Incident occurred with an employee operating machinery in the mine. He felt ill, got off the machine. The foreman immediately began questioning the opr and discovered he had forgotten to take his meds. Paramedics were called and found blood pressure high. They recommended air flight which transported to hospital. He was treated and released after controlling the personal concern.

Employee was walking down a slipped concrete surface to pick up a safe work permit that the wind had blown down. He slipped in some mud on the concrete surface. He turned to catch himself, his hard hat fell off and he fell on the hard hat liner and cut his forehead above his left eye and 1" long. 5 stitches.
