A mine employee was walking on uneven ground and slipped on some gravel causing EE to fall. EE received a laceration on EE's right knee which required 3 sutures. EE was released back to duty with lifting restrictions

The employee was loading a pallet with a forklift onto a flat bed trailer at the company office which is not on the mine site. EE was parking the forklift and reached to engage the emergency brake and felt a pop in EE's wrist.

Individual was helping install hoist bed pins on a 777 haul truck. EE bent over to get a tool and when EE raised up EE hit the back of head on the fuel tank guard causing EE to lose consciousness for a short time.

EE was using a sledge hammer to drive a shaft out of a fan bearing when EE's hand hit the fan housing causing a cut to the left pointer finger. 5 stitches were used to close the cut.
Employee was assisting in laying dripline on side slope of leach pad when they stepped on an unstable rock resulting in a sprain to their right ankle and knee.
Employee was cleaning up a cement spill using a shovel. This was a first aid injury until EE went for surgery on 9/23/22. First day lost time was 9/27/2022.

EMPLOYEE WAS STEPPING OFF A UNILOADER. HE HAD PLACED HIS LEFT FOOT ON THE STEP AND WHILE BRINGING HIS RIGHT FOOT FORWARD THE TOE OF HIS BOOT CAUGHT ON THE FRAME CAUSING HIM TO FALL FORWARD. HE ATTEMPTED TO JUMP CLEAR, BUT STRUCK HIS RIGHT KNEE ON THE LOADER BUCKET.
THE EE WAS CHANGING THE BUCKET ON A UNI-LOADER. THE BUCKET HAD TO BE RAISED SLIGHTLY AND ROTATED FORWARD BECAUSE THE RELEASE PINS WERE FROZEN. THE EE DID NOT NOTICE THE LEFT RELEASE PIN WASN'T LOCKED DUE TO THE AREA BEING CAKED WITH MUD. AS HE FOT THE RIGHT RELEASE PIN FREE THE BUCKET DROPPED AND STRUCK HIM ON THE RIGHT FOOT.
EMPLOYEE WAS USING AN IMPPACT WRENCH TO TIGHTEN THE SECURING BOLT OF A SUPER NUT. THE SUPER NUT IS USED TO HOLD THE LARGE BOLTS FOR THE SPIDER ARM OF THE PRIMARY CRUSHER. AS HE TIGHTENED THE S ECURING BOLTS, THE LARGE BOLT BROKE LOOSE FROM HEAD OF THE BOLT AND THE BOLT FELL STRAIGHT DOWN CAUSING THE EMPLOYEE'S HAND TO BE CAUGHT BETWEENTHE IMPACT WRENCH AND A METAL LODGE ON THE CRUS
EMPLOYEE WAS USING A PRY BAR TO CLEAR A BUILD UP OF MATERIAL FROM A CLAM SHELL CHUTE AND AFTER CLEARING THE MATERIAL A SECOND CHUNK OF MATERIAL FELL CAUSING THE PRY BAR TO BE PULLED UPWARD AND HE STRAINED HIS RIGHT SHOULDER.
