The employee was checking a hydraulic leak that had developed during operation. While checking the hoses, there was a pin hole that opened up spraying high pressure Hydraulic oil. Which cut the employee's RHT index finger approximately 2.5"" in length.

Slipped on ice, snow, uneven ground.

Employee was greasing the cubex drill. employee slipped and fell

Employee was walking towards the lube truck and slipped. When EE slipped, EE hit the left shoulder on the back bumper.

The employee was in LV3917, Employee was sitting in the back seat of the van. The driver hit a large hole, causing the van to bounce and the employee hit the ceiling with EE's head.

While employee was changing the feed cylinder on the cubex drill barrel, the cylinder unexpectedly lurched forward catching right hand between the cylinder and the drill cradle, trapping middle finger.

Employee stepped off a forklift onto the surface and slipped on the ice.

Employee was walking from the crew van to the underground administration office and slipped and fell. When employee fell they landed on their self rescuer.

Employee was installing an engine cover on a telehandler (30 lbs). Employee lifted the engine cover and turned upper body to place the cover into position and felt a pain in lower left leg.

EE and partner were capping off the water intake from Saval 3 to blow water out of the line to prevent freezing. They had climbed the ramp and were coming down when EE lost his footing and slipped. When EE was falling he heard a popping noise in his left ankle and felt pain.
