During a pipe breaking operation by the float crew on the #5 Matrix line near the #8 Lift Pump, the Leaderman was struck by a pipe and suffered fatal injuries. The investigation of the accident by MSHA and the company is ongoing. Autopsy results are also pending.

Drowning

The victim was operating an excavator near a canal when the excavator tipped on its side and went in the water. The victim was able to get out of the cab and transported to a hospital where he died at 5:45 p.m.

Caught in, under or between a moving and a stationary object

During the removal of a sheave from a pump assembly, using a block of wood as a protective buffer while applying force with a sledgehammer. EE A was holding the block of wood against the sheave while EE B struck the block with the sledgehammer. During one of the swings, EE A attempted to reposition the block of wood, & the finger was pinched/smashed between the wood and the sheave.

EE reported feeling a popping sensation in left wrist while using a pipe wrench. EE reported some tenderness in outer part of left wrist. No swelling or deformity noted.

EE reports via phone that while working to replace the dump cables on the #3 dragline, when beating them out of their sockets and missed the swing. EE reports that this missed swing caused the movement to snatch the EE left's shoulder. EE reports left shoulder pain with movement.

EE reports walking to open valve to fill water tank, when right foot slipped on slippery ground causing right knee to twist while trying to maintain balance and not fall. EE describes right knee pain that prevents normal movement and prevents ability to work.

EE was exiting the truck to clean windows and mirror on R side of truck and when EE stepped on the lowest step the step gave way and the EE fell hitting back and bottom on the ground and head on the bottom front tire. EE seen at ER w/no restrictions. On 12/15/25 EE had lingering complications. On 12/17/25 diagnosed w/fracture of L shoulder, not able to RTW until reevaluation.

EE while in the process of removing debris/sticks from sleeve of pump, EE tripped over a plate that is part of the sled and fell backwards into a washout landing on left side. EE reports neck, shoulder, mid & lower back pain with stiffness. EE reports previous back injury.

EE was aligning strap with suction flex when left thumb got caught and crushed between the strap and suction flex as it was being lifted.

EE reports getting off the #3 dragline on the off-cab shoe ladder. When stepping off ladder left ankle rolled. EE unable to bear weight and had notable swelling in ankle. EE was sent to ER for evaluation by supervisor.

EE reports a popping sensation & a sharp pain in right side after slipping in mud & nearly falling. EE denies fall explaining that it took effort to catch balance. EE completed shift & reported to the Clinic by phone on 7/14/25. Follow-up call on 7/15/25 morning = no improvement. EE reports that the pain has not improved with rest & states unable to do much because of the pain.

EE's right hand was caught between the belts & the sheave while in the process of changing drive belts on final product pump. EE had to roll sheave in the opposite direction to remove the stuck hand. EE visited Clinic for after-hours treatment & returned to Clinic on 7/14/25, had decreased grip strength, swelling to thumb, ring & little finger. Hand was obviously painful to move.

EE stated that while in the process of backing the haul truck to empty it into the #2 pit, the rear of the haul truck entered the pit causing the cab of the unit to overturn on the passenger's side. EE was able to self-extricate from the haul truck. EE was observed by co-workers to have a laceration on head and swollen right shoulder area. EE reported pain in both areas.

EE called Clinic to report neck strain that occurred while pulling hose, prevented return to work the next morning. EE reports stiffness in neck that prevents turning of head left or right. EE was instructed to visit the Clinic for evaluation by company MD. EE evaluated and referred to clinic for evaluation and treatment.

Nutrien Employee was in the process of loading a mines power cable PLM head into the reel of the reel loader. The cable snagged, causing NE to drop the PLM head. The head fell onto NE's right ring finger pinching it against the reel.

EE began experiencing pain in right shoulder while working on the control cards on the #5 dragline. EE states the repetitive movement & the work to get the cards in and out of the slots caused the problem with the shoulder as the work was difficult & repetitive. EE states decreased range of motion due to pain. Denies previous problems with shoulder & is unable to work due to this.

EE Reports left shoulder pain from excessive vibration and movement in the heavy equipment that has become worse since. EE reports pain with decreased range of motion in the left shoulder.

EE reportedly stepped into a small hole of missing grating causing a twisting fall to the decking floor. EE landed flat on back. EE found by ERT standing holding onto desk for support in control room. EE stated back pain that prevents normal walking. EE called Clinic on 11/18/2024 to report getting a blister from new work boots on 11/14/2024. EE missed work on 11/15/2024 and went to personal doctor. PMD treating wound for infection and provided prescription antibiotics. Followed-up with PMD and received instructions to remain off work and follow-up with podiatrist.

EE states pulling a cable yesterday and experienced discomfort in right shoulder that has become worse throughout the day. States that the pain in shoulder is causing decreased range of motion, numbness and tingling in the fingers of the right hand. Field tech knelt down on D6 dozer track looking for coolant leak and felt a pop in EE's (L) knee. EE stood up and had slight discomfort. Tech contacted immediate supervisor to notify them. EE continued to work but discomfort became worse and swelling began. Tech contacted work partners and was advised to ice knee and stay off of it.

EE states while lifting a bucket cap & pin, felt a sharp/stab pain in mid back & immediately dropped the bucket cap. EE states was in the process of kneeling down on left knee while working on dragline shoe and knee popped. EE reported to on-site clinic with pain and swelling behind knee. EE was seen by orthopedic institute on 5/31, given restrictions which cannot be accommodated. EE will be out of work until further notice.

After checking temperatures on #3 D/L EE was walking to maintenance truck, EE stepped on uneven ground & rolled left ankle, felt a pop and fell. Employee EE Reported to Clinic on 04/02/2024 that they stepped into a hole and started having lower leg pain. EE was given first aid and OTC medication for pain and returned to work. Employee-EE, first reported to Clinic on 04/02/2024 that on 03/25/2024 they felt something in left eye and has had pain and irritation since. On eval. on 04/02/2024, nothing was visualized and EE was referred to ER for evaluation and treated for eye irritation with prescription eye drops. EE was able to return to duty with no restrictions or lost time to report.

Employee (EE) reported to the Clinic on 03/21/2024, states while stepping down backwards they missed the step and fell back on to their left hand and reported left wrist pain. EE was referred to Company Care for Evaluation. Restrictions were given that cannot be accommodated. EE is off work until further notice.

Employee (EE) reported to the Clinic on 03/12/2024 and stated that while attempting to loosen a bolt with a ratchet, while pulling handle towards their body, they felt lower back pain. On 12/29/23 Employee was using a pry bar to open a door on a pit pump. The pry bar slipped and struck the EE in the face causing a partial thickness facial laceration. EE was treated at the onsite clinic and returned to work, with no restrictions or lost time to report.

On 11/17/23 while taking fitting off a hydraulic power ram, as it came off, the fitting flew off striking EE on the face in upper left eye area, along with hydraulic fluid, just above safety glasses. EE reported to clinic with pain, swelling, and 3 small lacerations to area above left eye. EE was referred to LCMC for evaluation and treatment. EE is off work until further notice.

On 09/27/2023 Employee-EE stated they pinched their right hand between a wrench and a cylinder and reported swelling and pain in the area. EE was referred to Co. Care for eval. and treatment. EE has returned to work with restrictions until further notice.

On 9/11/2023 EE felt a pop in right forearm after throwing a dolly block in a man-basket in bucket truck. On 09/26 EE reported to clinic they had no pain just swelling and tightness in area. EE was referred to The Orthopedic Institute for eval. On 10/06 EE was seen by Ortho. On 10/09 we received the Ortho report EE has restrictions that cannot be accommodated and is now off work.

EE reported incident to Clinic on 09/06/23. EE was exiting dozer and the door swung shut striking EE in the mouth. Impact from door broke a portion of top right front tooth. EE was referred to dental for tooth evaluation and possible repair. EE was able to return to shift and does not have any lost time to report.

EE was using a pry bar to turn the pump assembly for alignment when the bar slipped and struck the employee on the nose. The impact caused a laceration to the left side of the nose. EE went to a clinic for evaluation and treatment.

Slipping caused a fall to ground during which employee's right arm struck the hanger plate. EE reported incident to Supv. 6/11/23, reported to Clinic 6/12/23. EE reports back pain after standing up straight from a bent over position. EE requested over the counter meds on 4/20 but no other care was given. Clinic was notified on 4/24 that EE is off work due to back pain. EE was sent to Urgent Care and given restrictions the area cannot accommodate; EE is off work until further notice.

EE reports that right eye began to feel itchy while operating an enclosed cab bulldozer in cooling pond area. EE was worse after rubbing it, washed eye out and noted swelling. EE reports to clinic denies pain or foreign body, EE states eye feels strange. EE sent to an eye clinic and treated for possible allergic reaction with steroid eye drop.

EE experienced low back pain after spending several hours moving power cable in muddy ground conditions. EE reported to clinic and sent to an occupational medicine facility for evaluation and treatment. EE's restrictions cannot be accommodated. EE is out of work effective 2/15/23.

EE was removing a front knuckle assembly from pickup truck. When pulling on front knuckle, EE lost grip causing left thumb to get caught between the knuckle & part of truck. EE was referred to company care and treated for pain in forearm, abrasion/pain in thumb. EE placed on modified duty.

EE was using a pry bar to take swing brake off swing motor. When EE applied pressure to bar EE experienced pain under right pectoral muscle, initially filed as report only. Employee contacted Clinic 12/15/22 requesting evaluation & treatment due to on-going pain stating unable to breathe normally or sleep normally due to pain. EE went to LCMC-ED for treatment. No restrictions.

EE was operating a D8 bulldozer on pebble pile and backed off a drop-off of the pile. EE states: jarred neck & has tightness & soreness on right side of neck. Initially report only but presented to clinic via phone having increased pain from neck down to leg. EE was given restrictions and placed on modified duty.

Crew was in process of replacing dump cables at #2 Dragline. Employee was cutting U-bolt that holds the end of cable to the spool, when cable was cut free employee was in the line of fire & struck in hard hat & right side of face causing facial fracture, 2 chipped teeth and mild concussion.

Employee was climbing on equipment & experienced a pulling sensation in left upper arm. EE reported to clinic, follow up visits with on-site Dr, EE continued to work with no restrictions. MRI on 12/15 confirmed distal bicep tear. Surgery scheduled for 1/4/23. LDW 12/21/22

EE was installing a packing in WBC matrix pump. EE was turning shaft to tighten impeller against the packing sleeve, using a 48"" pipe wrench to turn impeller shaft. Shaft became difficult to turn but was not fully seated against packing. When EE pulled with force, EE felt pain in right elbow. EE was sent to company care and will be out of work, follow up appointment scheduled.

EE was pushing product from holding bin bypass discharge up to top of rock pile when dozer fell in open draw hole while turning the corner with full blade of product. EE reported to clinic 9/29 with back & wrist pain, follow up visits with onsite Dr. EE sent to neck surgeon on 12/15, physical therapy was prescribed for neck pain.

Employees were tying in a pipe on #4 Dragline area attempting to get bolts in pipe. Co-worker was using 1 5/8 wrench to tap the bolt in when injured employee placed a hand in the way causing wrench to contact left hand. EE transferred to Medical, appt scheduled with Ortho institute on 8/9/22. Possible crush injury to left hand.

Employee reported discomfort in right shoulder on 9/13/22. EE states right shoulder continued to bother EE since incident occurred 6/25. EE was referred to Ortho for additional evaluation. EE continued to work with no restrictions. EE had surgery on 1/27/23.

Employee was in process of pulling instrument cable that was buried in the dirt when EE experienced a popping sensation in right shoulder. Employee developed increased pain in the right bicep. Employee has been placed out of work and will be evaluated by Orthopedic Institute.

Shift began on 2/3 at 6 pm. Employee was looping a chain under a shaft on bolt head in an attempt to roll the shaft to remove bolts. While holding the hoist control in right hand & holding the chain in left hand EE hoisted up & chain slipped off bolt head & struck EE on left index finger cutting it open. EE was sent to LCMC for evaluation, wound cleaned & bandaged.

EE was walking from pit car and stepped into a hole. EE stated EE did not hurt just wanted to report the incident. The following day EE did not report to work. EE was sent to Company Care for an evaluation. EE is off work due to restrictions which cannot be accommodated. Similar incident occurred 10/18/21.

EE was pulling a loaded dolly in the west tunnel & it fell over striking them in the area of left upper leg/hip. EE presented to the clinic with a limp favoring the left leg. No bruising or swelling noted. EE was sent offsite for evaluation, EE was modified duty for the remainder of shift. EE's regular schedule had them scheduled off duty the next 7 days, released to full duty.

Employee reports was attempting to move a water cannon and experienced a sharp pain and pop in the right shoulder. Reports that the pain in the shoulder is a steady pain and has a buzzing sensation in the right hand. This is the same shoulder that was previously injured and may be a re-injury. EE was placed out of work 11/2/21, EE has a dr. appt. on 11/3/21.

Employee was walking in the area of the traveling water screens and the grating fell through the support causing employee's right foot to go into the resulting hole. EE reports EE was walking in tunnel & stepped on uneven area & noticed lower back pain, later noticed right foot pain. EE went to emergency room, while there EE contacted EE's supervisor to report injury from previous shift who instructed EE to call clinic and report injury. Diagnosis was crush injury to tip of right ring finger, placed on modified duty.

Employee was changing out agitator paddles, one fell & struck EE in the back of right hand. EE's hand was in midair, not on any surface. EE noticed mild pain at the time & noticed severe pain, swelling & decreased ability to move fingers at about 0300 on 9/3/21. EE was seen at onsite clinic and referred to ortho. Restricted use, no work. Right hand in rigid splint.

As EE was turning the pump sheave with a pipe wrench, the momentum of sheave caused the employee's finger to get pinched between the pipe wrench handle & a belt guard mounting bracket. EE was seen by onsite EMT & sent to LCMC where x-rays showed a fracture in tip of finger. EE will have surgery on 6/23/21.

When employees began to apply pressure to the ram, a coupling on hydraulic cylinder failed & struck one employee in left forearm. HEO was taken to clinic then to hospital, staff cleaned wound & prescribed antibiotics.

Employee was cutting a 600 - 800 pound piece of steel off of the center pin assembly of the #1 dragline tub. The steel was supported by a chain attached to a boom truck. When the steel was cut free, it shifted, striking the employee.

Flash burns (welding)

Employee & co-worker were welding flanges on pipes. Employee notice eye irritation after appr. 10 hrs. Irritation got progressively worse until he returned to work on 1/27 and received treatment from the company nurse. Prescription medication was administered.

Employee, Mechanic with 31 years experience, was fabing 'A' frame for de-watering barge. While using overhead shop hoist to move frame out of shop, chain on hoist slipped causing frame to move. Employee lost his balance causing him to fall to shop floor. Employee landed on right elbow which caused a fracture of the right humerus.

A fire was reported in Section 23. It was determined that the fire was started by lightening and was contained by pushing a dozer line around the fire. Company security was notified in order to monitor the area to prevent further spread of fire.

Around 12:00PM, PCS emp. was driving in mine area and spotted smoke in a muck pile. He notified supv. & dozer oper. who responded. Fire was possibly started by hot ash from nearby wildfire or recent lightning strike. No personnel or equipment were endangered. Fire was isolated & extinguished at time. No smoke was noted the morning of 5/18/07.

Employee was lifting buckets of bolts from the back of a truck. The employee did not notice any discomfort that day, but during the night she began to experience pain in her left shoulder. The next day she told her supervisor of the pain in her shoulder. On 2/8/07 she was evaluated by the company physician and was ordered restricted duty.

Employee, Heavy Equipment Trainee with 7 months experience, stepped over a piece of 20"" matrix pipe and the ground gave way twisting his right leg. He reported discomfort in his right knee to his supervisor and saw the company doctor on 1/25/07. He worked without restriction until he was placed on restricted duty by the company doctor on 3/21/07 for subjective pain.

Employee was removing the bottom bolts in 20"" matrix fitting (all other bolts had been removed). A 980 loader was holding the pipe. Employee was using impact wrench w/his R hand, L hand was supporting imp. wrench. As he removed a bottom bolt, it sheared, & another bolt pulled through the bolt hole. Pipe fell on employee's hand pinning his L thumb between the impact wrench & pipe.

On 3/30/06 employee reported a gradual worsening pain in his right hand. On 6/2/06 employee was evaluated by an orthopedic physician and diagnosed with carpal tunnel syndrome in the right hand.

Employee was replacing a damaged hyraulic line on a D8R tractor. Employee had to lie on his stomach to access the connection of the line. He then had to reach as far as possible to get his wrench on the bolt heads. As he went to loosen the bolts, his wrench slipped off the bolt causing a sudden jerk, straining his neck. Employee recently began physical therapy for treatment.

Employee was pushing rock into a draw hole on the rock pile using tractor. As he approached the edge of the hole, he misjudged the distance to the edge, which resulted in the hole collapsing causing the tractor to fall into the hole. The impact at the bottom of the hole caused employee to strike cab support with left shoulder.

Struck against stationary object

Employee reported to his Superintendent that he had struck the side of his right hand against the window frame of the dozer as he was turning in his seat. He did not seek immediate medical attention. He was later evaluated by the Company Nurse and further by the Company physician, who ordered an X-ray which revealed a fracture at the base of the right fifth metacarpal.

Employee was walking up a sand incline leaving a pumping station when his right foot slipped and he fell forward as his right leg shot outward. He experienced tenderness in his right knee and thigh. The company physician referred the employee to an orthopedic physician. Outpatient surgery was performed to repair the injured knee.

Over-exertion in pulling or pushing objects

Employee was changing out 8"" pipe on south side of Washer, requiring climbing and pulling on cables in pipe rack. He later felt discomfort in lower back. As a result from an Orthopedic consult, physical therapy was ordered for treatment.

Fall from scaffolds, walkways, platforms

Injured employee was preparing to fuel an excavator. He placed the fuel nozzle on the catwalk by the excavator and then climbed up onto the catwalk. Once on the catwalk he lost his balance and jumped/fell 4-5 feet to the ground. An x-ray taken on 8/18 revealed a cracked bone. Employee was then placed on light duty status.
