Miner was using pneumatic jackhammer (rivet buster) on a sidewall performing refractory demolition when the bit broke, causing the tool to slip and impact EE's left hand against the wall. Miner has laceration and fracture on left index finger.
Miner cut their hand and required stiches.

Foreign object went in right eye while driving mobile equipment. Eye became infected and required anti-biotics prescription.
Miner mis-stepped on uneven working surface and inquired their left ankle.
Miner pivoted on right leg to turn towards workbench. Their knee made a ""pop"" sound and immediately they felt pain and informed their supervisor.

While walking from B Belt up C Belt, slipped with coal on floor and twisted knee.
Employee was throwing a bag of trash into the dumpster.
Employee did the line change on the 6"" line to set up in cement Silo, then came down from Silo to inspect Kiln grease system and proceeded to clean additive system shoveling material and pain got worse. EE attempted to sit in the 320 loader, but the pain intensified. EE immediately reported it to the control room to report it to the supervisor.
Over the course of over three to four months and with increasing frequent use of grinding bowls and auto-sampler buckets, tendinitis set in to left thumb and right forearm.

Raw FK line that transports material was plugged with rock and wet material. Employee went to check line on top of electrical room. Employee then removed cap while line was pressurized. Left ear, Face and upper body impairment.
Team was attempting to seat the elevator take up sprockets back into the chain. The take up was stuck and the employee attempted to free it by striking it with a sledgehammer which accidently struck the hardened tooth and a piece broke free penetrating their left thigh. Employee was taken to Concentra and redirected to Urgent Care.
Was walking up the stairs and stepped off of and rolled their ankle.
Right hand pointer finger smashed
Contractor was rolling a conveyor chute out of the way and hand was pinched between the chute and the wall cutting EE's left pinky finger.

Hands were full, slipped off of step going upstairs. Hit left shoulder and chin (face).
While installing a coal mill top, the lid got stuck and the employee put fingers between the top and the mill and EE shook it. When the lid became unstuck, it came down and smashed EE's fingers.
While pulling blades from a separator, employee felt sharp pain in knee.

Employee was walking across parking lot and felt pop in knee. This was deemed NOT a workers comp case but then review by insurance providers and determined by laws in the state of New Mexico it was a working comp case. This is the reason for the late reporting.
While cutting an angle in a piece metal, the machine cut the metal and causing it to pop up and cut my thumb. Resulting in a laceration needing stitches to close.
Employee was found non-responsive around 5:30 pm on June 13, 2017. CPR was initiated and employee was not revived. Initial reports and indication points towards natural causes, possible heart attack. Waiting for autopsy results.
While patching a hole in the coal silo, and while coming down the ladder ee missed the last step and fell.

While working on #18 Cement Silo to ascertain the nature of a leak from behind the dust collector housing to get a closer look at the problem. As EE looked around the DC housing, approximately 2 to 3 feet from the leak, the wind changed direction and blew cement into face and eyes.
While work on #2 Coal Mill using chain hoist to pull motors back in place to tighten belts EE picked up the chain hoist and pulled on it. The chain flew toward EE's right foot and the chain hook struck right behind the steal toe and injured EE's foot little toe.
While pulling a probe, felt pain in upper back, shoulder area.
When EE pulled #2 Kiln Analyzer Probe, EE pulled something in upper back shoulder area.
The bowls used for making pellets for the XRF/XRD are very heavy and require repetitive motion with hands and wrists. Over a 12 hour shift 40+ pellets are required and pain has increased over several months.
While cleaning probe, employee tried to hold the probe in position but other employee hit injured employee's ring finger with the sledge hammer.
While adjusting a rotation switch sensor, the employee dropped the screwdriver. When EE reached to grab it, EE's finger was caught by the rotating target.
While working on airslide in dusty environment employee's glasses became foggy due to dust mask. Dust stuck to EE's glasses and obstructed EE's vision while EE was descending the ladder. EE missed a step and fell. EE bruised left hip and glute and left elbow.
Employee was stepping off of an excavator and slipped on the ice. EE hurt right ankle.

Employee was using a digging bar to clear a chute that was plugged. His middle finger was caught between the digging bar and a piece of metal part of the chute and caused my finger to break open in two places.
I went to cut back the North side skirting with the scissors and was not able to do it. I needed my left hand to cut the skirting. The pain in my right wrist was too bad.
Using welding equipment on grizzly bars and working in an uncomfortable position for an extended period of time.
This is a re-occurance of a previous injury. Employee had been released to return to full duty. Employee was tightening a bolt with a wrench and felt a sharp pain in his left elbow. When he went to lift the blade, he felt a severe sharp pain in the left elbow. Employee has been on restricted duty since July 3, 2015, but has been at work doing light duty.
Employee attempted to lift a 3'X3""it up over his head and felt a pop in his right bicep. He lowered the sheet to the floor and felt a sharp pain in his arm and it cramped. He was taken to emergency room with no diagnosis and told to make an appointment with another doctor. He was told on the 20th he would need surgery.

Employee climbed on top of the truck to grab the wheel chocks. He jumped off the rear bumper landing on both feet. He did not feel pain at the time, until 4 days later.
Employee sprained his left wrist while using a sledge hammer for a starvation on #22 gyp feeder. He was put on restrictions 10/6/14.
He was jackhammering a concrete block. He had one leg behind the other to get good leverage on the jackhammer. When he stopped and stepped back and stood up straight, his hip felt like it got stuck and felt a sharp pain in his right hip and could not walk.

Date of Accident: 2/19/11 Employee went to the doctor on 2/21/11 and he did not indicate that was a work related injury Employee's supervisor notified on 2/22/11 Employee went to our worker's comp clinic on 2/23/11 for evaluation.

Employee stepped off of stairway onto a concrete slab and twisted their ankle. This resulted in a hairline fracture to the bone.
Employee was working a routine shift in the control room when he felt he had heart burn at 4 AM. When he returned home at 7:30 AM, he looked pale and his spouse took him to the hospital. The attending physicians determined he had suffered a heart attack.

Employee lost balance and fell on to kiln anchors. Suffered lacerations to the right knee and face.
On 1/2/09, the boom from a crane bumped a concrete structure. A piece fell and struck an employee on his hard hat. Dazed, but OK he returned to work. Later, the employee reported pain in his neck and visited a Dr. on 1/15/09. The Dr recommended the employee not return to work and rest until a diagnosis is obtained later this month. His first lost time day was 1/16/09.

Employee was climbing down a ladder when he felt a pain in his back. At the time, he did not feel he required medical attention and continued to work. On 10/24/2008 he was lifting 18lb blocks, when he felt the original injury became aggravated and sought medical attention on 10/27/2008. His first lost time day began 10/27/2008 and his return to duty information is unknown.
Employee was responding to a metal detector alert on a coal belt. This belt uses an electrically actuated air jet to mark the location of tramp metal. He removed the metal piece and then went to use his radio, when the air jet fired unexpectedly sending coal dust into his left eye. Coal particle was removed by a physician and an antibiotic was prescribed.
Employee was moving debris with a skid loader. When he went to dump into the trash receptacle, wind blew back some material into his face. A small piece of metal was blown around his safety glasses and embedded itself in his left eye. Particle was removed by a physician and an antibiotic was prescribed.
Employee was moving the lids of a screw conveyor into place with a pry bar. After relocating several of these, the employee experienced pain in his right hip. Late that evening, he made a visit to the local hospital where he was told to rest. He is awaiting a complete diagnosis and has missed 4 days of scheduled work as of 6/10/2008.

Employee was striking a chute to dislodge stuck material. The material (kiln dust) released suddenly and made contact with employee?s right leg. The hot dust caused first and second degree burns to his right ankle and lower right calf.
Employee was moving an oxygen acetylene cart. One of the wheels rolled over a small piece of coal and the cart moved in an abrupt manner. Employee strained right shoulder trying to restrain movement. Attending physician gave pain medication and a sling to restrict movement.
Employee was restacking auxiliary separator fan blades. The blades shifted and pinched his middle finger on his right hand, cutting through his glove. He received stitches to this digitus medius at the local Occupational Medicine Clinic and was at work the next day.
Employee was assisting with the removal of a spring loaded support arm for a vent system. When the support arm was lifted out of its base, the spring released its stored energy and collapsed, pinching the employee's ring and little finger. Employee was taken to an occupational health center and received stitches for cuts on the 4th and 5th digits of his right hand.

Employee was removing a 6"" vacuum hose from a coupling on the Guzzler Vacuum truck. The hose had become twisted and when he pried it off the coupling, the hose uncoiled and struck the employee in the lower lip. 5 stitches were required.
Employee was assisting a fellow coworker that began to have a seizure by supporting him from behind. Victim of seizure began to fall; employee did his best to lessen the impact and injured his left wrist when both landed. Doctor issued employee brace and scheduled physical therapy sessions to lessen soreness.
Employee was reinstalling a welding receptacle that was still energized. While attempting to replace one of the bolts, he received a shock through the screw driver he had in his right hand. He has suffered pain in the right palm and has some internal burning of the muscles in that area along with inflammation.
Employee had his right hand fingers between a wire rope sling in a basket hitch configuration. When the sling was tightened to lift a piece of equipment, it pinched and twisted his fingers, severing the two middle digits at the first joint.
Employee was walking into an area where three hand grinders were being used. A piece of grinding media flew between his safety glasses and his face and lodged in his eye, scratching it.

Employee was on equipment inspection prior to start up of crusher. Slipped/tripped on the steps of the primary belt and broke right ankle. Cleanup was in progress at the time of the accident to clear the light dust and sparse rock.
Build up from the preheater dropped onto the feed shelf, which caused hot kiln dust to spill from the feed end air seal. This spillage then fell 26 feet to the ground floor.

Employee was checking the hardness of a sheet of plate steel. While making this check, employee's hand came in contact with the rough edge on a piece of plate steel adjacent to the piece being checked.
Employee was operating forklift in the plant warehouse. While backing up, the forklift hit a box, knocking it over. Employee tried to stop the box from tipping over with his right foot.
THE EMPLOYEE WAS MOVING A SOUND SCREEN AND DESCRIBED A MOMENT OF INATTENTION. AT THAT TIME HE ALLEGES HE HOOKED HIS RIGHT FOOT UNDER THE SCREEN FRAME AND TRIPPED, DROPPING HIS RIGHT KNEE TO THE GROUND.

INJURED EE WAS REPLACING A HUB LINER IN THE NO.1RAW MILL SEPARATOR, USING A 1/2"" ELECTRIC IMPACTWRENCH. WHEN THE WRENCH CORD BECAME HUNG UP THE EE PULLED UP ON THE CORD IN AN ATTEMPT TO FREE T HE CORD. THE CORD CAME FREE AND THE PLUG AT THE END OF THE CORD STRUCK THE EE IN THE TOOTH.

CO-WORKER FELL ONTO I NJURED EMPLOYEE'S RIGHT KNEE. INJURY BECAME ""LOST TIME"" BEGINNING 11/28/01 AFTER EMPLOYEE'S PHYSICIAN DETERMINED EMPLOYEE REQUIRED TIME AWAY FROM WORK TO RECUPERATE.
WHILE PERFORMING MAINTENANCE ON A CRUSHER, EMPLOYEE WAS USING A 4'PRY BAR TO ROTATE THE CRUSHER HAMMER. AS THE HAMMER TURNED OVER, EMPLOYEE'S HAND WAS CAUGHT BETWEEN THE PRY BAR AND A CRANE HOOK.
EMPLOYEE WAS STANDING ON THE GROUND NEXT TO AN ELEVATED WORK PLATFORM. A SECOND EMPLOYEE WORKING ON THE PLATFORM WAS USING A HAMMER TO AID A HYDRAULIC PRESS IN MOVING A LARGE COAL MILL PLATE. THE SECOND EMPLOYEE SWUNG THE HAMMER, WHICH GLANCED OFF OF THE PLATE AND HIT THE INJURED EMPLOYEE ON HIS HARDHAT.INITIALLY NOT REPORTABLE.RECENTLY EMPLOYEE RECEIVED MEDICAL TREATMENT FOR NECK.
EE WAS OPENING A DOOR ON THE SIDE OF #1 KILN PREHEATER WHEN HIS FINGER WAS CAUGHT BETWEEN THE DOOR AND THE SIDE OF THE PREHEATER.
EE WAS REMOVING TOOLS FROM A HOISTING CAGE AFTER COMPLETING A JOB. HE LEANED OVER THE HANDRAIL TO RETRIEVE THE 5 GALLON PLASTIC BUCKET OF TOOLS (APPROX. 10-15 LBS.), LIFTED IT AND TURNED TO SE T IT ON THE GROUND. AT THAT POINT HE FELT A PAIN IN HIS LOWER BACK.
EMPLOYEE ATTEMPTED TO MOVE A PIECE OF PIPE RESTING ON THE POWER HACKSAW AND A SUPPORT STAND. THE PIPE ROLLED LENGTHWISE TOWARD THE SAW, AND EMPLOYEE'S HAND WAS CAUGHT BETWEEN THE END OF THE PI PE AND THE SAW FRAME.
EE WAS PERFORMING AN INSPECTION IN THE COAL RECLAIM A BELT CONVEYOR AREA. A PIECE OF COAL WAS LODGED IN BETWEEN THE CONVEYOR BELT AND DRIVE-OVER HOPPER SKIRTING. EE WAS HITTING THE UNDER SIDE OF THE CONVEYOR BELT AND A RETURN IDLER, PULLINGHIS HAND IN BETWEEN THE BELT AND IDLER.
EMPLOYEE ENTERED CLINKER COOLER IN THE AREA ABOVE THE GRATE TO MAKE REPAIRS TO GRATES ABOVE THE #2 COMPARTMENT. A COMPRESSED AIR NOZZLE IN THIS AREA DISCHARGED, CAUSING EMPLOYEE TO COME IN CON TACT WITH HOT CLINKER ON HIS EAR, NECK, AND THROAT.

EE WAS CLEANING UP A PILE OF DUST ALONG #3 CLINKER BELT IN THE AREA WHERE THE FLOOR BEGINS TO SLOPE TOWARD THE FEED END OF THE BELT. AS EE WAS LIFTING A LOADED SHOVEL, HIS RIGHT FOOT SLIPPED O N SOME LOOSE CLINKER IN THE PILE. WHEN EE SLIPPED, HE TWISTED HIS RIGHT FOOT AND KNEE.

EMPLOYEE WAS ATTEMPTING TO UNPLUG A BUCKET ELEVATOR FEED CHUTE USING A STEEL BAR. EMPLOYEE'S FINGER WAS CAUGHT AND PINCHED BETWEEN THE BAR AND THE ELEVATOR HOUSING.
EE ALLEGES THAT WHILE UNLOADING 1-1/2"" CONDUIT FROM A FORKLIFT, HE INJURED HIS BACK.
EE ALLEGES THAT INJURY OCCURRED WHILE HE WAS DESCENDING THE STAIRWELL FROM THE 5TH FLOOR OF THE MILL BUILDING. EE STOPPED TO OBSERVE MILL OPERATION. WHEN HE STARTED AGAINS, HIS LEFT FOOT BECAM E STUCK ON THE FLOOR GRATING AND IN TURNING RIGHT, HIS LEFT KNEE ""POPPED"". EE CONTINUED TO WORK UNTIL 5-25-00 WHEN TREATMENT FOR HIS KNEE & ASSOCIATED SORE BACK REQUIRED TIME OFF WORK.
EE WAS REMOVING (CUTTING) TAPE OFF OF A MOTOR ELECTRICAL WIRE. DURING THIS ACTIVITY, EE TRIED TO GRAB A RADIO MICROPHONE THAT BECAME DETACHED FROM HIS SHIRT AND CUT HIMSELF WITH THE KNIFE HE W AS USING.
EE WAS STEPPING OFF OF A COAL MILL BEING REPAIRED AND TURNED TO ACCESS A LADDER. DURING THE TURNING MOTION, EE INJURED HIS LEFT KNEE.
EMPLOYEE WAS MOVING A 160 LB CLINKER BREAKER HAMMER OFF OF A PALLET TO HOOK UP TO THE SHOP CRANEEMPLOYEE PULLED LEFT BICEP MUSCLE AT THE SHOULDER.
