He was moving conveyor belt over after repairing splice to belt, prybar slipped & hit him above the eye & he fell back & hit his arm on the side of the screen deck. Approx. 1"cut above his left eye & a broken arm.

EMPLOYEE USED A PRY BAR TO REMOVE A ROCK, HE WAS PUSHING ON THE PRY BAR, IT SLIPPED AND SPRUNG BACK HITTING HIM IN THE LEFT CHEEK, WHICH RESULTED IN SUTURES.
