I,NAME WAS BOLTING FIRST CUT OUT OF #3 ENTRY THE FIRST BOLT WHEN BOLT DRILL STEEL AND UP PRESSURE HIT A CRACK IN ROOF ROCK CAUSING ROCK TO SLIP OFF ON ATRS AND APRON AND THEN ONTO OPERATOR PIN NING HIM UNDER ROCK & AGAINST THE BOLTER BREAKING HIS RIGHT FOREARM & BRUISING HIS LEFT SHOULDER.
