Struck by flying object

Contractor was replacing a rubber suspension boot for a water truck and it ruptured, causing pieces of rubber to strike worker in the face. It broke worker's palate and eye socket.

Gust of wind caused employee to stumble and trip on air hose landing on right leg and left arm fracturing left elbow.

Employee got foreign object in eye when welding/chipping slag. Employee was wearing welding helmet at time of injury.

July 5 employee complained of headache, nauseous and dizzy. Through medical evaluation it was determined that employee had allergic rhinitis. Claim was denied to be work related by insurance but on May 3rd, 2017 decision was rescinded and became compensable. This injury is being reported within the 10 day reporting requirement from the date it was determined to be work related.

Struck against stationary object

Employee operating grader in spoil area and blade hit a hidden bolder throwing the employee forward. Employee's mouth struck the steering wheel knob and chipped a tooth.

Struck by flying object

Maintenance worker was using a cordless impact wrench to drill holes in conveyor belt spice. When pulling trigger to drill 3rd hole debris flew out of wrench vent holes and went under employees safety glasses and into right eye. Antibiotic eye drops prescribed.

Caught in, under or between a moving and a stationary object

Customer truck driver was trimming their load of rip rap. Rip rap was preventing the tailgate from closing. When driver dislodged the rip rap the tailgate closed on the middle right finger resulting in amputation at the first knuckle.

Over-exertion in lifting objects

Two employees lifting printer enclosure from a pallet into the back of a pickup truck with a topper. While placing the enclosure on the tailgate one employee felt a twinge in their back. Employee was diagnosed with tilted down S1 vertebrae. Employee is a support person and not an actual ""miner"".

Struck by flying object

Employee was flipping impact bars on Stedman. Employee used 4lb short handle sledge and hit wedge to loosen. Chip from wedge broke off and struck employee apx 4"" above navel.

Flash burns (welding)

2 employees were to weld a broken hand rail & replace a v-belt guard. The 2 items were 5 feet apart. They decided to weld the handrail & then replace the guard. For reasons unknown the injured employee started replacing the guard while the welder was repairing the hand rail. This resulted in a flash burn to the employees right eye. The employee did not work his next shift.

Struck by flying object

Eye irritation while doing maintenance on crusher.Conditions were windy,hot,and humid.Employee was wearing safety glasses. Employee reported incident 08/02/2010 took to clinic. First aid administered. Return visits 8/4 & 8/6 still first aid. 08/09/10 sent to eye doctor, given perscription eye drops for inflamation.Which made this a reportable injury.

Struck against a moving object

Employee operating a Cat 613 Water Wagon hit a dip in road and felt pain in back/rib area. Employee taken to clinic, x-rays taken-negative. Overhead restrictions did not impede employee from doing normal job. Cat Scan negative 6/30/09 it was determined that the employee had a soft tissue injury and physical therapy required, injury then reportable to MSHA. No lost work days.

Employee was welding a safety plate above a feeder, when a rock slid down surge pile, striking employee in lower back. Employee was taken for medical treatment; no xrays required. Doctor told employee to remain off til Thursday, June 7th, when employee will return to doctor.

While removing a ride strut hold down bolt, the bolt broke free allowing his forearm to make contact with a factory bumper. The bumper had a sharp corner that punctured his skin. 6 stitches were required to close the wound.

SPOOL OF WIRE GOT STUCK WHILE PULLING IN; WENT TO FREE-UP AND JAMMED FINGER OF LEFT HAND INTO RAIL OF FORK TRUCK. FRACTURED FINGER ON LEFT HAND. NO MISSED TIME FROM WORK DUE TO INJURY. EE S OUGHT MEDICAL ATTENTION AT 3:30 PM ON 10/18/03. RETURNED TO WORK ON MONDAY MORNING, 10/20/2003. NOTE: REASON FOR REPORT BEING LATE WAS A DELAY IN THE ISSUANCE OF MSHA CONTRACTOR ID NO. ONLINE.

A BLAST HOLE DRILLER EMPLOYED BY INDEPENDENT CONTRACTOR GENERAL DRILLING WAS MAKING A REPAIR TO THE MUD FLAP ON HIS DRILL TRUCK. USING A PUNCH TO MAKE A HOLE IN THE MUD FLAP, HE MISSED AND HIT HIS LEFT HAND WITH HIS HAMMER. THE ACCIDENT OCCURRED IN THE QUARRY SHOP. HE WAS TREATED AND RELEASED AT A CLINIC. THE DOCTOR REPORTED ""NO PERMANENT IMPAIRMENT"". HE WAS RELEASED TO RETURN 9/29

WHILE INSTALLING THE DRIVE BELT ON A SCREEN AN EE GOT HIS FINGER CAUGHT B/W THE BELT AND THE SCREEN SHEAVE AND CUT THE END OF HIS FINGER. DUETO A BROKEN MOTOR MOUNT THE EE WAS NOT ABLE TO FULLY ADJUST THE MOUNT PROPERLY AND WAS ROLLING THE BELTS ON WHEN THE ACCIDENT OCCURRED.

AN EE WAS ADJUSTING A DRYER KILN BY TIGHTENING THE BOLT ON THE DRYER TRUNIONS WHEN THE WRENCH HE WAS USING SLIPPED ON THE HEAD OF THE BOLT & STRUCK HIM IN THE MOUTH. HE WENT TO THE HOSPITAL AND RECEIVED TWO STITCHES TO THE INSIDE OF HIS LOWER LIP. EE RECEIVED ANTIBIOTICS. EE RETURNED TO WORK W/NO RESTRICTIONS.

EE RECEIVED A FLASH BURN TO HIS RIGHT EYE AS HE WAS HOLDING THE OPPOSITE END OF A 10"" PIPE THAT WAS BEING WELDED. HE WAS ABOUT 8' FROM WHERE THEPIPE WAS BEING WELDED WITH HIS BACK TO THE WELDI NG. HE CLAIMS HE NEVER LOOKED AT THE WELDING ANDHAD HIS BACK TO THE WELD THE ENTIRE TIME.

OPEARTOR WAS DUMPING OFF ON STOCKPILE WHEN THE BACK OF THE PILE SLUMPED OFF. THE END DUMP SLID BACKWARDS DOWN STOCKPILE WITH END DUMP COMING TO REST WITH ITS. FRONT WHEELS IN THE AIR. UNKNOWN TO THE OPERATOR A LOADER HAD LOADED A CUSTOMER FROM THE BACK OF THE PILE, CAUSING THE PILE TO SLIDE WHEN HE BACKED UP.
