Employee (Mechanic) got cut on their left middle finger while inspecting the screw conveyor after locking it out. Employee inserted hand through sampling port after removing sampling port cover at the bottom of the screw conveyor to diagnose the issue. EE's finger got cut by screw flight.

Over-exertion in lifting objects

Directly supervised contractor was handling warehouse items (chain - 35 lb, carpet 2.5 lb) when EE complained about back pain. Next work shift individual sought medical attention and prescribed medication and a temporary job restriction.

Employee was bent over adjusting paper bags at granular packer (No lifting involved) on a conveyor belt (18 inch off the ground) and experienced pain in lower back and weakness in legs. Employee received treatment at regional medical facility.

Struck by falling object

Employee and partner were lifting the shroud off the ACM Mill when the chain slipped on the rigging assembly causing the 100-150 lbs. (45-68 Kg) shroud to drop 6"" (15 cm) and come down on employee's gloved left hand bruising the little and ring fingers (X-ray confirmed no fracture).

Bottom tread of employee's left safety boot got caught on grating of an inclined walkway. Employee rolled their right ankle on the next step, suffering an ankle sprain and hairline fracture. Resulted in Restricted Work case for facility. Employee was wearing worn safety boots and the boots were not laced tightly above the ankle.

Employee placed foot inside pallet while stacking bags. Coworker lowered the pallet lift, catching the employee's foot. Employee bruised foot and was prescribed ice and NSAID's. Resulted in LTA for facility. Employee returned to work on 5/16/23 with full medical clearance.

Contract operator stepped into an opening under a pallet lift and twisted their ankle. First aid was immediately provided. Employee visited a local clinic and was diagnosed with an ankle sprain. Employee returned to work with modified job duties. Protective toe plate had been removed under the pallet lift, creating the hazard.

Fall from machine

Operator was entering an excavator when EE's foot slipped from the step and caused EE to fall backward. EE attempted to ease EE's fall by landed on EE's left arm which caused a fracture.

An employee was walking down the incline ramp to retrieve tools in cart and experienced sharp pain in left knee. Medical was contacted and employee seen by contract doctor. After going to see medical provider, employee returned to duty. Restricted duty was not relayed to management at time of return.

Operator attempted to pull 50lb bag from top of completed stacked pallet. Bag fell from top jerking employee's arm down. Employee worked without restriction until 2 Feb when they went out of work for what was told was unrelated work injury. 8 Apr was notified that the injury was work related.

A rail car Loader Operator twisted an ankle when the Operator slipped and fell on top of the rail car the Operator was walking on that was wet causing a hairline fracture of the right heel bone.

Employee walked out of building after light rain and foot slipped on concrete rampway exit causing a turn to grab right side handrail preventing fall. The right knee and right shoulder were stiff and sore the next day. Employee provided first aid and diagnostic X-ray/MRI negative on knee but provided cortisone shot on 12/19/19 follow-up.

An employee had positioned a box onto the Lab Loading dock and felt right knee pop with pain when turning away. The site doctor prescribed first aid treatments and ordered an MRI diagnostic test with orthopedic review. A follow-up medical exam was performed on July 22 where the site doctor ordered days away from work until MRI performed and cleared by orthopedic doctor.

Electrician rolled right ankle on uneven ground in grassy area while performing routine ground checks on electrical substation. No job restrictions nor days away from work lost to date; however, follow up for pain identified bone lesion and determined as work related with medical treatment recommended.

Mechanic reported back soreness on 11/7/18 after placing motor into bracket and was seen by site medical with first aid treatment. However, on 11/15/18 had follow up with site medical with work restrictions prescribed and follow up evaluation with back specialist ordered.

The Front End Loader Operator on the night shift was leaving the Clay Shed work area to clock out at 7 AM. The Operator left the Clay Shed walking down the inclined driveway past the west side of Fluid Bed. The Operator fell to the ground injuring the left ankle after stepping on a patch of frozen clay.

The employee was preparing to move a loaded bulk rail car to the track scale and slipped causing his knee to bend under himself as he fell. The employee first reported this injury on 10/1/2015 and was treated with first aid measures but sent for a MRI when soreness continued. MRI diagnosed on 10/15/2015 with a complex right knee meniscal tear that will require surgery to repair.

While hitching a generator to truck, thumb caught between ball and tongue. Causing a laceration on left thumb resulting in three stitches.

He was setting up a welder getting ready to work on a bag house. He was working on laying out welding leads getting them up to the bag house from the welder. He went to walk down a small decline near the work area and slipped, fell on left side landing on elbow on concrete. The area was wet and covered in mud and product. It had been raining the night before and that morning.

A BASF employee was releasing the hand brake on a railcar when the tension on the brake caused the handle to recoil. The recoil caused the employee's right pinkie finger to come in contact with the brake chain of the rail car. The employee was diagnosed as having a fractured finger and received two stitches at the tip of his finger.

Employee slipped and fell hitting is hip, shoulder and head on concrete floor. The employee was seen by doctor the next day and recieved first aid care. The employee continued to do full job duties but had gradual worsening of shoulder pain. An MRI was conducted and revealed a small rotator cuff tear on 9/9/30 and the employee was givne prescription medication.

On March 16, an EHS Specialist suffered a back strain while ascending a ladder during a MSHA inspection. This incident has been classified as a recordable injury based on the medical determination that his work activity aggravated a pre-existing condition, incident was deemed work related on 4/23/10. Fall from ladders

EE was tightening a bolt from the 2nd step of a ladder. His wrench slipped off bolt causing him to lose his balance and fall. He landed on his right hip and cracked his right pelvis.

EE was assisting drilling crew in connecting cables & pipe, stepping up & down from the drill rig platform (repetitive motion). He felt a slight pain in lower back. EE has a previous back injury dating back to 2004 with surgery & restricted duty. He remained on restricted duty & had reoccurence in 2008 & now another & is scheduled for Dr. appt. 1/29/10.

EE was using a drill press in the shop to drill holes to a larger size in a piece of aluminum stock 4""X4""X4"". The vice had been removed from the drill press. He attempted to hold the piece in his left hand while drilling, his hand slipped & the drill bit cut his hand between his thumb and index finger.

Fall down stairs

Employee exited a control room to a platform and while descending steps caught the back of his boot heel on the previous step and fell backwards injuring his left knee.

Original incident Knee hurting on 5/08 by truck driver. Second 12/02 complained about a knee hurting. Dr.Visit and examination the Dr. found that the employee has a pulled/strained hamstring and the Dr. referred him to have physical therapy.

A mechanic worked on a dust collector bag change on 5/22. The next Tues,he called in with complaints of back pain, but did not indicate a work related injury. While receiving medical attention, he reasoned that the bag change must be the cause of injury. On 6/13, he met with supervision to make a W/C claim. An investigation is in process. Awaiting assessment by physicians.

Empl has history of hypertenstion. Med?s not controlling. Empl had headache early am, began getting dizzy with a tight feeling in chest. Left area where he was cleaning chutes & went to control room reporting this to another empl. Initial treatment was for heart attack, which was ruled out by normal EKG. Underwent further testing for what has been deemed as a personal condition.

Employee was using tapping tool to tap threads in a coupling. He was using a crescent wrench to apply motive force instead of supplied T-wrench. When he applied force the tap shattered and his right hand, which was guiding the tap, made contact with the sharp edge of the shattered tap, resulting in a laceration that required 6 stitches.

EE was attempting to remove bearing from bag packing unit to replace it.The bearing is located such that to remove it requires reaching into the structure approximately two feet and removing the bearing horizontally. He had removed the set screws and attempted to loosen the bearing by pulling to the side with his right arm. As he pulled on it, felt sharp pain in R shoulder.

Employee was welding, opened hood to inspect work. He had safety glasses on. Dust dropped down into his face from unknown source. He tried to clear dust from his eyes so he could see, was unable to clear vision completely. He made it to eye wash station, flushed eyes. Eyes still irritated and at that time he sought medical attention.

EMPLOYEE WAS USING A BOX CUTTER KNIFE TO CUT A PLASTIC TIE OFF OF A PALLET OF MATERIAL. HE WAS HOLDING THE PLASTIC ABOVE THE TIE WITH HIS LEFT HAND, CUTTING WITH HIS RIGHT HAND TOWARDS HIS LT. HAND. THE BOX CUTTER SLIPPED OFF THE TIE, STRIKING HIS LT.WRIST, RESULTING IN A LACERATION. 6 STITCHES WERE REQUIRED.

Employee reported a slip and fall on a concrete ramp entrance into the maintenance shop. He requested no treatment at that time. An x-ray and MRI found no damage so no treatment resulted. Around Christmas, he reported continued soreness and stiffness and referral was made to an orthopedic doctor who began treatment with cortisone injection on 2/15/05.

EE REPORTED ON 2/17/04 THAT ON 1/23/04 HE & ANOTHER EE WERE OFFLOADING HAYBALES FROM A TRAILER AT CRESCENT LAKE. AFTER COMPLETING THE WORK, HE STATED HIS BACK WAS HURTING, AND LAID DOWN FOR A FEW MINUTES.

EE WAS REPLACING A RUBBER BOOT TRANSITION PIECE WHEN HE FELT SOMETHING IN HIS EYE. HE RINSED HIS EYE WITH WATER, BELIEVING IT WAS A GRAIN OF SAND. IRRITATION PERSISTED, AND MEDICAL TREATMENT S OUGHT. A SMALL METAL FRAGMENT WAS REMOVED FROM HIS RIGHT EYE, AND PRESCRIPTION ANTIBIOTIC DROPS GIVEN TO PREVENT INFECTION.

INJURED EE WAS WORKING ON M-55 SCREW CONVEYOR TRYING TO RESOLVE A FEED PROBLEM TO THE PACKER THIS AUGER FEEDS. HE WANTED TO VERIFY THAT FEED WASGETTING TO THE AUGER, AND REMOVED THE CAP ON THE BOTTOM OF THE AUGER. USING THE HOA SWITCH AS HISISOLATION OF ENERGY, HE STATED THAT ""HE THOUGHT HE HAD IT OFF"" BUT IT WAS IN AUTOMATIC. HE PLACED HIS HAND INTO THE MOVING AUGER.

THE INJURED EMPLOYEE WAS EXAMINING THE WORK AREA WHERE A DUST LEAK HAD BEEN REPORTED. HE SPOTTEDWHAT APPEARED TO BE A HOLE IN A CHECK VALVE ON THE PIPE OF THE PNEUMATIC TRANSFER SYSTEM. AS HE EXAMINED IT CLOSER, THE LINE PRESSURED UP AND BLEW DUST INTO HIS EYES. PRESCRIPTION DROPS WERE PRESCRIBED FOR IRRITATION TO HIS EYES.

INJURED EMPLOYEE WAS ATTMPTING TO MACHINE A HUB IN THE METAL LATHE IN THE MAINTENANCE SHOP. THE HUB HAD A LIP THAT PREVENTED THE JAW OF THE CHUCK TO HAVE FULL CONTACT WITH THE PIECE. AS HE S TARTGED THE LATHE, THE CUTTING TOOL CONTACTED THE HUB AND EJECTED IT INTO HIM, RESULTING IN A LACERATION ON HIS CHIN THAT REQUIRED FOUR STITCHES.

EE WAS SWEEPING FLOOR WHEN HE FELT SOMETHING IN HIS LEFT EYE. WITH OTHER EE ASSISTANCE, EE FLUSHED HIS EYE WITH WASH BOTTLE THEN AN EYEWASH. HE REPORTED CONTINUED DISCOMFORT THE FOLLOWING DAY. HE WAS CHECKED BY AN OPTHAMOLOGIST AND A PREVENTIVE ANTIBIOTIC PRESCRIBED FOR MILD CORNEAL ABRASION WAS GIVEN.

EMPLOYEE WAS WORKING ON 2 FT WIDE PLATFORM WELDING A HANDRAIL INTO PLACE. THE ANGLE FOR WELDING WAS NOT GOOD. EMPLOYEE WENT TO STEP BACK ONTO A LOWER PLATFORM AND CUT RIGHT LEG-SHIN ON EXPOSE D GRATING, PUNCTURE, CUT TO SHIN AREA.

EE WAS STACKING 50# BAGS ONTO PALLETS AT GEL SHIPPING. HE REPORTED PAIN IN HIS LEFT ELBOW, WHICH GOT PROGRESSIVELY WORSE. HE WAS SENT FOR MEDICAL TREATMENT, WHERE THE DOCTOR DIQAGNOSED THE CON DITION AS TENDONITIS.

Fall down stairs

EE WAS EXITING THE STOREROOM AT THE END OF HER WORKDAY. SHE BEGAN DESCENDING THE STAIRS AT THE LOADING DOCK, USING ONE HANDRAIL, WHEN HER FEET DSLIPPED. SHE FELL AND HIT HER ELBOW ON THE EDGE OF THE STAIRS. THREE STITCHES WERE REQUIRED. THESTAIRS HAS THREE TREADS CONSTRUCTED ON EXPANDED METAL. IT HAD RAINED MOST OF THE AFTERNOON, BUT NO MUD WAS ON STEP.
