EE WAS REMOVING A SPROCKET FROM THE MOTOR SHAFT OF THE DYNA-TEC CONVEYOR. EE WAS UNDER THE SPROCKET AND WAS WEARING HIS SAFETY GLASSES, WHEN SOMETHING GOT INTO HIS LEFT EYE.

EMPLOYEE WAS UNSCREWING A SODIUM SILICATE PIPE WHEN THE LIQUID HIT THE EMPLOYEE IN THE FACE, KNOCKING OFF HIS GLASSES THUS PERMITTING SODIUM SILICATE TO ENTER HIS EYE.
