After performing the annual hearing exams, the employee was designated as having threshold shift that required MSHA notification.

Third party aggregate hauler was exiting vehicle after being weighed, when the hauler slipped on the step of the hauler's truck injuring right knee. The individual's employer transported the hauler to the hospital where the hauler received surgery on knee.

Mechanic was dismounting wheel loader, as he stepped down, he stepped on a rock. This caused his right ankle to twist causing the injury.
