Struck by flying object

While swapping ends of the steel, the Breakout Wrench was attached to the steel and power applied to it when the wrench failed and an undetermined projectile struck an employee on the chin standing approximately ten feet away causing a small cut that required 4 stitches.

Over-exertion in lifting objects

BHDF temporary employee was stacking bags of barite on a pallet in the bagging area located at the Mill for approximately 30 minutes when the employee noticed that his back was hurting. The employee kept on stacking bags of barite for an additional 30 minutes and noticed that his back was still hurting and then notified his supervisor.

BHDF employee was on top of loader #30 replacing a hydraulic line on the right tilt arm at the Argenta Mine. When the employee completed replacing the hydraulic line the employee stepped down onto the back of the bucket like he has done in the past for this particular job. Then the employee stepped off the bucket onto the ground and twisted his right knee.

Employee had set the brake on a railcar. When he climbed down from the railcar he stepped in a snow covered hole twisting his ankle. There was a snow storm at the time and visability was limited.

Employee was performing a walk a round inspection when he decided to cross a patch of ice. His left leg splipped and caused him to fall to the ground. He reported pain in his left knee.

Employee was walking to his vehicle at the end of his shift. He slipped on the snow and ice in the parking lot. The employee slipped backwards and fell on his back injuring the back of his head. His head was seeping blood from a cut about an inch long. He received medical treatment at the ER. He received six stitches. The employee was given full release to go back to work.

Employee was closing mine gate, when he slipped on the snow and ice, falling on his right side. He was admitted at the ER where he received a chest x-ray, muscle relaxer shot, and pain medications during the visit. He had no fractures, only a pulled muscle. He was given full release to return to work.

Employee was moving a railcar from the loading area to couple to the next railcar in line. He climbed on the car to release the brake. When he dismounted the railcar, he stepped on the track and his foot slipped causing his right ankle to develop a hair line fracture.

Employee was installing wear shoes on the product classifier. He was driving a stuck bolt with a hammer when he hit the shoe in front of the one he was working on. The hammer glanced off, striking his left thumb, causing a hairline fracture.

Employee was climbing onto a dozer to fuel it-it was full of snow and ice. EE slipped and hit right knee.

Caught in, under or between a moving and a stationary object

At approx. 7:30 am on Fri. Sept 24, 2004, ee was working on a LeRoy compressor. As he was putting the belt guard back in place, it slipped out of place & smashed the middle finger on his right hand. He stopped work & washed his hands, but there was no cut or blood, so he continued work. Ee reported incident to his supervisor 6 am Monday morning. Upon x-ray, fracture was noted.

EMPLOYEE WAS OPENING THE FEED DOOR ON JIG #6. HE SAID IT FELT TIGHT AND HE HAD TO PULL A BIT. THEN HE REPORTS FEELING A MUSCLE OR TENDON IN HIS LEFT SHOULDER PULL. EMPLOYEE REPORTED SEVERE PAI N IN LEFT SHOULDER, LEFT SIDE OF HEAD AND NECK AND SOME PAIN IN LEFT THIGH.

EE REPORTED BACK PAIN ON MONDAY AM. HE SAID HIS BACK WAS HURTING HIS LAST SHIFT PRIOR TO THE WEEKEND BUT THAT HE HAS EXPERIENCED THIS PAIN BEFORE AND IT USUALLY GOES AWAY. HE REPORTED THAT HE DOESN'T RECALL A SPECIFIC TIME WHEN THIS PAIN BEGAN AND AGAIN REPORTS A HISTORY OF THIS PAIN SINCE HE BEGAN WORK HERE BUT THAT IT USUALLY GOES AWAY.

EE WAS USING A 3/4"" IMPACK WRENCH WITH A SWIVEL ON IT. THE PIN THAT HELD THE SWIVEL CAME OUT & CAUGHT BETWEEN THE MIDDLE & RING FINGER ON LEFT HAND. THE SKIN WAS MANGLED BETWEEN THE FINGER BUT NO MUSCLES OR TENDONS WERE INVOLVED

EE WAS CHANGING MILL PLOWS. ONE OF THE PLOWS SLIPPED AND EE GRABBED AND PULLED UP THE PART TO KEEP IT FROM LANDING ON HIS FOOT. EE FELT A PULL IN HIS SHOULDER.

EE WAS WORKING ON THE BAGGING CREW-STACKING 100 LB. BAGS FROM THE CONVEYOR ONTO PALLETS. EE TOOK A BAG FROM THE CONVEYOR AND MOVED TO THE PALLET. WHEN EE STARTED TO STAND UP HE FELT A PULL IN HIS BACK.

EE WAS CUTTING OFF ONE SIDE OF A WALKWAY-THE WALKWAY WAS BEING REMOVED-AT THE CRUSHER WITH A TORCH. WHEN HE CUT THROUGH ONE SIDE, THE WELD ON THE OTHER SIDE BROKE. DROPPING THE EE APPROX. 6 FT ON TO THE ROCKS AND GROUND BELOW. EE LANDED ON HIS BACKSIDE. HE FELT LIKE HE PULLED SOMETHING IN HIS NECK.
