While inspecting a hopper for blockage, the injured turned to get further instructions from the operator. The injured was unaware that his hand dangled between the top of the hopper and the hopper door. The hydraulic hopper door automatically cycled closed and pinched the injured's hand causing a fracture to his left wrist.

While a drum was being lifted, the lid came out of the drum and struck the injured on the left arm near his elbow. A sharp jagged edge on the lid punctured his elbow requiring first aid including two (2) inner sutures and seven (7) outer sutures.
