Employee was going to get in the loader started up the stairs forgot to get ink pen turned around and let go of safety gate, bolt on handle hit his left thumb cutting his thumb, required six stitches

Using expanded metal to make the guard the employee was using his knee to hold the metal in place so he could mark the spot where the self tapping screw was going to go. While marking the spot on the guard the expanded metal slipped off his knee and sliced his left index finger.

Employee was folding a stacker when the pry bar slipped and crushed his thumb between the conveyer frame.

WHILE MOVING ROCK AROUND (ROUGHLY 12"" DIAMETER) WITH SKID LOADER, ONE ROCK ROLLED BACK INTO CAB STRIKING LEFT LEG CAUSING LACERATION TO UPPER SHIN ON LEFT REQUIRING STITCHES.

REPAIRING AN AIR BRAKE LINE, WHILE CUTTING LINE WITH KNIFE, EMPLOYEE SLIPPED AND CUT LEFT THUMB, REQUIRING SIX STITCHES.
