Caught in, under or between a moving and a stationary object

Miner was installing V-belts on a motor pulley and EE's fingers got pinched between the sheave edge and the V-belt.

Miner was hammering in a choker plate for material feed, missed the plate and struck EE's R-index finger. Miner was wearing leather gloves at the time and sustained a fracture to EE's fingertip.

Miner was operating a 777 haul truck and hit a pothole in the haul road causing an injury to lower back. Truck was empty at the time.

Miner has had an audiometric standard threshold shift in R-ear. Miner states they have been exposed to work noise for 41-years and has been exposed to gunfire in the past. Miner wears earplugs.

Cut left index finger during maintenance. Hand was placed on gravity take up guide pole, 966 loader bucket that was supporting the counter-weight bled down hydraulic pressure causing the counter-weight to lower and finger got pinched by the guide pole sleeves.

Over-exertion in lifting objects

Was replacing a return roller on a conveyor, used a wooden board for leverage to lift belt and hurt EE's shoulder. Continued working regular duty but found out EE will need surgery.

Miner was a hammering chisel wedge to separated a liner when the wedge released striking the miners left side of the nose. This caused a laceration requiring stitches.

Miner was brushing sand from a broken pipe. The miner cut finger by not using SLAM or using proper PPE. Due to accumulation of companies safety points program. The miner reached 12 points in a rolling 12 months.

Jaw crusher plugged filling the surrounding area with material, EE & 2 workers removed the material they used a crane & sling operation because some of the damaged deck could be a trip hazard they removed 2 sections ee was standing with his back to 1 hole he then turned & lost his balance fell approx. 12' onto his side, ee had a bad cut over his right ear

EE was cleaning up the material that had spilled around the sand screws discharge collecting box with a shovel and in performing this task he slipped and lost his footing and fell thus cutting his right forearm with a 2.5 cm cut. He wasn't sure what actually cut his arm, he was released to come back to work from the clinic but he decided he needed a day of rest.

EE TURNED ON THE 7' BREAKER & IT TRIPPED THE MAIN BREAK SO HE SHUT OFF THE 7 ' BREAKER & RESET THE MAIN, THEN PROCEEDED BACK TO LOOK AT THE 7' STARTER & EVERYTHING LOOK OK SO HE WENT TO THE N EW BREAKER BOX, LIFTED LID TO LOOK INSIDE & AT THE SAME TIME A COWORKER TURNED THE MAIN BACK ON& THE BOX HE WAS LOOKING AT EXPLOSED
