Employee was shutting down the Trommel screen. When he pulled the lever to shut it down, a hyd. hose blew & sprayed him w/oil. He was wearing his hard hat & safety glasses but some still ran in his eyes. On Tues. he called & said his eyes were burning - going to doctor. Wed. he called again - said his eyes still were bothering. Was told to go to outpatient.

EMPLOYEE WAS GETTING OUT OF TRUCK AT THE GRAVEL PIT AND SLIPPED ON THE TRUCK STAIRS AND STARTED TO FALL. HE WAS HANGING ONTO THE HANDLE AND WHEN HE FELL IT HURT HIS LEFT SHOULDER.
