WHILE MOVING ANOTHER PIECE OF STONE INTO POSITION ALONGSIDE THE BLOCK, THE STONE BEING MOVED CAME DOWN ON THE HAMMER HANDLE CAUSING THE HAMMER TO FLIP UPWARD STRIKING HIM ON THE HEAD.

EE WAS SENT TO THE DOCTOR 9/27 & WAS RELEASED TO PERFORM LIGHT DUTY UNTIL NEXT APPT. IN 2 WEEKS. (11/2/04: EE SAW SURGEON; WILL HAVE HERNIA SURGERY IN NEAR FUTURE.)
Employee was splitting a slab of sandstone which was laying flat on the ground using a stone chisel and 6 lb. hammer. While attempting to strike his chisel the hammer first struck the ground causing him to strike his left thumb.
Employee was peeling the face of a piece of stone using a stone chisel and 4 lb hammer when he inadvertently struck the first knuckle on his left thumb with the hammer. The injury was not reported until the morning of 9-20-04. His thumb swelled up over the weekend. He was sent to the doctor on 9-20-04.
Ee was splitting stone with a chisel and 6 lbs hammer. as he swung his hammer toward his chisel, the stone unexpectantly separated causing him to pinch his right index finger between the stone and his hammer handle.
EE WAS SPLITTING A LARGE BLOCK OF SANDSTONE USING A CHISEL AND 16 LB. SLEDGE HAMMER. WHILE ATTEMPTING TO STRIKE THE CHISEL WITH THE HAMMER, EE INADVERTENTLY STRUCK HIS LEFT HAND WHICH WAS HOLDING THE CHISEL. RESTRICTED WORK ACTIVITY FOR APPROX. 1 WEEK.
While using metal banding to secure stone on pallets, a metal fragment punctured the palm of ee's left hand. Ee extracted the metal fragment using a needle, at home that night. The injury was not reported until his hand became infected on 8-4-04. Ee was x-rayed, given pain & antibiotic medications.
EE was splitting a piece of stone with a hammer and chisel. He hit the chisel head with the hammer and a small particle of steel projected off the chisel and penetrated his left arm 3"" below the inside elbow.
EE WAS STACKING STONE ONTO A PALLET WHEN HE STEPPED FORWARD WITH HIS RIGHT FOOT ONTO A LOOSE STONE CAUSING HIM TO LOSE HIS BALANCE. AS HE TWISTED HIS UPPER TORSO TO REGAIN HIS BALANCE HE SUSTAINED A SHARP PAIN TO HIS LOWER BACK.
EE WAS SPLITTING STONE WHEN A DUST DEVIL CAME CLOSE TO HIS SPLITTER STATION BLOWING DUST/DIRT INTO HIS LEFT EYE. HE DID NOT REPORT THE INJURY UNTIL THE FOLLOWING DAY 4-23-04, AT WHICH TIME HE QUANTIFIED IMPAIRMENT - NONE, DAMAGE - 1 LOST WORK DAY.
EE WAS LIFTING A SLAB OF STONE HE HAD SPLIT WHEN THE STONE FRACTURED & BROKE IN TWO. THIS FORCED HIM TO QUICKLY LOWER THE PIECE OF STONE HE WAS HOLDING ONTO, PINCHING HIS FINGERS WHEN THE STONE LANDED ON THE TOP OF THE BLOCK OF STONE THE SLAB WAS SPLIT FROM. NO BROKEN BONES.

WHILE REMOVING A LARGE BLOCK OF STONE FOR PROCESSING USING A 220 VOLVO LOADER, THE STONE BROKE CAUSING HALF THE STONE TO FALL FROM THE LOADER. THIS IN TURN CAUSED THE LOADER TO JUMP UPWARD AND DROP. THIS JARRING MOTION TO EE'S BACK CAUSING PAIN.
SET SELECT STONE ON PALLET, TURNED AWAY TO CONTINUE SPLITTING BLOCK, SELECT PIECE FELL ON LOWER LEG ABOVE ANKLE AND SCRAPED MUCH FLESH OFF OF LEG.
WHILE SPLITTING BLOCK, SWUNG HAMMER HAND BACK AND HIT NEARBY PALLET, CAUSING HAMMER TO RICOCHET BACK INTO HIS CHISEL HAND, SMASHING IT.
SPLITTING STONE & PULLED MUSCLE IN STOMACH.
SPLITTING PATIO STONE ON UNLEVEL TERRAIN & TWISTED HIP.
SPLITTING STONE WITH CHISEL AND HAMMER AND FELT PAIN IN ARM AND SHOULDER.
EE PUT SODA CAN TO MOUTH TO DRINK FROM IT, A BEE WAS IN AND STUNG HIM ON THE UPPER LIP.
HE WAS LIFTING A LARGE BLOCK WITH HIS HANDS AND FELT A PULL IN HIS STOMACH.
SPLITTING STONE AND A PIECE OF STONE BROKE OFF AND FELL ON HIS RIGHT TOE.
WHILE SPLITTING STONE, SOMETHING FLEW UNDER HIS GLASSES INTO HIS EYE. THE DOCTOR DID NOT FIND ANYTHING IN HIS EYE BUT SAID IT WAS IRRITATED. HE DID NOT REPORT THIS UNTIL HE CAME INTO WORK ON M ONDAY 3/24/03.
