Struck against a moving object

Employee had been assigned to operate a haul truck in the pit area. As EE was returning to the pit after dumping a load in the primary crusher, EE hit a pot hole in the road and felt a pain in back.

Accident type, without injuries

The operator is filing this 7000-1 under protest and to comply with the mandatory abatement provisions of the act. Constructing a pond, personnel filled the pond to ensure impermeability. The next morning the water level had lowered. MSHA was notified immediately. No soil disturbance or material failure was discovered.

EE REACHED INTO A DRAWER AT THE JAW OPERATORS BUILDING & CUT HIS HAND ON A 2"" BAND THAT WAS BEING STORED FOR OUR WATER HOSES.

THE EMPLOYEE WAS REPAIRING AN ELECTRIC BOX WITH A SCREWDRIVER. THE SCREWDRIVER SLIPPED AND HIT A HOT WIRE THAT ARCED AGAINST THE BOX BURNING HIS HAND.

EE WAS WELDING HUB ON 1840 CASE SKID STEER. ONE OF THE CHAIN LINKS CAUGHT ON FIRE. HE RAISED HIS WELDING HELMET TO CHECK AND A FLASH FIRE CAUGHT HIS FACE AND NECK CAUSING 2ND DEGREE BURNS.

EE WAS SHOVELING AROUND PLANT WHEN HE FELT A PULL IN HIS BACK.

Fall from ladders

EE WAS STANDING ON A 2' LADDER CHANGING AN OIL FILTER ON AN R-50 EUCLID. WHEN HE SHIFTED HIS WEIGHT THE LADDER CAME OUT FROM UNDER HIM CAUSING A FALL. THE EE'S SHOULDER STRUCK A BATTERY THAT W AS ON THE SHOP FLOOR.

LIFTING A 44"" X 74"" PIECE OF 1/2"" STEEL PLATE WITH A SHEEPDOG ATTACHED TO A HOIST. THE SHEEPDOG SLIPPED OFF THE METAL AND HIT EE IN THE BACK AND ON THE BACK OF HIS LEGS.
