While removing the use liner from the head of an Omni Cone Crusher, the employee was lifting with a crane and hitting the liner with a 3 lb hammer to break it loose. The hammer struck the employee's right middle finger.
While changing screens on the wash screen bottom deck, the employee felt a low back strain.
The employee began to notice pain in his low back after using a 16 lb hammer to install a head bolt of a crusher. As the pain began to intensify the employee requested medical intervention for the low back pain.

Two employees had tied a rope around a large piece of steel (14' x 3"" x 1""; 120 lbs) to lift the steel out of a jaw feeder. While pulling on the rope to lift the steel one of the employees felt a pop in his left side along with severe pain.
The employee was pushing Jaw Toggle Plate over to replace positioning block with a hydraulic portable jack. The toggle slid to the left allowing the metal shim block to drop, hitting the employee's finger.

Hopper beneath S3-L had filled with rock dust when the shaker's bottom conveyor failed apparently due to the failure of the convyor's v-belts. The employee entered the hopper via the bottom or tail end to shovel out the plugged hopper. The material shifted inside causing the conveyor to move pinching the employee between spilled material and a hopper cross-brace.
The employee began to experience pain in his left hip due to an unknown reason. No specific event occurred.
The employee was removing the outside bolts of a tailpulley take-up unit. The inside bolt was missing from the unit which allowed the top of the take-up unit to come loose, striking the employee on the left side of his face.
The employee was cleaning a screen deck with metal weight. The weight slipped from his hand and pinched the right thumb on the screen frame.
While loading 11"" wide plate to a service truck the employee lost his footing in the west ground and fell. The plate pinched the employee's hand to the ground.

AFTER WORKING HIS REGULAR SHIFT, THE INJURED EMPLOYEE CLEANED UP AT HOME AND FOLLOWING A SHOWER,NOTICED PAIN IN HIS TORSO. HE SOUGHT MEDICAL ATTENTION THAT EVENING AND THE PHYSICIAN DISCOVERED THAT HE HAD SUFFERED A BROKEN RIB. AT PRESENT, IT IS UNKNOWN HOW THIS INJURY OCCURRED, HOWEVER HE BELIEVES HE MAY HAVE SLIPPED UPON ENTERING AND/OR EXITING THE FRONT END LOADER.
DRIVING ROCK TRUCK, STRUCK ROCK IN THE ROAD CAUSING A JARRING TO THE NECK.

WEIGHT OF MATERIAL ON BELT CAUSED CONVEYOR TO REVERSE. HEAD PULLEY & GEARBOX ROTATED IN UNISON, THE TENSION ON ""COME-ALONG"" INCREASED & GEARBOX ANGLE IN WHICH IT WAS ATTACHED CHANGED CAUSING HOOK OF COME-ALONG TO DISENGAGE. HOOK FLEW BACK AND STRUCK EMPLOYEE IN THE FACE.
EMPLOYEE WAS DIGGING DIRT AND GRAVEL FROM AROUND THE BASE OF OMNI #2 CONE CRUSHER. MUSCLE SPASMS IN THE BACK OF THE RIGHT SHOULDER CREATED PAIN FOR THE EMPLOYEE.
