STAR Excavation, LLC · Over-exertion (Not Elsewhere Classified)

Working off short ladder when ladder twisted. Miner trying to catch self with EE's arm on a bracing, resulted in tearing of EE's bicep. Miner missed no work or days off. Miner was put on light duty task for EE's safety. Had miner surgery to fix the problem with no complications.

Thomas Crushing LLC · Fall to the walkway or working surface

Tripped over concrete approach to step. Fell onto rock surrounding the approach. Hit knee cap causing minor injury, Went to ER. No apparent injury but to follow up with doctor in a few days. No loss of work time, only just suggested taking it easy. Placement of smaller gravel in area to avoid any similar situation.
