Nielsons Inc · Struck by falling object

TWO EMPLOYEES WERE USING AN EXTENSION LADDER AND HAD FAILED TO INSURE THAT RUNG LATCH HAD PROPERLY ENGAGED. ONE ASCENDED LADDER WHILE THE OTHER MAN (INJURED) HELD BASE OF LADDER. LATCHES FAILE D ALLOWING TOP SECTION TO SLIDE DOWN LACERATING HIS THUMB.
