Contact with electrical current

Notified that an incident occurred. Upon checking near the main shop area, Miner 1 (an electrician) was found lying on the main floor, surrounded by fellow miners. It was determined that the 440 main breaker had experienced a catastrophic failure. Subsequent observations revealed that Miner 1 had borne the brunt of the failure and sustained 2nd-degree burns to both hands.

Struck by flying object

Miner A was chipping debris in the discharge area of the finish mill, when Miner B noticed a fresh wound on Miner A's right forearm. It is believed that material from the mill caused this laceration. Miner A was conveyed to the hospital and released later that day. Miner A returned to work the next day.

Description of Incident: Employee was attempting to remove pallet from lowered hi-lo forks by hand and sustained a small laceration of left middle finger from an exposed packing nail. Employee was taken to ER and treatment was rendered in the form of two stiches. Employee was cleared and returned to work.

Employee was holding a hose and working with another employee to remove a fitting which was taped to the hose. The other employee was cutting the tape and hose when the knife slipped and struck the back of the employee's right middle finger causing a laceration. The laceration required sutures to close.

The employee went to put an air lance down the pipe, when doing so the air came back faster than expected and the material went right over safety glasses and into the employees eyes. The employee was not wearing EE's face shield.

Miner entered a confined space that shut down 8 hours before hand. The miners boots were not tied. 6"" of hot material was at the bottom. The miner stepped into hot material. That material got into EE's boot and burned EE's foot. 2nd degree burn.

Using a crane and hopper to load balls into finish mill. An employee tried to connect the crane hook to the clevis. The operator lifted the load before employee was done attaching the hook.

EE was cleaning area for compressor to be installed and was struck by PVC pipe. EE said EE felt fine but wanted to get checked out. EE is on FMLA and usually takes 1-2 days off a week. EE was taken to emergency and was released without any restrictions. EE went to EE's FMLA doctor and asked for medication for the tightness and was given rest of week off.

Employee walked from fuel pump to open door looking down on 9 tail pulley to check on water presence in tunnel. Employee turned to the right and walked away from chained doorway when right foot slipped on ice. Left leg bent underneath employee, left arm slipped out over employee's head trying to stop fall.

EE, a Thermico, Inc. was working as a contractor at the St. Mary's Cement plant in Charlevoix, MI and the clamps holding a 4-ft bar broke loose and fell 40 feet striking the employee in the right upper arm. He sufferer a fracture of his right upper arm and was treated at the local hospital.

While screenign balls, ball screener plugged up. EE climbed ladder stand and poked the chute to unplug the ball screener. The ladder stand collapsed, causing EE to fall.

Abandoned car noticed near stack around 1:30pm. Police were called-they stated its' an abandoned car and left. In the morning, smoke was coming from car. EE put out fire. Police were called again, they checked inside of car and found body of a man. Car and body were removed by Detroit Police around 10:30am
