. Injury appears to be caused by over exertion on the part of ee.

EMPLOYEE WAS BENT OVER FORWARD TRYING TO PUSH A 3"" THICK X 3' X 3' SLAB OF STONE ONTO A PALLET WHEN HE SUSTAINED A SHARP PAIN TO HIS LOWER BACK. CONDITIONS CONTRIBUTING TO THIS INJURY APPEAR TO BE OVEREXERTION BY THE EMPLOYEE. STONE OF THIS SIZE AND WEIGHT SHOULD BE MOVED BY MECHANICAL MEANS (10 LOST WORKDAYS).

EE was walking a piece of flagstone onto a pallet when a gust of wind pushed him forward causing him to slam the stone he was walking. a piece of stone broke off the top of the stone striking his left index finger, causing a cut.

EE was lifting a piece of sandstone onto a pallet when he felt a sharp pain in his lower back. He was sent to the Dr and was restricted from working for two days, the, put on light duty for an undetermined number of days.

HE TWISTED HIS UPPER BODY TO THE RIGHT TO REACH FOR AN INSTALLATION BOLT WITH HIS LEFT HAND, CAUSING A SHARP PAIN TO HIS LOWER BACK. EE WAS PLACED ON LIGHT DUTY BY DR 3-2-04.

WHILE PICKING UP STONE TO BE LOADED ON A PALLET EE GRABBED A LARGE PIECE WHICH WAS COVERED BY SMALLER PIECES OF STONE AND TURNED TO ONE SIDE TO DISLODGE THE STONE FROM THE BOTTOM OF A PILE. TH E STONE DID NOT DISLODGE BUT HIS LOWER BACK DID CAUSING LOWER BACK PAIN.
