DRAW ROCK IN FACE AREA. OPER WAS ATTEMPTING TO CUT LAST CAR IN FACE. OPER HAD ONE ROOF BOLT OVER HEAD AND ONE ROOF BOLT INBY HIM. FACE AREA FELL IN AND 2 PIECES OF ROCK FELL BETWEEN THE LAST T WO BOLTS.

THE EMPLOYEE WAS LOADING AN OXYGEN BOTTLE INTO A SCOOP BUCKET AND CAUGHT HIS FINGERS BETWEEN THEM. IT CUT THE END OF HIS FINGER.
