Employee (EE) climbed over platform guardrail onto raised forklift carrying screens, EE and screens fell from forklift 15 to 17 ft; EE landed on his feet, knees buckled, he collapsed to the ground & apparently one or more screens fell onto employee's body-trunk area abdomen region. Co-worker & injured worker moved screen(s) off of EE & EE scooted himself away from screens.

Fall down stairs

The employee slipped while descending two steps on the north mezzanine floor in Coloring. During the fall, they lost right-hand grip on the handrail, fell backward, and struck the corner of a toe board with the upper left side of their back. The injury resulted in fracture of the left thoracic 8-9 ribs and a bruised lung.

While unscrewing a bin-level probe by hand (without using a tool) for maintenance/troubleshooting, the employee felt pain in their right hand. The probe was threaded tightly and the employee applied force as it was not budging. It gave way quickly, resulting in the injury (sprain).

The employee was placing a hooded sweater into a gang box. When the employee reached inside the lid/door moved forward & down, hitting employee on top of head. Since employee removed hardhat so employee could take off the sweater, it left employee's head exposed. Employee received a 1 inch laceration.

While ratcheting the come-along used to close a chute the pad-eye/anchor failed and contacted the employee on the head (hardhat), face, and back.

Employee Caught The Tip of Their Left Ring Finger Between The Lever and The Chain Binder While Tightening The Lid of a Rotex Machine.

Employee was attempting to put the hub back on a roll crusher. As they were screwing the hub back onto the shaft, the employee thought the hub had caught the threads. Employee let go to reposition their hands to continue to screw the hub on. At this point, the hub dropped off and smashed the right index finger between the hub and pulley.

Fall onto or against objects

Employee was heading home after work and fractured right hand pinky finger while walking up a flight of stairs at the facility. Employee lost footing while walking up the stairs and used right hand to break their fall, resulting in the injury.

Employee was doing a preshift inspection on the sweeper. Employee took the filter out and bent over to tap it on the ground to clean and when the employee stood up, EE felt a pull in their lower back.

Employee reports they were setting up hoses to guzzle in the south mezzanine. Employee experienced a sudden sharp pain in the lower back when bending over to pick up a hose. The injury occurred during the movement/motion of bending, prior to lifting the hose.

Operator used a pipe to bang feed gate slide chute to close on #33 Rotex. Operator returned to work the next day & reported to supervisor that they woke up with their right shoulder hurting & aching.

An employee stated that they felt a snapping pain in the left knee while closing the top doors of a railcar. Employee went to the clinic. No medication was provided, but employee was placed on modified work duty.

The employee was removing a rotex screen from the screen rack on the 3rd floor and felt discomfort/pain in the left shoulder.

On 2/24 employee was removing a filter & strainer with their right hand and felt a slight pull in their right shoulder blade. Employee went on and finished the work day and went home. The next day, Saturday the 25th, employee woke up with noticed discomfort. Employee went to the plant nurse on Monday, the 27th, to report the issue.

Over-exertion in lifting objects

Employee felt discomfort and noted that the soreness may have resulted at work while mixing pigments (lift/empty 50lb pigment bags into the mixing pot) 12 days prior. Employee informed employer of potential job related discomfort on 2/27 and was seen by the facility nurse, then sent to the clinic. Restrictions are being accommodated and employee returns to the clinic on 3/13.

Employee was going to the east side of the building in the alley next to 12 conveyor to pick up the 936 loader. Employee walked around a puddle of water but there was a tiny bit of wet material about 1/8"" high. Material was slippery so when the employee walked over it they twisted right knee and fall to the ground.

Employee felt a pull in the lower left abdomen after dumping a bin containing waste granule. EE pushed bin on it's side and lifted it from the base in order to dump it into the larger bin below.

On 6/29/21 Employee requested to be sent to the clinic for lower back pain. Employee said EE felt a tightening of lower back muscles soon after arriving home from work. Employee couldn't pinpoint any particular task/incident that could have caused the pain and they only performed normal routine duties (cleaning, sweeping shoveling, bin tender operation).

Employee reported pain/discomfort radiating from the right lower back from bending and lifting pigment bags into the mixing pot.

Employee was hauling waste on the 966 loader with a full bucket and with the bucket up to the stock pile. The right side of the bucket clipped and bent the I beam on the headlap scale. Employee stated: was driving that way instead of around to get more loads out before employee went home. Employee subsequently also complained of neck and back pain on 3/16/2021.

Struck by flying object

Irritated left eye due to foreign object being lodged in it. While shoveling rock the employee got a particle of dust/sand/rock lodged in the left eye. Employee flushed eye and applied eye drops, but was unable to remove particle. Employee taken to Occupational Medicine for first aid.

Material Handler lifted horizontal switch to move railcars. Due to the awkward body position and effort required to flip the switch employee felt a pain/strain in lower abdomen.

Operator opened hood to guzzler & once open the operator was about to step away, hood did not stay put, hood continued to fall & struck operator on head.

On 7/22/2020 Employee reported they had experienced mild discomfort in the lower back the day before while shoveling materials.

Accident type, without injuries

Fire occurred on buffer land. Cause was determined to be mylar balloon on electrical line. Fire department responded. Approximately 10 acres burned. There was no property damage and no injury. Plant personnel had completed shift and left property several hours before occurrence. Fire was reported to MSHA on 6/13/2020 at about 8:55 pm

Employee complained of chest burning after opening bottom doors on railcars. The railcar company delivered rail cars on site that were not completely empty. Rail car contained limestone.

Emp was training another emp on the guzzler. Activating the hydraulics on the equipment causes a burst of air toward the ground. There was dust on the ground below the guzzler which appears to have been stirred up and some got into the employee's eye.

EE was performing maintenance work on aerial lift. EE was working in a tight area underneath vehicle using a hammer to tighten hydraulic hoses. Hammer slipped & slid about 18"" down the hose, hitting EE in mouth. EE reported damage to crown on tooth. EE was sent to dentist for evaluation & damage to tooth reported. Employer did not receive knowledge of damage to tooth until 3/5/19.

Employees were attempting to open a bottom rail car door that was jammed. The injured employee was using a bar to open the door and the door unjammed suddenly as the employee was pushing down on the bar. The bar made contact with the track, pinning the employee's left pinkie finger between the track and the bar. The employee experienced a fractured finger tip and required stitches.

Employee alleges that he felt pain in the elbow as a result of working in Shipping, while helping out in that department the week before Christmas 2013 . When it did not go away and got worse he reported it 2/17/2014.

Mechanic was attempting to track 32 waste elevator by removing a shim on the east tail pulley bearing. He had install a wedge to separate the bearing from the base frame. As he attempted to remove the shim the wedge slipped and the bearing smashed his finger.

Employee was struck on the head by a counterweight the weighed approx. 14 pounds. The counterweight broke and dropped approx. 12 feet striking the employee. The employee was wearing his hard hat but it knocked him down. He never lost consciousness and was taken to the hospital via ambulance.

Employee was hit in the mouth by a drop light while he was on the top lining railcars, resulting in a chipped tooth and lip abrasion. He said he was bending over to wash out the railcar the hook on the drop light caught onto his lanyard. As he went further down the drop light became unhooked from his lanyard and then swung around hitting him in his mouth.

Employee was walking down to the basement and he missed the last step and he lost his balance. When he lost his balance he fell, fracturing his lower leg.

Employee was opening rail cars and when he went to open a rail car, he started to open the car and felt a pull in his back. Normally the cars are empty when the employees go to open the doors. The employee asked another employee to look inside from the top to see if it was jammed and that is when it was found that the rail car still had approx. 30 Ton of rock inside.

Employee stated that while loading a bulk clay bag onto the bulk clay skid, he felt a strain in his shoulder. He placed the skid on top of the new bag, and was securing straps to skid. Employee felt he had enough clearance to reach in and secure the third strap. He used his right hand crossing over his body and reaching the strap to pull it, causing a strain in his shoulder.

When employees lifted the screen to the rotex opening one employee said ""felt a pop like cracking your knuckles in mid-back area"". Employee continued working and finished screen change. FTE was cleaning scrubbers. He climbed the 3' ladder; hit his head on the overhead pipes. He said he was ok. FTE was asked if he wanted to go to clinic, he wanted to wait until Monday. On Sept.14 EHS Manager did a follow up, FTE feeling better. 10/2 FTE wanted to go to clinic, paperwork received on 10/5, with prescription.

Miner was attempting to remove the lid/cap on the #7 mandrel. When he started to turn the cap his hand slipped off of where he was holding/pushing and he hit his hand on the left side of the pinky knuckle on the steel cap causing his hand to bruise.

EE iced & elevated the knee, stated that swelling had gone down.

As the Miner was preparing to load a truck he went outside of his office to instruct the driver and explain he would be loaded next. As he returned to his office he was rushing to start loading the truck. He fell and twisted his ankle. First Aid was applied, ice on and off for 40 minutes, he was than taken to the clinic where x-rays were taken and his ankle proved to be broken.

MATERIAL HANDLER SAID THAT THE GROUND WAS SOFT AND GAVE WAY AS HE STEPPED FORWARD. MATERIAL HANDLER REPORTED THAT HIS ANKLE WAS SORE. HE WAS TAKEN TO THE CLINIC FOR EVALUATION. MATERIAL HANDLER WAS SEEN BY PHYSICIAN AND PRESCRIBED 550MG NAPROXEN.

Miner was re-welding cracks on the rotex frames and at some point a piece of the metel got in his eye. He was not aware that this happened until he arrived at home and his eye started to bother him. He went to the ER and they took a small piece of metal out of his eye. He was wearing the proper PPE during the welding.

While making a batch the Mixer Operator went to open the slide on the #14 Conveyor and cut his left hand on the conveyor cover. The operator was not wearing gloves. There was a shop work order written to install rubber around the edge to prevent future incidents, this was completed that same night. The operator received 2 stitches

Mixer Operator was handling 50 Lbs of acti-min clay. He went grab another bag and felt something pull in his right shoulder. When he looked closer at the bag it appeared to be bigger than the others. The operator weighed the bag and discovered it had been over filled and weighed 74.6 pounds instead of the marked 50 pounds.

Mixing on side 2, slurry splashed up and got into the eye of the mixer operator.

While working in bagging, operator cut the banding on a new pallet of minibags. He tried to rip the banding out from underneath the pallet. The banding splice hung up on the pallet, and caused injury to the operators shoulder. Operator is going to be taken to the clinic for examination.

Employees were installing new ducts for transfers points, one of the ducts got jammed ee kicked the duct to free it up, thus hitting him in the upper body. this was no damaged done to equipment.

While changing a light bulb the light bulb broke, causing a laceration on the index finger of the left hand. The employee saw the nurse, she sent him to the Urgent Care and he received 3 stitches.

Employee started at 12 noon on the day of the accident. He was moving a piece of a frame to the rotex. He lifted it and that is when he felt the pain in his wrist. He waited to report it until 9/12 at 6:00 PM. It was at that time he was sent to the clinic.

Employee was drilling a plate for a pump stand in the maintenance fab area and some how he got his right hand with a glove on it tangled in the rotating drill bit. Which twisted his hand and gave him a few small cuts and a dislocated ring finger. Took him to the medical center they gave him first aid reset finger.

Employee had just sent a sample and was coming across 11 conveyor on the crossover steps. On the last step coming down his foot slipped off the step and with his left arm he caught himself from falling by grabbing the rail with his left hand. His right spun him around which hurt his left shoulder.

Employee went to the nurse on June 6, 2007 and stated two weeks prior his right knee started to hurt, due to driving the 936 and 988 loaders. He states that the rough drive and having to push hard on the pedal is making his knee hurt. Nurse treated and will have to due follow up.

Process engineer tripped and fell on walkway edge. The edge is located between the warehouse and the west side walkway of #1 kiln. Engineer recieved a scraped knee. Engineer cleaned his knee and stated he was ok. On 12/08/2006 the knee was swollen. We sent him to the doctor and he recieved prescription medicine

Supervisor stated the electrician was touching a partially insulated screwdriver to a screw, and it looked like the screw driver touched the panel which created a metal to metal contact. There was an arching, flash and the electrician suffered burns to the right side of his face, right arm and right hand and complained of photosensitivity.

EE walking on a flight of 4 steps, when he was on the second rung from the bottom he slipped and fell backwards; his right calf was visibly swollen, he complained of pain in the right calf after the fall; denied other injury; no other injury found by MD.

Msha report filed 4-13-06 x2 problem w/ on-line filing, call MSHA Denver 4-13-06. Report not in system; call msha: 11-17-06. Employee reported burn to supervisor on 4-11-06:""a hot piece of material went down my right sleeve when welding or burning. I washed burns and did nto pay attention to the burns; now elbow sensitive to touch, looks like infection in skin over elbow.

There was no accident, employee stated he felt eye irritation, blinked and it went away, eye irritation came back driving home and then it returned again driving back to work. EE had a corneal foreign body right eye that was removed.

at 12:30 a.m., was releasing hand break, stepped back/ down onto uneven ground -twisted left ankle; kept working until 4:30 a.m., went up ramp, flipped gait and turned to close the slide, ankle gave out and felt her left lateral malleolus contact the cement, she fell forward and caught herself, and reported both incidents at that time.

EE changing the drive pulley for #7 crusher feed belt using pulley, the shaft/mounting block became missaligned and he attempted to realign (co-workers helping). The pulley adjusted from the opposite side employee, causing the pulley to drop approximately 1"", crushing the left index fingertip; wound sutured, nail sutured into place, tuft fracture fingertip.

EE swatted a hornet that was near his right arm, when EE struck the hornet, the hornet stung the employee on the right forearm. EE right arm swelled, became firm, red, and he complained of numbness and tingling, was treated x 1, seen in f/u and discharged cured.

BIN TENDER HAD MOVED CHUTE FROM TRIPPER ACROSS WALKWAY TO FILL BIN ON THE OPPOSITE SIDE AT BIN 4. DURING THIS PROCESS HE NEEDED TO STEP OVER THE TANK TO GET TO THE OTHER TRIPPER. HE SLIPPED ON GRANULE ON THE EDGE OF THE TANK TOP. AS HE WENT DOWN HE CAUGHT HIMSELF WITH HIS LEFT HAND, WEIGHT GOING ONTO THE FOURTH AND FIFTH FINGERS THAT WERE BENT UNDER.

EE was on top driller filling tank with foam solution. Some of the solution spilled over making surface slippery. When he stepped off to transfer to own truck, he slipped and fell forward on to outstretched hand. Sustained Fx of right wrist.
