Western Slope Materials, LLC · Fall onto or against objects

Truck driver was filling water truck from trailer mounted pump at irrigation ditch. Tripped on water hose hitting his forehead and right elbow on concrete head wall. Was given first aid on site and taken to hospital where he received 4 stitches on nose.

EE WAS LEVELING SCREEN DECK WHEN HANDLE SLIPPED OUT OF JACK CAUSING EE TO FALL BACKWARDS STRIKING HIS HEAD ON A CONCRETE BLOCK. EE WAS USING THE WRONG SIDE OF JACK, NOT PAYING ATTENTION TO HIS SURROUNDINGS.
