operator was working on the journal in the Rm when EE's hand became pinched causing a small Skin Avulsion

Third party temporary laborer hit their hard hat on the frame of a conveyor in the central crushing area. The impact resulted in a neck strain. this was not reported until the 12th, at which time the laborers company (ICR) sent them to their industrial clinic

Over-exertion in pulling or pushing objects

Employee was using a pry bar to dislodge broken plow tips in RMI. They felt the job was strenuous but was alright when they completed it. They had a prescheduled day off 10/28/24 and returned to work on 10/29/24. Their back was stiff and mentioned it to the Maint. Coordinator. By 10:00 AM their back had a spasm, and they could hardly walk.

Over-exertion in lifting objects

Employee was on the Sentinel Haul Road, got out of EE's haul truck to remove a small tree that was in the roadway. EE was on unlevel surface, did not test the weight of the tree and did not use safe lifting procedures. Later in the week, employee began to feel pain in EE's abdomen and reported the incident on 4/19/2024.

Fall from machine

Employee arrived at work and was getting out of EE's truck to go clock in. As EE was getting out of EE's truck, EE's left foot slipped on black ice, and EE fell out of the truck onto the asphalt injuring left shoulder. It had snowed during the night and the parking lot was covered with black ice and snow.

While removing the MyMill metal frame from the building, mechanics were rotating it 90 degrees because of limited spacing and to avoid overhead electrical wiring. The mechanic's right ring finger got pinched between the metal frame and the eyebolt connected to the motor.

Electrician was descending the stairway. EE had one hand on the handrail, but was carrying a pipe in the other hand. EE missed the last step and as EE stepped down EE injured EE's right foot and ankle. EE heard a loud popping sound

Employee was pushed and as EE stepped back, EE stepped down missing a step, lost EE's balance and fell on EE's right knee sustaining injury to multiple body parts.

While tightening lug nuts on a 992 loader the employee was using a 1 inch impact and EE noticed that the ring holding the safety pin on the extension was coming out and by the time EE could react the pin had come out and caught EE's wrist, causing the cut. A fellow employee helped administer first aid to stop the bleeding and bandaged it up.

Employee was using a Kenworth water truck to provide dust suppression and ran over an unseen rock causing the truck to swerve and the operator felt pain in EE's neck from the movement. The operator went to the company clinic and was placed on modified duty for 1 week till re-evaluation

Forklift operator was loading a contract van truck and they had a miscommunication and the driver started to reposition the truck by pulling forward at the same time the operator was returning to start loading. The forklift did not fully enter the truck and the rear tires of the forklift did not make it into the trailer causing a jolt to the operator and a sore back.

The geologist was collecting drill hole samples on a quarry bench and did not see a rock and stepped on it causing ankle to twist. This employee filed a report at that time and thought it would be ok. After some time the discomfort returned and the employee went to visit the DR. and the subsequent modified duty was issued.

At 1527 on 10/29/2020 was notified commercial motor vehicle driver was found collapsed/unresponsive at upper load out scale. EMS arrived onsite and took over CPR. At 1655 per doctor on duty at Medical Center driver did not survive. Driver suffered a heart attack that was fatal due to natural causes. Mine condition and equipment were not involved.

Employee was using a hand-held drill to insert a screw into an electrical box he was holding. The box flipped causing the drill to slip resulting in the screw bit puncturing the soft tissue fingertip, ring finger, left hand.

Hearing screams for help looked up and saw coworker on fire. Ran to him from ground up 1 level and began snuffing flames with gloved hands, then grabbed victim's coat and snuffed the flames with it. Took off gloves in process and as a result 3 fingers and thumb on left hand received burns because melted polyester stuck to them.

Employee was welding an inspection port on Raymond Mill #1 feed silo when his clothes caught on fire causing burns to hands, arm, and abdomen area. Another employee helped put out the fire and was also burned. Two employees were injured.

Employee was walking in the mill bldg, 2nd floor, when he lost his balance and stumbled to the floor on the same level hitting his left knee against equipment causing an open wound.

On 9-27-2010 employee submitted an accident report claiming that on 4-19-2010 his foot got tangled on banding material stuck under a pallet causing him to trip and fall off a platform and hit a hopper before landing on the floor. Accident was reported 23 weeks after the injury. Employee went on FMLA on 5-12-2010.

Employee was walking up the mill building stairs when he hit a stair step edge with his boot causing him to fall forward. When falling he hit handrail support post between 4th & 5th fingers on his left hand causing a laceration.

On 6-23-2008 employee placed into position 82234 conveyor motor when he felt a pull muscle on his left shoulder. Emp did not want medical evaluation at that time. Emp asked for medical evaluation and will be receiving physical therapy on 11-5-08.

Employee was torquing terciary crusher liner segment bolts with a hydraulic torque wrench. When the wrench was operated it slipped and squeezed his right hand's index and middle finger against the wall of the crusher.

Employee was instaling an axle on a haul truck when the drive gear slid back on the spline pinching his left palm between the gear and retaining ring resulting in an open wound requiring stitches.

On 6-28-07 employee was reaching over his head pulling and twisting a haul truck bed pin in order to remove it. Once removed, employee felt soreness in his right shoulder. Employee mentioned his soreness but did not want to medical attention. Soreness continued. On 7-31-07 employee requested medical evaluation and was put on modified duty.

Employee placed a rolled up conveyor belt onto a pallet with another employee. When he climbed onto a forklift to move the pallet he felt a pain in his lower back. Pain got worse as he continued to operate forklift.

The employee was climbing down the ladder of payhauler truck when his foot slipped on the last rung of the ladder. He was holding on with both hands so he wouldn't fall and his body swung toward the ladder causing a contusion to his right chest wall and abdomen. This injury requires modified work.

EE WAS REMOVING A ROCK FROM THE ROAD, SLIPPED ON GRAVEL AND FELL. HE DID NOT NEED MEDICAL ATTENTION THAT NIGHT. ALMOST ONE MONTH LATER HE REQUESTED MEDICAL ATTENTION ALLEGING HE WAS EXPERIENCING PAIN IN MULTIPLE BODY PARTS. HE WAS SENT TO THE DOCTOR ON 8/24/05 AND NEEDS PHYSICAL THERAPY. HE WAS ON REGULAR WORK STATUS. PER CALL WITH PAT ON 09/02, HE IS ON RESTRICTED DUTY

EE WAS WALKING DOWN THE 82 PALLETIZER WITH A BROKEN BAG OF CALCIUM CARBONATE. EE WAS AT THE BOTTOM OF THE STAIRWAY WHEN HE TWISTED ANKLE AS HE STEPPED DOWN TO A LOWER STEP. EE FELL AND LANDED ON KNEE ON THE CONCRETE FLOOR.

TESTING AIR HEATER FOR RM1 SINCE IT WAS REBUILT BECAUSE OF THE FIRE OF 1/26/04. THE HEATER WAS STARTED AND THE PRODUCT WAS CHECKED. IT APPEARED GRAY AND THE HEATER WAS SHUT DOWN IMMEDIATELY (JUST A FEW MINUTES AFTER IT WAS STARTED). THE FLAME WOULD NOT GO OUT AND THE FIRE DEPARTMENT WAS CALLED. AN INVESTIGATION OF A FIRE IN OUR FLASH DRYER REVEALED THAT DIESEL FUEL HAD COLLECTED IN THE COMBUSTION CHAMBER OF THE HEATER. WHEN THE HEATER WAS TURNED ON THE FUEL IGNITED CAUSING THE FIRE. THE FIRE WAS CONTAINED IN THE CHAMBER AND THERE WERE NO FLAMES OUTSIDE THE HEATER. THE ENTIRE INVESTIGATION IS NOT COMPLETE & A REPORT WILL FOLLOW UPON COMPLETION OF OUR INTERNAL INVESTIGATION

EE WAS IN A MANLIFT REMOVING AN ELECTRICAL BOX.THE ELECTRICAL BOX FELL ONTO A PAIN CAN THAT WAS LAYING ONT HE BOTTOM OF THE MANLIFT. THE BOX PUNCTURED THE PAINT CAN AND WHITE PAINT SPRAYED ALL OVER THE EE S FACE(EYE, NOSE, MOUTH, EARS) AND COVERED HIS HANDS.

EMPLOYEE AND A FORKLIFT MADE EYE CONTACT AND THE EMPLOYEE WAS WARNED BY ANOTHER EMPLOYEE TO MOVE OUT OF THE WAY. THE FORKLIFT DRIVER TURNED THE CORNER AND MISJUDGED HIS CLEARANCE FROM THE EMPL OYEE AND THE BACK OF THE FORKLIFT SWUNG AROUND AND FORKILFT DRIVER RAN OVER THE BACK OF THE EMPLOYEE'S RIGHT ANKLE.

HE DID HAVE HARD HAT ON BUT T HE IMPACT CAUSED THE ADJUSTMENT KNOB ON HAT TO BREAK & HAT FLEW OFF. HE HAD A LACERATION ON THE TOP OF HIS HEAD THAT REQUIRED STITCHES.

WHILE USING AN OVERHEAD STACKER CRANE, EE WAS MOVING A SMALL BLOWER FROM A PALLET TO THE BOTTOM OF THE STORAGE SHELF. WHILE POSITIONING THE BLOWER, THE WEIGHT SHIFTED & EE'S RIGHT THUMB WAS SM ASHED BETWEEN 2 BLOWERS.

THE MAINTENANCE TEAM WAS LOWERING A HEAVY STEEL TABLE AT THE ROLLER MILL #2 MILL BASE. THE SENIOR MECHANIC INSTRUCTED THE OTHERS TO MOVE OUT OF THE WAY. THE TEAM WAS USING A CHAIN-FALL BUT SHOULD HAVE USED ADDITIONAL LIFTING DEVICES TO CONTROL TABLE.

BUT CHOSE TO LIFT THEM AS OPPOSED TO USING A FORKLIFT OR OTHER TYOES OF MECHANICAL LIFTS. EE FAILED TO REPORT THE ACCIDENT. THE EMPLOYER DID NOT FIND OUT UNTIL 1-25-01. 1ST LOST TIME DAY 1-24

EMPLOYEE WAS PUTTING A COMPUTER BACK TOGETHER. WHILE LOWERING THE COMPUTER CASE, HE LACERATED HIS FINGER ON THE LOCKING METAL TAB ON THE CASE,REQUIRING TWO STITCHES.

Fall from machine

EE WAS STANDING ON THE BACK OF THE SERVICE TRUCK, PICKED UP LARGE OIL DRAIN PAN WHEN A GUST OF WIND CAME UP AND BLEW HIM OFF OF THE TRUCK. HE LANDED ON A SOFT BERM AND WAS NOT INJURED AT THE T IME. HE REQUESTED MEDICAL TREATMENT SEVERAL DAYS LATER.

EMPLOYEE STEPPED UP ONTO THE FOURTH PALLET HIGH IN A STACK OF PALLETS SO I COLUD REACH A LOOSE BOARD AT THE TOP OF THE STACK. AS EMPLOYEE GRABBED THE BOARD, THE RING ON LEFT RING FINGER GOT CA UGHT ON A NAIL AND AS EMPLOYEE STEPPED OFF THE PALLETS LEFT RING FINGER WAS AMPUTATED.
