Employee was loading a training roller into the back of a mantrip. When roller got stuck IE lifted roller heard and felt a pop in their right arm

While checking the tightness of the belt on the skid steer, IE had ahold of the belt when other employee tried starting the skid steer, it pulled their right index finger into the pully and pinched it between the pully and belt.

Employee was pulling cutting motor on longwall with chain and come-along and the hook accidentally broke causing the hook to accidentally strike employee in the shoulder causing injury

IE Grabbed a rotating drill steel causing their glove to get wrapped around drill steel, jamming their fingers into their wrist

Placed ladder on uneven surface, climbed ladder with a 3"" pipe on shoulder, ladder shifted causing employee to fall striking ladder and ground.

IE stated that IE was trying to unplug a chute in the prep plant screen building using a scaling bar. IE was attempting to bust up material from the chute working upwards. The debris came loose falling below. The weight of the material sloughed down the chute catching the bar. The bar springboarded upward striking the IE on the left jaw, causing a laceration requiring 13 sutures.

Employee states that employee was dragging ventilation tubing and tripped landing on knee. Employee says that the knee landed on a rock or chunk of coal.

IE was installing a roof bolt. While holding the bolt in the drilled hole, the bolt inadvertently moved, bending the bolt causing EE's hand to get caught between the drill chuck and the drill guide. This caused a laceration that required sutures.

Employee's left heel was caught between the relay bar clevis pin and the headgate debris plate, causing an avulsion to the heel requiring surgery.

IE went to the tailgate side of shear, opened the cabinet door to run the tailgate cowl off the valve bank While operating the cowl, the cabinet door closed striking their right forearm. IE was diagnosed with a hairline fracture. They returned to work their next shift

IE installed a washdown pump in the wash plant. IE stated they had opened the valve for the incoming water to the pump when impeller housing broke apart. The 4"" inlet pipe came towards the employee striking the employee in the knee. On the morning of 10/6/2024, Employee began missing work on 11/12/2024.

Employee was unthreading a water hose from the wet dust truck while the hose was stretched. When the hose was undone the water hose sprung back striking the employee in the cheekbone causing a contusion just under the right eye. After further evaluation from an ophthalmology specialist, IE was diagnosed with a closed fracture of the right orbital floor on 7/9/2024.

Two bolter operators had finished bolting the mine roof, backed the roof bolter up and started to hang a section of exhaust tubing. A large piece of rock fell out from the rib striking one employee on the leg causing a deep tissue contusion. IE was unable to return to EE's normal work duties. EE was placed on restricted duty restrictions due to weight bearing on the calf muscle.

IE was moving steel pipe on a metal pallet that needed to be rearranged to a smaller length pipe. The stack gave way. Subsequently, the IE's hand was caught in a pinch point resulting in their left thumb being injured. The employee was wearing impact gloves at the time of the incident and was taken to the Hospital for further evaluation.

IE had right hand placed on top of the miner, when a piece of cap rock came down from the top and stuck the IE on the right hand, causing a laceration and fractured bone.

IE was working on a belt splice. As EE was pushing on the end of the belt, EE started to slip. EE grabbed the belt chain to prevent a fall. EE felt arm pop.

Accident type, without injuries

A non-injury accident occurred. An unplanned roof fall occurred at the 6 East belt 26+01 crosscut, #1 entry intersection. A breaker row of timbers were set inby and outby the roof fall in # 1 entry. A breaker row was set in the intersection of the fall on the # 2 entry side of crosscut 26+01. A rehabilitation plan was submitted to MSHA on 08/05/2023 and approved.

While bolting in 3 entry on 406 bolter the bolter helper was setting the ATRS when the left side bolter operator boomed in and smashed the helper's finger between the frame over the levers and the bolter boom tray. Left middle finger is the injury.

IE was bolting on the right hand station. IE was bolting in the roof on a corner at an angle. IE had the drill box all the way up. On the way back down IE had the left foot below the drill box. Once IE got the drill box toward the bottom IE smashed the foot. IE has a broken left, big toe. IE is in a boot and on crutches. At this point IE is seeking orthopedic care.

Crew was loading a techseal bag (4000lb) on to the teachseal platform. Two of the upper straps broke. The crew was repositioning to use the lower straps. A strap pinched the right pinky finger of the IE, causing a crushing laceration to the finger. IE received 5 stitches.

Grating had been removed from a floor drain. The IE could not see the drain that was full of water and had stepped into it causing a sprain/strain to their left knee.

A fitting failed in a valve block causing hose to blow off.

Accident type, without injuries

Responsible Person (RP) was conducting a pre-shift examination. RP was traveling into 7E - 2E- XC 39+75. When the RP noticed a substantial roof fall above anchorage. RP immediately called management. Management immediately notified MSHA through the 800# and called the local MSHA field office.

Fall to the walkway or working surface

Employee was walking on the second floor of the train load out to trace out a strip of heat tape for the load out waterline. The employee tripped over a used motor and pump that was out of service sitting on the floor. The employee landed on elbows causing pain to left elbow and shoulder.

Injured Employee (I.E.) was tramming a 12CM12 continuous miner (company #317) into the face and positioned on the right side. I.E. was found pinned between the continuous miner tail and the rib. Current known injuries: 7 broken ribs (right) 4 broken ribs (left) and a collapsed lung. I.E. is stable.

EE was walking through Surface Weld Shop when they slipped in a puddle, catching themselves with right arm. EE did not experience any pain as a result of the fall until next morning when they began to notice pain in shoulder. EE was referred to an Orthopedic and continued to work until 08-30-19 when they had surgery to repair a torn rotator cuff.

Employee rolled EE's ankle on a piece of coal but finished EE's shift. Sought treatment on 5/8 after walking on it for a week, was released but was referred to orthopedic for closed fracture of fibula, saw ortho on 5/14 and was put in walking boot and taken off work.

IW was assisting in installing a draw bar on the pump monorail. IW was helping hold the drawbar in place as the trolley was pushed into position. IW had hand on the monorail for balance when the trolley wheel pinched IW's finger.

IE was setting up to do an alignment on a motor to pump coupling on 5mn xc 25. as ee was walking around the pump skid ee stepped off the pump skid rail, rolling foot outward. IE sustained a fracture to the 5th metatarsal bone and missed next scheduled shift on 11/14/18 making this reportable at that time.

Employee was diagnosed with bilateral carpal tunnel and had surgery on 11/12/18. Surgeons work related determination was not received by the company until January 7th, 2019. It was at this time that we accepted the claim and are filing the report for the occupational illness.

Fire Boss bumped IE's head on a pipe; later in the shift began to experience vision issues, i.e. spots/floaters. IE had a broken blood vessel in the eye which in turn caused a torn retina. It appeared to be a result of the bump to IE's head. IE had laser surgery to repair the retina on 11-2. IE has unrelated existing retina pathology in other eye.

Employee reported ongoing shoulder pain in August, but has no specific event or incident. IE had surgery on 10/25/18 for repair of the bicep tendon, rotator cuff and labrum. Clinical findings were bicep tendonitis, AC joint arthrosis, fraying of the distal supraspinatus tendon, etc. Surgical report received 11/8/18.

IE caught left hand (second and third finger) in fan area of roof bolt machine.

Injured worker was involved with a slip, trip and fall incident in which the IW fell striking EE's head on the mine floor. IW was taken to the emergency room and immediately released to full duty. IW remained working, but continued to experience symptoms related to the injury. IW's first loss time date was on 10/25/2018.

IE was cutting a hole in belt to put rope through to pull belt. EE was cutting right to left. Left hand was on the belt, knife was in the right. The left hand was cut on top of the hand when right hand continued it's direction of travel after knife blade broke. IE was wearing EE's cut-proof gloves at the time of the incident. This was submitted in Oct., but it's not on the DRS.

A roof fall occurred in the #1 entry, 8 East section, at crosscut 31+25 extending approximately 70 feet outby. Approximate dimensions were 70 ft long by 22 feet wide by 6 feet high. The area was bolted with 6 foot long resin encapsulated rebar bolts.

Contract miner was injured while cutting rib mesh away from a bolt. The employee was outby the stageloader on the off-walkway side. Miner was struck in the arm and hand by a piece of rock that had fallen from behind the mesh. The miner received two broken bones in the hand requiring medical treatment.

IE was climbing down from belt structure during the section belt move. IE had both hands on the structure chain & stepped off the structure. The IE contacted 2nd chain & hook caught on the inner seam of pant leg. IE stepped off the structure the hook caught IE. IE transported to the ER for treatment and received sutures to close the wound.

IE was hanging cable with two drill steels. One came apart IE in mouth chipping one tooth and knocking one out.

IE was bolting in face. As IE was drilling IE's drill bit would not make a hold due to angle and was just spinning off of desired mark in a downward direction. As IE did IE's drill wrench came loose out of drill chuck and due to angle of drill steel caused the wrench to be propelled at IE's station, striking IE in the face on lower jaw.

IW was roof bolting. IW was holding the bolt with the plate above hand and just above the stationary portion of the jaw assembly. As the IW fed the bolt into the hole, the bolt caught on a ledge in the hole causing the bolt to bend smashing the IW's right little finger between the bolt and stationary portion of the jaw assembly.

A plant operator was helping lift a piece of HDPE pipe to reconnect the bent sieve circuit. Another employee was lifting the pipe but was having difficulty getting it into place. The IE employee stepped onto the top of the bent sieve bin and at this time, the pipe shifted, knocking the IE off balance causing IE to fall.

IE was installing roll pins in the flights in the feeder. IE was driving them in with a hammer and missed striking IE's left thumb, which was holding the pin with the hammer. The blow caused the thumbnail to be removed from the cuticle and exposed. The IE received sutures.

IE attempted to catch the drill steel with right hand and the drill steel struck 4th digit finger. IE informed supervisor. Once IE arrived home after shift, IE removed first aid gauze and sought medical treatment since laceration would not stop bleeding. The emergency room diagnosed IE with a laceration and fracture to the tip of IE's finger.

Miner checking HG Sprocket area for rib bolt. Upper & lower doors open, upper door put back, not seated. Injured contractor put the lower door in place, reached up & grabbed the pin/anchor tab. Tab slipped off the lug pinching finger between tab and lug. Wound was cleaned & EE refused further trtmnt. On 3/26, we found that EE visited the ER after the incident & received sutures.

Over-exertion in lifting objects

Employee went in to collect seal samples that needed to be tested. EE drove in to 9 East as far as ee could, then lifted the box & carried it to truck. EE didn't seek medical attention until June & was diagnosed with a bilateral inguinal hernia on 6/14/18. The mine was notified of the June 28 diagnosis as work related on July 6.

IE was installing monorail hangers for the pump monorail. IE drilled the hole & installed the bolt. The drill box was raised to extend the second stage. IE had placed hand at the top of the first stage of drill mast frame. As IE lowered the drill box IE's fingers were caught between the second & first stages. IE broke one finger and dislocated another requiring surgery on 8/4.

EE notified supervisor of pain in shoulder in Dec. 2016. EE had noticed pain in shoulder that was bearable prior to that. Had surgery on shoulder in 2011. Notified supervisor of increased shoulder pain after work in the shop but thought it'd go away given some time. IE has been maint. mechanic for 5 yrs, did outby work for 2 yrs & CM production for 6 yrs prior to that.

IE works on the longwall crew presently. Prior work was in the CM section, primarily bull gang work. IE was leaned over, walking through the headgate shields and felt pain in back. EE finished shift but when ee got home that night back tightened up to where ee had difficulty moving.

Taking tilt cylinder pin out from eye where it attaches to head frame. Took bucket off, backed scoop up, had head frame tilted forward. Pin still in the cylinder eye and one side of bracket. Scoop was shut off. Coworker exiting scoop and heard the head frame hit the ground. Coworker went to front of scoop and saw IEs foot was under the head frame. IE had 3 fx'd bones in foot.

EE was walking across floor and stepped on some flocculent that had been spilled and not noticed. Employee fractured a wrist.

IE had shoulder pain. Was taken to the ER. ER speculated that the side to side swinging motion with hands at waist level was what cause the injury.

IE was struck in the mouth with a hook from a ratchet strap, resulting in a broken tooth. The employee did not have any treatment until 3/26/17. That information wasn't received until 4/5 & it wasn't clear whether or not the treatment was for the broken tooth or cleaning required before the dentist would do other work. The cleaning was required due to poor dental hygiene.

EE told safety on May 24th of back, leg and knee pain. On June 2, EE sought medical treatment. Diagnosis unclear with possibility of two separate issues or all back related. EE stated stepped in a hole January 28, felt discomfort but continued work. In May EE's pain was below the knee & up the thigh. MRI & surgical notes show degeneration/torn meniscus. Meniscectomy on 7/11.

EE diagnosed with lateral meniscus tear & other knee pathology in 2014 that will eventually require knee replacement. EE returned as a fire boss in 2015 and in Jan. 2017, slipped doing ee fire boss duties. EE felt pain in knee, MRI revealed the same meniscus tear. IE subsequently had surgery 3/3/17 for meniscectomy. Additional info, including MRI's sent.Det OI 3/10/17

Person was installing roof bolts. EE had put bolt in roof and was using tension gun to tighten bolt. EE put tension on the bolt and they tightened it up and in the process ee had hand on top of roof mat which tighten left hand to roof smashing fingers. EE broke one finger requiring surgery and required sutures in another.

Employee diagnosed with Carpal Tunnel Syndrome and had surgery on 12/26/16.

employee complained of ongoing knee pain. Injured this knee in 2004 and had subsequent degeneration following 2004 surgical intervention. Employee had surgery on December 7,2016 for meniscectomy and chondroplasty of the femoral condyle and patellofemoral joint. Surgeon indicates cumulative trauma.

Struck by falling object

Employees were packing a bottom frame down the ramp of an overcast when the injured employee slipped and dropped the frame on foot. Employee was wearing metatarsal boots and it was determined that the employee fractured metatarsal bones. Received medical treatment on 11/8/16 making this reportable.

Struck against stationary object

Employee notified safety on 12/5/16 that EE bumped knee against a bench in the bath house on 10/31/16 while walking to EE's locker after showering after EE's shift. EE finally saw a doctor in March 2017 and was prescribed PT in July of 2017; EE's first session was July 13, 2017. This has recently been accepted as work related for patellar tendonitis.

Employee notified safety of worsening pain in left shoulder. EE had surgery on 10/25/16 and the following noted in the operative report received on 1/3/17: partial thickness tearing of the anterior distal supraspinatus, glenoid joint anterosuperior synovitis, rotator cuff partial thickness tear, AC joint arthrosis, synovitis and hypertrophy.

Miner was performing pre-shift exam of the NM beltline. At the rock dust transfer tank break, the dust pipe came apart at the elbow. The pipe struck the miner in the forearm resulting in a fracture.

Iron worker was cutting apart an old discharge chute with a cutting torch. When the chute section was cut apart it shifted causing the employee to lose EE's balance and strike EE's left wrist against a sharp piece of metal. EE received a laceration to the top of EE's left wrist and ulna tendon which required surgery to repair.

Employee notified supervisor of pain in shoulder on 7/18 while performing normal maintenance activities. EE requested medical attention and subsequently had surgery on 8/31/16 to repair torn rotator cuff, subacromial impingement syndrome, and long head bicep tenosynovitis. EE's condition has been attributed to mine maintenance activities over the course of EE's career.

IE was installing an 8' bolt, and the bolt began to bend. IE spun the bolt and as IE was rotating the bolt--the short extension snapped at the weld and a piece hit the injured above the right eye. Received 21 stitches in the forehead

Employee notified the company on 7/22/13 about worsening pain in shoulder. Employee came back to management in April 2016 requesting to be seen by a doctor for the shoulder since it was becoming a worsening problem. Employee missed no days until 6/20/16 when EE had surgery for right shoulder. No known event occurred.

Employee reported worsening pain in right shoulder. MRI indicates Labral tear, tendinosis in subscapularis tendon, intralamellar delamination infraspinatus tendon and synovial cyst. Diagnosis was cumulative trauma. EE is on restrictions as of 6/14/16 and awaiting surgery at present time.

Employee notified mgmt of ongoing pain in shoulder that wasn't going away. EE has had pain historically that was treatable with ibuprofen. Employee placed on restrictions awaiting surgery for shoulder repair. MRI reveals torn subscapularis tendon, supraspinatus and infraspinatus tendinopathy, AC joint degeneration and labral fraying. MRI results received on 5/18/16.

While placing a mat on the ATRS, the IE's belt accidentally activated the ""jaws"" lever. EE had right hand on the jaw assembly which closed on EE's finger.

Employee notified safety of ongoing pain in shoulder. EE cannot remember a specific event of injury, but the pain is now bad enough to seek medical attention. EE was placed on restrictions on 6/3/16, had an MRI and will be scheduled for surgery soon. Diagnosis was degenerative disease.

Employee (contractor ID A3234) was starting to drill first hole and got finger caught in the jaws. Employee was holding onto drill steel and lowered left hand/arm and inadvertently contacted the lever to close the jaws. Employee was taken to the ER for medical treatment. The employee received 10 sutures to close the wound.

IE stated that EE was assisting 2 other employees lift a metal side board for a feeder. When placing onto a flatbed of truck, it slipped--smashing finger on bumper of truck

Employee had ongoing shoulder pain and had surgery on 2/22/16 for significant synovitis and disease of the biceps tendon, AC joint degeneration repair, side-to-side rupture of the supraspinatus repair and small focal posterior labral tear. Surgical notes received on 4/7/16 where the injury was determined to be work related from cumulative trauma.

Over-exertion in pulling or pushing objects

EE injured shoulder roof bolting. EE pulled on a roof bolt in the tray and felt pain in shoulder subsequently diagnosed as a dislocation and major tearing of the labrum, which has been diagnosed as degenerative prior to this event allowing the dislocation. EE had been prescribed PT in March. Company notified on 4/20/16 of first appt being the 1st week of April.

Pain in knee after bolting in 01/2016. IW declined treatment. 01/2017 the IW had surgery for lateral meniscus repair and debridement, patellar tendon debridement and repair of chronic patellar tendonitis. Diagnosis was a longitudinal tear of the lateral meniscus and chronic Patellar tendonitis, and early parameniscal cyst formation consistent with chronic, degenerative conditions.

Employees were opening the metal door on a chain bucket, when the injured got latch unhooked EE headed to go help the other person unhook their latch. The door was on the down grade side of the cross cut and the employee was standing in front of it when it opened unexpectedly falling on EE's foot.

Over-exertion (Not Elsewhere Classified)

Employee presented ongoing shoulder pain and limited use of left arm growing increasingly worse.No event noted. MRI shows degenerated superior labrum, mild tendinopathy, and glenoid fossa chondramalacia, and mild AC joint arthrosis. Employee has done primarily bull gang/utility work. This has been identified as an illness. MRI results were received on February 4th.

Employee experienced pain in shoulder. Has had previous shoulder injuries, and subsequently diagnosed with a SLAP tear, and biceps tendonopathy. Injury is attributed to consistent heavy overhead lifting and other overhead work meeting the requirements of occupational illness. This injury was claimed as work related on January 22 with the receipt of the medical notes.

Employee was walking to perform pre-shift on his mantrip. He slipped on the metal grating causing him to fall. He received a laceration to his knee as a result of the fall, which required sutures to close.

Employee informed us of increased pain since closing a valve underground but continued to work until surgery on 2/8/16. MRI was performed and subsequent surgery to repair tendons and ""clean out"" shoulder. Diagnosis was tendon tearing, bursitis, and mild degenerative arthropathy. Medical notes received on 4/7/16 and determined this was work related.

Emp was lifting dust line with left forearm when he felt pain. Employee went to the doctor on 9/8/15 and was diagnosed with tenosynovitis of the left wrist. He was placed on restriction and given a brace for a week.

Employees were moving cable. The cable fell from the roof and struck the injured employee in the forearm, resulting in a closed fracture of the ulna. Employee received medical treatment (a cast) on 8/18/15.

Employee notified safety of long term shoulder pain on 1/14/15. He was diagnosed with degenerative shoulder pathology and had surgery on 8/11/15. Surgical notes received on 10/8/15 noting AC joint arthritis, partial thickness articular surface tearing of the subscapularis tendon and diffuse labral tearing. All conditions of cumulative trauma from LW maintenance/utility activities.

Employee was closing a manual door. The material used to seal the ventilation device would not roll over to let the door to close. The employee was kicking it and pushing the door closed when it came off the hinges and fell on him. The company found out on 9/24/15 that the employee contacted the Dr. to request physical therapy on 9/8/15, which he was prescribed.

EI was wearing safety glasses bolting when the jaws rattled shut. IE opened jaws and got hit in the face and eyes with hyd. oil. His eyes were flushed UG and on the Surface. IE's condition worsened after returning to work and he was taken to see the optometrist. He received minor corneal abrasions and swelling requiring prescription drops to treat the condition.

EE was connecting an extension steel to the starter steel for drilling a bolt hole. He was feeding his steel up with the feed action of the drill box and at the same time the off side bolter was lowering the drill head off his bolt resulting in pinching and lacerating the flesh between his thumb and index finger. He was transported and received five sutures.

Employee was walking up the stairs to the control room and tripped on the 3rd stair, landing on his left knee. The impact caused the skin to rip just below the knee cap. The wound required sutures to close.

She eventually had an MRI taken of the shoulder, identifying tendonitis in the supraspinatus and infraspinatus tendons as well as arthropathy (arthritis) in the AC joint. Approx.6:00 pm 12/30/14 employee was disassembling a vulcanizer to set it up on a conveyor belt where a repair had been made. Splice needed to be heated to complete process. Employee removed his gloved for approx.5 min. to loosen bolts. Weather conditions were extremely cold (sub zero). On 1/1/15 employee went to E/R in UT due to pain in hands/fingers. Diagnosis - Frostbite.

After approximately an hour of bolting, the chuck collar beneath the wrench on the bolter separated and one bolt struck the I.E in the right side of the mouth. When injured, he didn't get the crib raised soon enough and pinched his finger between the crib & conveyor stop. Employee was wearing gloves and holding the crib by the ends. Employee fractured his L middle finger requiring medical treatment.

Employee was trying to place an end cap on vent tube. He was face to rib and tube (do to closeness of last row of support) he placed cap on end of tube, it stayed on for a moment then fell, Employee dropped his head to avoid being struck in face but not in time it struck the end of his nose.

The Versa trac was broken down in the set up room The IE and fellow EE were preparing to install belt on the waiting shield haulers. The IE guided the truck back toward the shield hauler. The versa trac came back into service so the shield hauler warned IE that they were going back up and pull into setup to install belt. IE ended up being pinned between shield hauler and truck.

Employee states he was in the process of pulling the bit sleeve off a miner. The sleeve was tight on the miner, so employee used the end of a chisel to shim the ram, in order to push the sleeve out. As he removed his hand from the chisel, the chisel shot out and punctured employee's right hand.

He was on light duty for two days and returned to full duty on 5/18/2011.

EE was cleaning up tools, He was packing a 8"" Grinder he placed on a load bed the cord on the grinder wrapped around his tool belt pulling the grinder down. It fell and landed on his left hand pinky finger. Partial amputation.

Since we are unable to determine how much, if any, of the cervical issues are work and non-work related, medical care on 3/31/2011 relative to his neck will be handled as an OI claim.

Employee states he was assisting in installing a concussion door. As he was holding the door in place, it was hit with a blast of air, knocking him backwards and causing him to fall. He received a laceration on his head requiring six stitches and a dislocated finger on his right hand. Employee received medical care and was released for work.

The initial cave behind the shields after a longwall move has been completed, is expected after mining the first 100-feet. The initial cave for the 17-left Longwall occurred on December 3, 2010 at 5:53am. MSHA was contacted immediately, which resulted in MSHA placing a 103-J order on the mine until ventilation was restored in 17-Left longwall.

As the mantrip started to climb the fault, the operator immediately noticed a scoop pulling a trailer which was hidden by the grade/top. Operator attempted to brake but was not able to stop. The mantrip crashed into the trailer being pulled by the scoop.

The 7MN recovery crew led by a Twentymile employee was heading in by on 3MN 5 entry at tne bottom of the fault (starting to climb). The employee was cutting on the longwall when he stepped off a shield causing him to stumble striking his left side on a base lift. He was taken to the ER where he was then diagnosed with a fractured rib. He showed up for his next scheduled shifts that weekend. The roof mesh failed and the immediate roof fell about 4 feet high between the can cribs and the barrier pillar for a distance of about 20 feet. Passage was possible but not advisable between the can cribs and the solid mining block.

Employee states while checking a hot roller the shirt sleeve on his left arm got caught by the roller, which resulted in his left arm being pulled into the roller. He sustained two fractures to his left arm which required surgery. Also his shoulder and face were burned while they were up against the roller until the arm was disengaged.

The employee states he was installing roof bolts and fingers got caught between steele and mast. His ring finger and middle finger were fractured.

Fall from ladders

Employee states he was cutting mesh down on a ladder to form a seal. Rib came down, employee jumped off of the ladder onto kennedy panels onto left ankle. The injuried employee was taken by ambulance to the ER where it was determined that he had a serious sprain to his left ankle.

Employee states he was getting out of a scoop and when he went to get back in his left knee popped. He went off work on 05/06/2010 for knee surgery.

EE was hauling Kennedy rails in the truck and the load shifted. EE went to move the rails when they shifted again and cut his left arm. Wound was treated at E.R. location and EE RTW the same day.

Employee slipped on wet/icy roadway and put their right hand down to catch themselves, jamming their wrist. Employee was diagnosed with right elbow lateral epicondylitis (right wrist sprain). His work restrictions made this event reportable.

He and another employee were dragging & pulling a mini-conveyor from the belt line to a truck, employee felt strain in groin left side. They received assistance from 4 others to finish moving conveyor and load in truck.

Exiting shower, post shift, slipped and fell and hit L knee on bench, causing laceration.

Employee states he was helping to take a staple out of a Longwall shield when the staple started to move, striking the employee just above the left eye. He received 13 sutures to close the wound.

EE was riding in last car of man-trip...hit pothole ... bounced, hit head ...slight concussion.

Employee states he was carrying a top roller in the beltline when he set down the roller he knocked over a spreader bar causing a rock to fall striking him on the left ear. He received 10 stitches and was released to full duty that same day

Fall from machine

Employee states he was getting into a uniloader he lost his footing and his hand slipped off the handle. He fell backwards landing on his left shoulder and hitting his head on top of structure skid.

EE injured his back on 06/09/1998: he had surgeries 2001 and 2002. There has been no new injury but he has experienced intermittent, chronic discomfort since that time. On 10/28/09 he requested medical care due to his worsening condition. He went in for surgery on 03/01/2010.

Employee states he was in the borehole tank room and a pipe on the rock dust tank came loose, filling the air with rock dust. As he exited the room to get out of the rock dust, he walked into the vent air stream, which pushed him against the rib and bruised his left shoulder. Employee was placed on light duty.

Employee states he was walking in the mine, slipped, fell and landed on his right knee. Employee has not missed any time from work; however due to ongoing discomfort with his knee he was placed on light duty 11/02/2009.

An unplanned roof fall occurred on 2 Main North, #5 Entry at Crosscut 28. The fall was apparently the result of deterioration over time allowing the roof to fall above anchorage. The fall is approximately 35 feet by 30 feet by 7 feet high.
