Fall from machine

Employee fell from cab of drill when drill tipped over. Employee fell approximately 40 feet from the dab to the next lower bench. Fall resulted in fatal injuries.

Employee was in the process of installing the lock ring on a 240 ton haul truck tire when the ring snapped into place causing the clamp to make contact with the employee's right index finger resulting in a laceration.

Worker was working on installing a driveline on a DR412i equipment. One side was connected and the other end of the driveline fell. Worker's hands were trapped between driveline and pumps. Worker removed both hands after getting pinched adding to the injury. Worker climbed out of pump area and off the machine.

Struck by flying object

Employee was using hammer and chisel to remove a carbide insert when a piece of metal broke from the chisel and punctured the inside of the left wrist and becoming embedded beneath the skin.

EE fell and landed on their right wrist while trying to avoid a light fleet vehicle that was backing up to retrieve tow slings. EE received a splint during the ER visit.

Employee got a piece of slag in their right eye while a nearby employee was using a grinder.

EE was walking outside and slipped on a snow packed road, fell and landed on their shoulder causing a dislocation of the left shoulder.

Mechanic's hand was briefly pinched between a hydraulic shovel roller assembly and the shovel frame during installation. Contact occurred when the assembly being installed unexpectedly pivoted upward while the mechanic's hand was on top of it. Nondisplaced fracture of the second metacarpal bone, right hand.

While backing into spotted shovel bucket, the tail of production truck made contact with shovel bucket. The operator reported discomfort in their neck and was transported to the clinic for evaluation. After evaluation, the operator was prescribed physical therapy for a neck strain.

EE was walking on a blast pattern and stepped into a small depression and hurt left knee, continued to work. On 07/31/19 EE was prescribed physical therapy due to continued pain.

As the EE bent over to pick up a two wheeled cart EE felt pain in EE's back. The mechanism of injury was idiopathic in nature and due to a chronic condition. The EE was seen by a physician and placed on restrictions.

EE was using a pocketknife to cut a zip tie holding a pair of cut resistant gloves together. The pocketknife cut through the zip tie and contacted the EE's left middle finger resulting in a minor laceration. Two sutures were required.

Struck by flying object

EE was removing a bearing race off a shaft. The race was first cut with a grinder and then the EE attempted to split the race with a chisel and hammer. While removing the race, a small piece of shrapnel struck the employees left palm. Medical treatment was required to remove the foreign object on 5/14/2018.

EE was driving a production truck, and reported a sore back from being shook while in the cab.

A shovel operator felt some discomfort in lower back when doing the pre-shift inspection. As the shift continued, the pain increased and EE sought medical attention after the shift.

Elevator broke down with employee inside. Employee safely exited the elevator about and hour later. The elevator was taken out of service.

EE states the while egressing off a utility loader ee had back pain. When ee was asked if ee jumped or fell ee stated it was a normal dismount with 3 points of contact. On 3/16/18 ee was diagnosed with a back strain and placed on restrictions.

A non-employee trespasser was found dead hanging in a tree by a city of Eveleth employee. Death was ruled a suicide by ligature hanging. This is an inactive area of the mine adjacent to the City of Eveleth. Eveleth Police Dept. ICR# 17086466.

The EE was installing a wing shroud on a dozer. While attempting the pound the keeper pin in place, the ee hit the pin and glanced off hitting left index finger with the hammer. The ee was sent into the clinic and received two sutures. Risk 1. Low

The EE was cleaning out plugged crusher. As they were pulling material out a rock rolled down, struck hand and cut finger. The injury required three sutures.

Working at crushing site outside. Employee was removing jacket from prox cord with a utility knife when the knife slipped and caught middle finger.

The EE was jack hammering concaves when the bit became lodged and became stuck. The EE retrieved a new tooth and was working near area of the stuck tooth when a section of the concave broke off and shot out due to the stored energy in the liner. The chunk of concave struck the employee in the jaw and neck.

On 11/13/14 the EE was coming down off a berm when he fell forward. He landed on his hands, chest and chin. His knee hit a rock at the base of the berm. The EE had no apparent serious injuries at the time and refused medical attention. On 12/31/14 this became a lost time work related injury due to shoulder pain.

The resistor grids on a Unit Rig MT4400 240-ton haul truck caught fire while travelling to the shovel. The operator reported smoke coming from the resistor grids safely evacuated. There was a mechanical failure of the blades on the fan used to cool the grids; this lead to a fire which was localized and contained to the rear resistor grid bank.

An employee was driving a Komatsu 830E 240ton production truck when it caught fire. The employee stopped the truck, shut it off and safely evacuated the vehicle. Local fire departments were called, they responded and safely put out the fire. This is now under investigation to determine the cause of the fire. No injuries occurred from this incident.

EE was changing a deck bushing on a rotary drill when he bumped his shin. The next day swelling occurred on his leg and a red spot formed. The EE was seen by a Dr. and diagnosed with Cellulites.

The EE was in the process of completing his pre-operational inspection on a Cat D-11 dozer when he slipped off the dozer platform and fell to the ground. He was taken to the hospital and diagnosed with fractured ribs.

The incident occurred on April 23, 2013 when an employee slipped on a stairs and reported pain in his shoulder. The incident became reportable on 5/13/13 when he was seen by a DR. and prescribed physical therapy and put on restrictions.

Fuel truck slid out of control on an icy roadway causing the fuel truck to tip over onto the passenger side. The employee was wearing his seat belt and was suspended by the seat belt which he unbuckled with his other hand. This resulted in him falling to the passenger side of the vehicle.

EMPLOYEE WAS ASSIGNED TO CHANGE HYDRAULIC HOSES AND PUMP ON CAT 992G LOADER. WHEN REMOVING SUCTION LINE FROM HYDRAULIC TANK OIL SPILLED FROM SUCTION HOSE ONTO EMPLOYEES HEAD, AND ENTERED HIS EYES. EE EYE WAS IMMEDIATELY FLUSHED BUT WAS SENT TO CLINIC AS A PRECAUTION AND WAS PRESCRIBED ANTIBIOTIC DROPS.

Core Drill Helper (Victim) was pulling a tube out of the rods when he strained his back. He was brought to the E.R., told to rest over the weekend. He saw an occupational medicine phys. on 6/5/12 and release to work.

Production truck driver was performing his pre-shift inspection on a 240 ton production truck when he opened the folding engine inspection ladder and pinched his middle finger on his right hand. The employee was sent to the local medical facility were he received sutures.

The employee states he was at work using machinery to dig through rocks and hand jarring that day. The jarring increased his neck and upper back pain. He stated that he has treated for many years regarding his neck and back pain. Discussions with the clinic indicate this is an exacerbation of a degenerative disease/illness not an injury.

Employees back was sore at the end of the shift. MRI was completed showing disc degeneration and dehydration of disc fluid. There was no disc hernia or rupture present. There was no event just soreness at the end of the shift from working.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was cleaning out the crusher pocket when he began feeling lightheaded and ill, ems services were called and he was transported to the hospital. The investigation discovered the employee was over exposed to carbon monoxide due to a pickup parked outside the crusher that was left idling. The wind direction caused the exhaust to blow down into the crusher pocket.

The employee was operating a dozer pushing in a road when he reported that his back became sore. This is a reaggravation of a preexisting condition.

While getting off of a loader the operator stepped into a hole. He reported that his right foot was starting to swell and that his ankle was unaffected. He did work the remainder of his regular shift. Later in the evening the employee went into the local clinic and was diagnosed with a foot sprain.

The employee was taking a measurement to place a pad eye in a level position. The employee stood up and took a step forward striking his forehead against a 4""x6"" steel removal block that was previously installed. This caused a laceration to the employees forehead.

EE was emptying a core tube when a piece of jammed core slid out striking the EE's right hand. The sharp core cut his hand resulting in a laceration requiring 6 stitches.

Employee was wiring generator on Loader and felt a pull in his left shoulder muscles. The employee reported the incident but did not treat at the time. On 8/22/11 the employee was diagnosed with left shoulder pain and prescribed physical therapy.

The injured employee was working in the pit on a windy day. He was wearing non prescription safety glasses while doing maintenance work when a gust of wind occurred. The felt a particle enter his eye, his eye was washed out but still felt some irritation so he was sent to the clinic where they diagnosed him with a corneal abrasion and prescribed antibiotic drops.

Employee was climbing off the drill at the end of the shift. His grippers caught the bottom step and he landed on the side of his ankle. He was sent to the hospital. Accident resulted in a fractured right distal fibula.

Mechanic inspecting 9028 loader for engine coolant leak. Near left side of engine close to engine fan when a particle was blown into employee's right eye. Became worse as the shift progressed; was washed out by First Responders but found it necessary to send employee to clinic. Foregin body washed out, prescription med dispensed.

Employee was performing preventive maintenance on a loader. He states he felt a pull in his back due to the incident. He treated with the chiropractor. Company investigation not completed until 9/24/10 when it was determined that there was an incident resulting in some type of back pain.

Employee states his hands go numb after running equipment for extended periods of time. No treatment beyond first aid until 9/22/10 testing for CTS. Work comp insurance received information on 9/23/10 forwarded to United Taconite on 9/27.

A trespasser operating an ATV on mine property lost control of the AVT rolling it on to him causing internal injuries. The injured was then taken off of mine property, by other illegal ATV riders, to a county road and emergency services were called. The injured died at Virginia Regional Medical Center from his injuries.

Employee was getting into the shift change van. While entering the van on the side door, the employee slipped. It was dark and wet outside. His shin hit the metal edge of the van, causing an open wound.

Employee states he was to guide 9016 shovel back into stall 17, he was in the stall facing west when he claims to have slipped on the right strip of rubber belting used to protect the floor. He did not report the incident until 12/31/09, he sought medical treatment on 12/31/09, we are unable to accommodate his restrictions.

Employee was removing a bolt with an impact wrench. When the bolt spun without loosening, he reached into the engine compartment to find out why the bolt wasn't coming loose. Sharp edge, out of sight in compartment then sliced through his glove cutting his right index finger.

Employee came down dust collector steps. Bottom step to ground level had uneven transition at ground level - it had been filled with crushed rock. Employee's boot slipped on bottom step to crushed rock area fracturing his right ankle.

Employee was in the process of removing right rear wheel on 9029 Loader using impact gun. Gun popped off the bolt, employee reacted to catch it, hitting the back side of left elbow on the rim of the wheel. States elbow hurts cannot fully straighten arm. Sought medical treatment.

Employee states he was partially lifting mud flaps and turning when he felt a pain in his low back and left hip area. Employee stated at clinic visit on 11/2/09 this is due to 12/3/08.

Employee was servicing 9513 pickup. He had vehicle in raised position. States he noticed the blockheater plug had come undone. He reached up to plug the cord back in said dirt fell off the outlet & got between his glasses & face entering his right eye. First responders flushed out eye but later sought treatment at the clinc. Cleaned out eye with Burr and wrote out prescrption

Uniy 9595 Pit F250 service pickup truck was run over by a water truck. The incident happened by the lower pit stand pipe, the one used for filling the 190 ton water trucks. The employee operating the pickup truck jumped from the pickup truck to avoid being run over. The result of this is a sore knee requiring medical attention.

EMPLOYEE HAD JUST FINISHED UP GRINDING ON A LOADER BUCKET. HE WAS CLEANING UP THE SURFACE FOR THE ADAPTER TO BE INSTALLED. WHEN HE FLIPPED UP HIS FACE SHIELD DUST ENTERED HIS RIGHT EYE. THE DUST HAD COLLECTED ON THE BRIM OF HIS FACE SHIELD WHILE GRINDING. WHEN THE SHIELD WAS FLIPPED UP THE DUST WENT BETWEEN HIS SAFETY GLASSES AND HIS EYE BROW AND GOT INTO HIS EYE.

Employee was removing plugs in preparation for loading blast holes. After he had removed one of the plugs, he turned on the ""chip pile"" to step down onto the roadway. As he stepped, his right ankle rolled. Employee reported that his bifocals contributed to poor depth perception as he stepped off the chip pile.

Unit 26 was replacing cracked turtle(cover plate) on 18 shovel, the shovel operator was assisting unit 26. Unit 26 noticed the operator's face /cheek had some blood on it. The EE was transported to the foreman's office and the 1st responder transported EE to E.R.

Employee was underneath Cat pushing starter up in place. He experienced pain in his back and legs started to go numb. He came into dispatch to report his incident and the guard gate was called to bring him in for medical treatment.

First load of material was dropped on canopy causing material to spill all over. Truck was tossed about as it was hit by loader bucket. Truck operator tossed around violently. Doctor stating off work until seen by own physician.

Employee took vehicle to Lundgren Motors for vehicle repair. As he exited the drivers door he slipped and fell. Doctor visit of 3/13/08 indicates hair line fracture with restrictions of 5 lbs lifting. Next appointment is 3/27/08. We will need to file an amended copy at a later date once restrictions have been removed.

While replacing wear plate on 57 loader employee was holding plow bolt in place by hand while his partner was removing the nut with an impact wrench. Plow bolt turned in place and cut gloved hand of employee. 7 stitches were required.

Employee was inside #3 secondary removing rock to find the steel that plugged it. While inside his leg was against the crusher wall and a large chunk about 1 1/2' by 6"" x 6"" slid down the rock slope hitting employee on lower right leg causing a laceration requiring stitches.

Repetitive motion cumulative trauma from years of using impact wrench caused tennis elbow as diagnosed. Received diagnosis, investigated claim, determined this is work related and investigation complete on 2/28/07.

Loading of employee's truck occurred at around 6:20 pm the first bucket contained a large chunk which impacted the truck box. Employee stated he felt numbness radiating from left shoulder to left elbow due to severe jolt caused by the chunk. Received narrative from physician on 10/8/06.

strained upper body by repeatedly lifting rear wheel locks into place on 240 ton production truck installing top wheel locks stretching out and hanging wedges on studs using one hand also replaced cylinder heads and installed kits on 9190 water truck. Employee pulled under 9028 loader with 9206 production truck. 2nd bucket of ore had large chunk on edge of loader bucket & it dropped onto top edge of truck box behind operator cab. Employee states felt headache develop & stiff neck. It appears this is an aggravation of old injury from 1999, 98, 97 & 96. He treats periodically with same chiro for same area of body.

Worker on top of eng. module on Hyd. shovel installing air cleaner assembly, switching safety harness lanyard from one eye to another. Crane swinging away from area, glove became entangled with hook as he was pushing hook of crane out of the way. Worker was suspended until glove tore. He fell to ground resulting in fracture of left wrist, and acute L2 compression fracture.

Employee installing fuel tank on 9578 pickup on hoist. Due to design of bed on the truck a straight up lift is not possible. Employee and helper were manually fitting the tank into position which is dfficult. This appears to be a recur of an old injury same body part injured - investigation completed 9/22/05 appears to be work related.

Employee was changing a ripper tooth on 9231 D11. Used a 2-wheel cart, with the bottom lip of the cart used to pry up the bottom of the tooth to aid in alignment. Once aligned, a hammmer was used to drive the locking pin into place. When the pin set, the cart slid out, causing employee to lose balance & fall back, striking head against truck. Stitches were required.

Employee was performing PM on 9228 dozer. While loosening the filter with wrench he had to reposition himself to get better position to remove filter. His foot rolled back on track pad causing sharp pain in calf muscle. Employee was restricted in activity for one day before going on vacation for a week. Received physician notes on 9/22/05.
