The Fane truck driver was unloading a trailer full of clinker. As he was raising the 30' box it became unstable at half it's height position and tipped both box and truck onto the driver's side. NOTE: LEHIGH NORTHEAST CEMENT CO WAS NOT AWARE, UNTIL RECENTLY, THAT THIS INCIDENT RESULTED IN INJURIES THAT WOULD HAVE MADE THIS MSHA REPORTABLE.

The injured is a Temporary Laborer. While pulling pipe for the vac truck apart, the plastic pipe bounced and hit the Temp employee in the mouth, cutting his lip and requiring stitches.

Employee was working in slag dryer DC fan shroud with concurved base, as he was inside he threw out a piece of steel ring he had cut from the fan plenum. As he threw object employee slipped on the incline in the fan housing and fell against the housing hitting his right side; shoulder, ribs, arm and a small laceration under his right eye.

Lifting corner of bridge crane with 50 ton hydraulic cylinder (port-a-power) to remove cribbing from under crane. As the crane was being lifted, the cylinder was ejected from under the crane and hit employee in chest. Contusion to chest, heart, lungs, spleen, laceration to liver and broken rib(s).

Employee was using a chipping bar to clear chunks from the Feeder.As employee was poking a chunk he was flipped by the bar as the chunk fell onto the pry bar. Employee fell and struck upper right side and thumb on the mid-rail of the floor handrail system.
Two employees working on seized pump. Employee locked the pump out, unseized it; took off the lock and reset it. Pump seized up again so the employee instructed his partner to turn the pump back a second time with a 4 foot pipe wrench. The pump started and the wrench flew out of the partners hand and hit the employee in the right forearm.
Walking around Feeder, employee allegedly stepped on a piece of flooring and lost balance, falling and striking his left arm, elbow and shoulder.

Employee was inspecting the Packhouse Incline Belt. Somehow the employee's glove got caught causing his hand to be caught between two rollers. The employee recieved an abraision to the top of his left hand.

THE EE WAS REMOVING CHECK VALVE FROM SILO 51 PURGE AIR LINE, WHEN CEMENT DUST CAME BACK INTO HIS FACE, UNDER HIS SAFETY GLASSES.
THE EE WAS ON TOP OF THE #1 MILL PULLING THE DOOR OUT WITH STRAPS ATTACHED. AS THE DOOR WAS BEING LIFTED BY THE CRANE, A BOLT ON THE DOOR CAUGHTON THE MILL. THE CRANE CONTINUED LIFTING, THE ST RAP STETCHED AND SNAPPED, AND CLEVIS RELEASED UNDER PRESSURE, STRIKING THE EE'S LEFT HAND.
WHILE BLOWING OUT THE SEPARATOR, THE EMPLOYEE GOT DUST/ FOREIGN BODY IN RIGHT EYE. NOTE: 8/21/03 WAS THE FIRST TIME THIS OFFICE WAS NOTIFIED OF INJURY.
THE EE WAS HELPING TWO OTHER EE WHO WERE COME A LONGING A LOADING SCOOP INTO A LOADER BUCKET. THE EE SAID THE SCOPE GOT CAUGHT ON THE BUCKET LIP. HE TRIED TO FREE THE SCOPE. WHEN HE DID, THE S COPE, WHICH STILL HAD TENSION ON IT, POPPED LOOSE AND CAUGHT HIS LEFT HAND POINTER FINGER BETWEEN THE BUCKET AND SCOPE.
