THE EE WAS REMOVING A SEAT FROM A D-9 DOZER. AS HE PICKED THE SEAT UP & TWISTED TO HAND IT OUT THE DOOR HE FELT A PAIN IN HIS BACK.

THE INJURED EMPLOYEE AND THE TIRE TRUCK OPERATOR WERE CLAMPING A PIECE OF METAL TO THE FRAME OF THE TIRE TRUCK. THE OPERATORS HANDS SLIPPED AND ALLOWED THE METAL TO STRIKE THE EMPLOYEE JACKY HUFF IN THE HEAD. THE LACERATION REQUIRED FIVE STITCHES.
