March 3, 2012 NV · Metal/Non-Metal front-end loader, scraper-loader operator, pan operator, payloader, scraper rig operator HANDTOOLS (NONPOWERED)

Employee was working off of an 8'step ladder tightening bolts. While over reaching the wrench slipped and employee fell about 5' to the ground. Employee was not tied off and over reaching, and did not maintain 3 points of contact. Ladders are not work platforms. These contributing factors are what caused his injuries, laceration to his head and one broken rib.
