Lifting drive sprocket and lost grip. Hand was pinned between sprocket and adjustment bolt. Fatty portion of hand was pinched and slit open. PPE was worn to include gloves. Miner received stiches and no time was lost.

Miner was standing on a box attempting to locate a part. As EE stepped down the box caved causing EE to fall back. EE's head struck the floor. EE's initial report of injuries was pain in neck and lower back, along with a laceration to the back of EE's head. EE was transported to Atrium Health for scans and X-rays. Miner was cleared with 4 staples and released for work.

Cutting with a torch, using a grinder, welder, and placing T-Bolts inside Kiln 7 cooler. Debris got into the maintenances workers eye. The maintenance worker noticed the eye irritation around 08:00 pm after being home for a few hours. Most of the work was performed overhead.

Weather was clear about 65 degrees. EE was moving 3 steel plates measuring approx. 2'x2' 1/4"" thick. Weighing approx. 10 lbs. according to EE. EE stated that EE had tossed them to the side and then felt a ""burn"" in EE's lower back shooting down EE's leg when EE started walking.
EE stated, EE & two other workers were working in the throat area of #3 cooler when a piece of castable fell from the kiln area above approximately 7 1/2 feet and struck EE on the lower left side of EE's back. The piece of material was approximately the size of a 16.9 ounce water bottle. Not sure where the piece of castable fell from.
Weather: clear, no rain. Miner was walking on the walkway between kilns 1/2 & 3/4 and was going up 3 steps and pulled self up with the handrail & felt something pull. EE didn't think anything was wrong but as EE said the pain progressed. So, EE went to the doctor and was diagnosed as a sprained tendon or tear. Has not missed any days for EE's shift.
Employee went up a fix ladder to close a water valve, coming down, at the bottom the employee step on clinker (rock) and rolled their ankle.
IE and co-worker went up to the 3/4 stacker feed belt to get material off due to it being overloaded. After they were done IE went to step off onto the ground and when IE did IE's foot caught the side of a rut and their ankle twisted due to the ground being frozen.
EE went to the baghouse on #5 due to the duct work being stopped up. As EE was coming out of the duct work to get back in the man lift, the handle caught a piece of metal which caused it to open up releasing the dust into EE's eye.

Employee was cutting a few feet off a hose to replace a section on a waterline that was leaking. While doing so employee went to cut the piece and that's when they cut their middle finger on the tip of the left hand.
Employee reported they were in the man-lift and man-lift basket got stuck on a piece of concrete. When the basket broke loose from the concrete it knocked them off their feet and they hit their head on a piece of steel causing a laceration on forehead. They report wearing their hard hat. Employee was taken to the ER for evaluation, they required stitches to the forehead.
EE went to number 6 dust seal to attempt to clear it out due to it being clogged. Within 30 seconds or so of beating the chute dust broke loose causing it to start flowing freely. Before EE had time to react, a small hole above EE's head allowed dust to get on EE.
EE walks into the baghouse to see why the auger wasn't working. EE then walks back to the motor and sees the cover where the fan is looked like it had been beaten to death causing the fan not to run due to obstruction. EE was pulling on the belts to get the shaft to turn and that's when the motor kicked on causing EE's hand to get caught in between the pulley and the belt.

Employee reported employee had slag in left eye from working beside someone polishing boiler tube. Employee reports employee was wearing safety goggles. Employee presented to urgent care to have eye flushed. Employee now reports no discomfort to eye.
Performing a weekly task the employee grab a handle, the handle broke causing the employee to striking EE's right hand.
While on the lower pier EE was holding the chain from the crane that was wrapped around the housing. While doing so the chain rolled causing EE's hand to roll with it, causing EE's hand to get caught between the chain and the housing.

Employee was leaving the electrical building heading towards number 6 ID fan. While walking across the loose gravel, EE remembers stepping on a rock and feeling a pop in the right knee. EE then tries to put weight on it and move it around but EE couldn't because of the discomfort.
Went to 3&4 baghouse to start the lime pump. Employee called for assistance when employee wasn't successful in starting the lime flow. The supervisor came in to see there was no lime flow and the pump was not energized. The supervisor started the pump. The helper was standing at the pressure end of the pump, the hose failed, spraying the helper with lime.
Not sure, believes while cleaning around 3 and 4 thrust roller on upper pier.
While checking a slow overflow chute on the No. 2 rock silo, the employee placed this hand on the floor to look under a conveyor. A rock from the overflowing chute fell and struck the employee on the finger. The rock made contact with the finger just behind the fingernail at the joint.
Employees not being aware of coworkers location during plant feed of hot materials. The employee, not making other aware of their location, was burned by steam from materials that would have not been fed into the hopper if coworkers would have been made aware that EE was in tunnel.
Traveling a path without clearing it first, and doing so with hands full of materials.
EE was tightening the fasteners on the black plastic pipe (48""diameter) for the #1 kiln stack. EE had 7/8"" wrench in left hand and a 1/2"" impact with a 3/4"" socket in right. When they pressed the trigger on the impact, the nut caught a burr on the bolt threads and the wrench spun out of hand and slapped EE across the knuckles.

Employee was exiting the break room through the side door. When the employee open the door, EE then held the door to close it. EE smashed EE's finger between the door and the door frame.
Employee was mounting and dismounting on the 20 ton Grove Crane placing lifting cables onto the pallets of castable material. EE was mounting using the grab irons hand rails. As EE was mounting EE felt EE's shoulder pop.
The injured employee along with two others were locking down large threaded nuts on the coal mill roller journal assembly, using a large open end wrench and a 10lb sledge hammer. The injured EE was holding and adjusted the wrench when the hammer hit EE's middle finger.
Employee was transferring oil from one gearbox to another using an oil transfer pump. When pulling the hose out from the inlet side of the pump, they felt a pop to right arm.
Employee was standing inside the #1 Kiln weight room and reached up to grab a drink off the shelf and felt soreness and pain under right arm and upper back on right side.
Employee was spinning the # 3 Coal fan blade with right hand to see if it was free from rubbing anywhere. While spinning the fan, right little finger got caught between the fan housing and the fan blade.
Employee was operating a JLG man lift. EE had just emptied some kiln leaf seals from the basket into the dumpster. Traveling back to the job site, they ran over the cut out trench that was designed for water drainage, and when the JLG accelerated their feet lifted from the basket platform. When they came down, chin hit the control panel bar. Also feet are bruised.
I was pushing the top of the fork on the telescopic handler sliding them closer together. The fork stopped suddenly jamming my left thumb.
Employee was traveling in man lift basket. Employee hit a pot hole, they lifted from the man lift basket platform. When they landed, they twisted their left knee.

Employee was trying to position a sprinkler head on a conveyor using zip tie straps. Seeing that EE had positioned the sprinkler head incorrectly, EE was going to use a utility knife to cut the zip tie. Sometime during this task, EE cut self.
Our construction crew was removing trunnion roller housing caps to inspect the inside for wear. A foreign body material got behind employee's safety glasses into the left eye.
Employee climbed steps to the 1/2 and 3/8 belt platform to get a rock sample. When EE stepped onto the platform, EE rolled EE's right ankle and had to catch self.
Employee was walking down the stairs. When EE stepped to ground level EE twisted EE's left ankle.
Employee was washing off the 5, 6, 7, kilns cooler deck. Fine debris was airborne. Debris got behind their safety glasses into an eye. Employee couldn't remember if they had rubbed the eye.
Employee was using a hand held grinder on the seal flange on the # 4 feed end of the Kiln. A lot of material was airborne. Airborne material got into EE's shield and got past the safety glasses and into EE's eye.
Employee was attempting to remove a coal mill roller from its journal using a 5 pound sledge hammer. Not being successful, they then began using a 10 pound sledge hammer. On about the 5th swing, they lost movement to their right shoulder.
Employee was attempting to mount a aerial man-lift. As EE was stepping up holding on to the handrail, the other leg slipped on rocks out from under EE. Holding on to the handrail the sudden jerk to EE's arm, strained right shoulder.
Employee was pulling out a rolled 3 inch by 20 foot rubber hose from a wooden crate. Employee stepped backwards and tripped on another hose. They fell backwards to the ground.
Employee was removing the V-Belts from the motor sheave on the #7 dust auger. EE wasn't aware the sheave had a sharp edge, it cut EE's fingers
Employee was walking out from the breezeway hall, where the weight room is located underneath the 3 and 4 Kilns. As EE stepped down to a lower concrete level, EE twisted left knee. No debris in the area.
Employee was performing maintenance on a dust pump. After replacing a butterfly valve and an auger bearing, the system was activated. The pump wouldn't discharge so the employee pressed the power switch again, the pump discharged sending fly ash out through the sliding hopper gate. Fly ash entered around the employee's safety glasses into eyes.
Employee was removing 3/4 threaded nuts from the screens rails. One got hung in the 3/4 Impact wrench socket. EE put left index finger into the 3/4 threaded nut, then accidently pushed the Impact wrench trigger. It rotated onto the tip of EE's finger
Employee was using the stairs to the 5/6/7 Kiln control room. Employee struck hand against the handrail leading onto the stairs. They felt a pop and a burning sensation in the left hand / wrist area. It started swelling over the next few minutes, so we wrapped the hand into a ace bandage.
Employee was grinding on a flange on the #4 rotary kiln ring gear. Some dust got into EE's right eye. EE rubbed the eye probably imbedding the material from the dust. Later that day EE's eye became irritated.
Employee was trimming the rail rubber with a utility knife so the wire mash screen could clear and go underneath the lip. The knife slipped and cut their left hand.
Employee was working from a 60 foot Telescopic Handler, unloading a piece 36inch by 80 inch 2x4 bar grating. As ee was sliding the bar grating onto the preheater platform, ee felt a pop in right shoulder.

Employee was trying to remove a nylon coupling from a hydraulic pump with a small pry bar. Bar slipped from employees grip hitting EE in the lower lip causing a laceration requiring 7 sutures. Employee returned to work the following day with only restriction was to keep injured area clean.
Employee was using a 55ton hydraulic jack and a 6 inch I Beam to reline a rotary kiln hood. The jack slipped out of place, striking the employee to the face.
Employee was clearing lime dust material from a hopper. EE had stepped back while the hopper's vibrator was energized. Suddenly the material inside broke loose splattering EE in face and body area. Apparently some material got behind EE's safety glasses into eyes. EE may have rubbed eyes before using eye wash.
Employee heard 3/4 silo belt lock down, ee went to the control room to turn it off. EE then went to investigate what was wrong, ee found some rock had wedged and locked belt down. EE went to get a ball pin hammer. As ee was returning to the conveyor, ee stepped onto uneven ground, ee lost balance and fell against the silo.
Employee was walking carrying two buckets of castable material, to be used to patch holes inside the #1 rotary kiln. Employee stepped onto uneven ground twisting the right ankle.
Employee exited the shipping department control room using the stairs. At the bottom was some more concrete stairs, when EE stepped down EE rolled EE's ankle.
Employee was hooking up the welding trailer using the hydraulic lift from the tractor. Positioning the tong of the trailer and using the controls from the tractors hydraulic lift at the same time, the tong receiver on the trailer slipped off the ball hitch smashing EE's index finger.
Employee was cutting metal using an Oxyacetylene torch. It appears some rust was behind the metal employee was cutting, hot molten metal splashed back into employee's face. A small piece of molten metal worked its way behind employee's cutting goggles and attached to employee's eye.
Injured employee was helping change out a roller on the #1 kiln. The wind was blowing hard yesterday. EE felt something in right eye, like it was scratched. That afternoon eye became sore, ee seen a speck of something on eye. EE tried to remove it but was unable to. EE reported this to supervisor the following morning.
Employee was walking around the rear of the articulated front end loader, inspecting due to the loader over heating. EE slipped on the snow/ice strain EE's left knee.

The employee was replacing the wheel chocks back onto the 644 J John Deere articulated front end loader, using a one inch rubber bungee strap to secure them. As ee was pulling on the strap, the anchor side of the strap came unhooked striking the employee at the temple of face.

The injured employee was walking to turn off the augers power switch. Employee twisted employee's ankle when employee stepped over a 6 inch steel pipe.
EE was weighing in and out customer trucks. EE began to have chest pains, then later numbness to EE face and hands. More so to the right side.
Employee was cleaning fine material from underneath the #4 discharge conveyor. EE was using a shovel to push the material into the flow of water from a water hose. As EE was pushing the material, EE felt a sharp pain to the lower back.

The injured and another employee was cleaning out a lime slurry tank. The steel rod that they where using got stuck inside the tank. As they where pulling the rod out lime slurry started to flow. They where closing the gate valve, lime splashed into injured employee's eyes.
Employee was working on cleaning out lime from the #2 lime tank, using a pressurized water hose, also was opening and closing a 6 inch gate valve. They said that their shoulder got a pain in it that evening and they just thought it was arthritis, but the pain was still there after a couple of days. So they thought they should report it.
Employee was cutting a piece of rubber with a utility knife. Employee accidently cut hand.
Employee was installing a grease O-ring seal to a rock crusher. The O-ring slipped off from the back side where employee started. Employee fell forward to the edge of the brass socket liner.
Employee was sitting at EE's desk. EE was turning EE's head to the right as EE was standing up, EE hit EE's right forehead on the corner of an electrical panel box.
The injured employee was carrying a guard up a stairway. EE raised one end up an felt pain to right shoulder.
Employee was attempting to install a coiled spring into a collar that holds a bit of a small jackhammer. EE almost had the spring inserted, but was having a problem with the spring going in all the way. EE removed right hand glove to push the spring into place. The spring embedded into finger.
Employee was shoveling off fly ash from the top of 1 and 2 dust tank. The employee alleged that something popped in their lower back when they lifted a shovel of material.
The injured employee and another employee was erecting a 30 foot conveyor into place. The conveyor was suspended with chain and cable hoist. The conveyor needed to shift to the left a couple of inches. The injured employee had thumb in the area where IE believed it was hung up. The conveyor unexpectedly shifted forward pinching IE's thumb between the chute and conveyor.
Employee was climbing down a ladder, near the end of EE's descent EE's foot slipped off the ladders rung causing EE to twist EE's ankle.

Employee was carrying a bucket to gather some rock from a conveyor for his hourly sample. He stepped down into the discharge belt conveyor area. The employee didn't look where he was stepping. He stepped on a clinker causing him to twist his ankle.
The injured an another employee was replacing the bearing on the drag chain sprocket. The sprocket was suspended, as they was positioning the sprocket into place. The sprocket moved forward wedging the injured employee's thumb between the sprocket and drag chain compartment.
Employee went to the warehouse to get a wire cloth screen. He pulled the screen from the rack. He leaned the screen against the trailer. He was lifting the screen lost his grip, the screen was falling toward him. He reached quickly to catch the screen. He felt a pop slight burning between his shoulders.
Employee was inspecting the #4 kiln pinion gear. He stepped from one level of concrete to a lower level, planted his foot wrong twisting his left ankle.
Employee said that he exited the #1 and #2 Kilns control room to the # 1 kilns firing deck. He felt a twitch or pinching feeling to his left knee. He returned to the control room. When he would put pressure to the heel of his left leg, he felt sharp pains to his left knee.
Employee was washing material off the concrete in the 5/6/7 discharge conveyor area. Something got into his eye.
The employee cut a 3/4 inch hose to length. Then he was making a vertical cut downward splitting the hose lengthwise. Holding the hose in his hand he cut through the hose his glove and the palm of his hand
The injured employee used his left arm grabbing the steering wheel to mount the rubber tired tractor. He felt a sharp pain when he pull himself up.
Employee hooked the crane cable block into the lifting eye of the conveyor belt carriage. When the crane operator began cabling up, employee seen the cable block drifting toward a 55 gallon oil drum. He pushed on the block with his right hand. The outer cable caught his right thumb, pulling it into the cable block.
Employee was using a parts puller to pull a coupling hub off a shaft, he said it was tight. He was using an adjustable wrench to turn the treaded shaft. He was using repetitive motion to remove the hub. After removing the hub, he turned and felt a sharp pain to his neck.
Employee stepped off the #4 kiln coal mill 5 inch high concrete platform, to retrieve a welding lead. He planted his foot on the edge of the 5 inch high concrete platform and twisted his ankle.
The employee exited his vehicle and was walking to his work area. It had been snowing for several hours. He stopped to make contact with the front-end loader operator working in that area. He took a couple more steps lost his balance and fell.
Employee stepped out of the 3&4 control room onto the #4 side kiln firing deck. Upon stepping, some grit blew into his eye. He stepped back into the control room and removed his safety glasses, then rubbed his eye. The grit scratched his eye when he rubbed it.
The employee was using a hammer and punch to remove the cut bolts from the connecting ring on the ID fan stack. A piece of the punch chipped off the top cutting his hand. At the time of the injury, the employee didn't realize that a piece of the chipped punch was in his hand!

# 7 Rotary kiln was taking down with a problem to the drag chain. The injured employee was washing material away from the drag chains drain chute. The maintenance man dropped his flashlight, he ask the injured employee to toss it back to him. the light hit the wall fell onto the conveyor belt. The injured employee reached for the light, tripped on a plate that divides the tunnel.
Employee was squatting down washing underneath the tail pulley on the 3 deck screen conveyor. When he raised up he felt a pop in his lower back. He had tingling to his groin and both legs.

Employee was cleaning up after a maintenance job on the # 6 baghouse. He had bent down to grab the welding and ground leads and a cutting torch. As he proceeded to move forward and began to stand up, he rammed his head into an air pressure receiver pipe that was in clear view.
EE and another ee where changing the oxygen bottle on a portable welder trailer. The other ee set his end of the tank onto the trailer. As ee lifted his end to stand the tank upright, the other end slipped of the trailer, jerking ee left shoulder.
Employee was walking toward the silo talking to the loader operator on a hand held radio. He stepped on a rock, twisted his ankle. He caught himself before he hit the ground. He stood up real quick, he felt something pop in his lower back.
Employee was exiting the cab of the water truck. He hold the steering wheel with his left hand and the grab iron with his right, with both feet on the top step. He placed his left foot on the lower step, as he lifted his right foot his left foot slipped of the step, allowing his right foot to be caught between the cab and first step. With ee falling backwards to the ground.

EE was watching his Rotary Kiln the wind blew fine gritty material underneath his safety glasses scratching his left eye.
A couple weeks ago he had a crick in his neck. It went away but stayed irritated up until 08/14/12, this is when he reported to his supervisor that his neck was pain. The loader pins are worn causing the bucket to jolt his neck back and forth. The loader was taking out service on the 15th an the bucket pins replaced.
The employee was using a sledge hammer to clear dust from a hopper. He believes he strained himself swinging the sledge hammer.
The injured miner was replacing a trust roller on the #1 kiln upper pier. The wind was blowing foreign materials around, when the miner wiped his brow, a piece of foreign material got into his eye.
The injured miner was preparing to change a trust roller on the #1 kiln upper pier. The miner was clearing material away, and the wind blew foreign material into his eye.

The employee was helping another employee carry a woven wire screen. As they were setting the screen down, the edge of the screen rubbed the injured employee's elbow cutting him approximately four inches.
The EE was welding inside the loader bucket, using a LN25 wire welder. That Friday afternoon his eyes were sore, by Saturday evening his eyes became worse and began to swell.
Employee's were working inside the #2 Kiln cooler system. The employee that was injured had his arm over one of the cooler flight, he was tightening the bolts that holds the cooler flight in place. Another employee placed a cooler plate onto the flight and pushed in into employee's left hand.

The employee was replacing the maintenance service door on the #1 coal mill, as he was tightening one of the treaded bolts, it broke causing the employee to suddenly lose his balance. He regained his balance a short time later he felt a sharp pain to his lower back (Lumbar area).
The EE had gathered a five gallon bucket of stalite material and emptied a five gallon bucket of coal. Minutes later he was walking to the EE's break room and felt a pain to his chest area and he developed a cold sweat. He thought he was having a heart attack.
EE was using a small jack hammer to remove the castable material from the #5 firing hood. A piece of castable material fell and hit the top of his left foot.

EE and a contractor EE where working from a bucket truck erecting an induced draft exhaust stack on the #7 kiln. On the decent the bucket truck boom moved suddenly giving EE a jolt.
EE was walking to inspect the clinker tunnel conveyor. The area which he was walking was wet from a leaking water pump. The wet area had grown some algae and no one had noticed the slippery spot. The EE walked through this area and slipped and feel to the concrete pad.
EE's were working on the 6 and 8 ""middle deck"" of the three deck screening tower installing new screens. The EE that was injured was intending to use an electric impat wrench to tighten the 3/4 inch bolts that holds the screens in place. The other EE tossed an electrical extention cord to the EE which was injured when one end of the cord struck him at his upper lip.
On Monday 3/9/09 maintenance crew was doing repair work in one of our kilns. At this time no accident had occurred. On Tues morning the employee woke up with a crick to the right side of his neck. He alleged that the crick in his neck came from work being performed at the Carolina Stalite plant. He didn't report it until 3/16/09. Sent him to the dr. the same day.

Employee closed the sliding glass door on the 20 ton crane. A small piece of foreign material got into his right eye. He rubbed his eye which most likely embedded the material.
Employee was using a ball pin hammer and the concrete floor to straighten out a bent retaining collar ring made of metal, that is a part of the assembly of a Dodge speed drive reducer. Employee believed he struck the edge of the collar sending the collar directly toward him striking him above his right eye. The employee was wearing his safety glasses.
Our EE was inspecting the lower pier trunion rollers on the #4 Rotary Kiln. He alleged an approimately 60 lb metal heat shield that was propped against the handrail, fell against him, knocking him to the walkspace. As he pushed the hot metal shield off of him, it caused him a lumbar strain.
Employee was dumping a load of coal from approx. 30 gallon drum into a front end rubber tired loader bucket. He pushed the drum into the loader bucket then reached and pulled it out. This is when he believed he strained his chest area.
EE was helping maintenance pull cages out of the cloth bags that are a part of our dust collector system. He was pulling on this particular cage, when the cage became stuck. EE slid a screw driver insided the top of the cage to give himself some leverage. EE was pulling down w/his legs. Heard a pop and sharp pain to his lower right abdomen.
The EE exited the #1 and #2 control room to wash down the #1 kiln firing deck, when the EE knelt down to wash between the kilns firing hood and the control room , he said he heard amd felt his knee pop.

EE was lubricating rotary kiln equipment. Around 6:30 he had dinner, then around 8:30 ee & co-worker met with their supervisor in the control room. He said he felt hot, so he took off one of the shirts he had on & stood outside of the control room to cool off. His supervisor went with him to the breakroom to relax for a while. He started having chest pain, another ee called 911.
Two employees were moving a 7' X 3x2 metal box to a new location inside the welding shop. They were using a two wheeled hand cart to do this task. As they were moving the box, all the weight shifted back onto injured employee's left arm that was against his chest, which was pinning him against the wall with all the weight of the metal box.
EE was helping another employee change cooler plates inside the #6 kiln cooler system. EE was located in the lower compartment cutting a T-Bolt out using an Acetylene cutting torch. During this process he has his head slightly tilted, allowing a piece of hot molten metal to enter his left inner ear.
Employee was operating a Cub Cadet tractor. He traveled underneath a coal conveyor, a piece of foreign material got inside of his safety glasses and entered into his left eye. He rubbed his eye imbedding the foreing material.
EE was welding inside the welding shop on a piece of equipment. A hot piece of molten metal fell on top of his work boot. It laid on one corner of his boot, then burned through the fabric causing 2nd degree burns to his toe area before he was able to remove his boot.
Inj EE and 2 others, were attempting to remove a harden sleave from a shaft on kiln cooler system. Inj EE was heating sleave with torch, the other EE's were prying on the sleave with a crow bar. The pry bar slipped and struck inj EE on the bridge of his nose.
EE was attempting to lift up an approximately 4x8 sheet of flat steel plate with a 2x4 board. As he began to left the steel, he twisted and felt a burning sensation in his lower back. **On June 22nd the physician put ee on restricted duty for a lumbar strain.**
Employee was using a hammer drill to place anchor holes in concrete. The drill bit became bound causing the hammer drill to twist in his right hand. The employee was looking into the #3 rotary kiln, when a foreign body material blew back & stuck to the employee's left eye (cornea area).
EE was shooting a ring out of the #6 Rotary kiln. A piece of metallic material ricochet back toward him penetrating into his left shoulder.
Employee was struck in the forehead by a 24 inch conveyor belt hanger that was left in a vertical position.
One of our employees was having trouble putting v-belts on the auxiliary unit. EE stepped down from a two foot concrete platform that the auxiliary unit is mounted to. When EE stepped down he stepped into a trough also made of concrete. He stepped on the edge of the trough & twisted his left ankle.
EE was dismounting the caterpillar 980-B frontend loader. EE claims he had mud on his boots that caused his foot to slip off of the top step of the loader he fell to the ground he hit his knee when he fell.
EE was welding in the #1 kiln, there were some metal angle braces welded inside of the kiln where he was working. He leaned over to remove some slag, his welding shield started to fall off his head. He raised his head backward, when he did he hit his head on the metal braces.
Process of rolling a tank to reposition. EE was told to move his chock since it was loose. There was a manway in center of the tank so had to put the forklift on the other end of the tank to releive pressue from bound end to remove that chock. At the same time the bound end had been freed the forklift was backing off tank to reposition. Tank rolled & pinned EE R Leg.
Employee was in a squatting position washing down a concrete pad. As he was backing up with the hose, he slipped on some stalite material and the hose came out of his hands. The high pressure hose whipped back toward him. The high pressure water hit him on his left cheek.
EE was pulling on a torque wrench, he was torquing the head bolts down on a crusher head. When he pulled on the wrench he felt a twinge to his left upper chest muscle.
EE was washing off the #5 kiln concrete firing deck. A piece of flat bar metal was leaned up against the kilns doors. Vibratioin caused the metal to fall and strike EE on his upper right calf.

EE WAS CLEANING OUT SOME DRY LIME CLOTS OUT OF THE #7 KILN DROP BOX. THE WIND WAS BLOWING TOWARD HIM AS HE WAS CLEANING OUT THE BOX. THE WIND BLEW SOME LIME INTO HIS RIGHT EYE.
EMPLOYEE WAS LOADING FLYASH WITH A SMALL AMOUNT OF LIME INTO A FRONT END LOADER BUCKET. THE MATERIAL CAME OUT OF THE STORAGE TANK ALL AT ONE TIME CAUSING THE ASH TO SPLASH OUT OF THE LOADER BUCKET. SOME OF THE MATERIAL GOT INTO EE'S RIGHT EYE.
EE WAS HELPING SHOVEL ROCK OUT OF THE #2 KILN DRAG CHAIN COMPARTMENT. AS HE WAS EXITING THE DOOR, HE ALLEGEDLY SAID HIS KNEE WAS ON THE DOOR JAM. WHEN HE PULLED HIS LEFT LEG OUT OF THE DOOR HIS RIGHT KNEE SLIPPED OFF THE DOOR JAM AND HIS KNEE POPPED.
EMPLOYEE WAS CUTTING AN 8""I-BEAM WITH AN OXYGEN & ACETYELENE TORCH. WHEN HE HIT THE OXYGEN IT BLEW HOT METAL SLAG BACK INTO HIS FACE. HE DID HAVE HIS CUTTING GOGGLES ON, THE HOT METAL BURNED HIM. HE REMOVED SOME OF THE METAL & DEBRIS HIMSELF ALONG WITH THE SUPERVISOR & THEN WAS TREATED AT THE HOSPITAL. EE WAS RELEASED BACK TO WORK THE NEXT MORNING.
EMPLOYEE WAS GOING TO TURN THE HIGH PRESSURE WATER LINE OFF. HE LET GO OF THE NOZZLE TO TURN THE VALVE OFF, HOLDING THE WATER HOSE WITH THE OTHER HAND. WHEN HE LET GO OF THE NOZZLE IT WHIPPED BACK & STRUCK HIM IN HIS FOREHEAD & NOSE.
Co-worker was on a ladder & injured walked up a ladder with a piece of sheet metal to hand to him. When injured stepped off ladder he stepped down on a 24"" long X 18"" wide piece of loose concrete & lost his balance & twisted or sprained his right ankle. This was at 3:30pm. About 30 minutes later he removed shoe & ankle was blue.
EMPLOYEE SHOVELED LIGHT WEIGHT AGGREGATE (STARLITE) APPROX. 30 - 35 POUNDS INTO A FIVE GALLON BUCKET. HE REACHED OVER & PICKED THE BUCKET UP TO PUT IT INTO THE BACK OF THE GOLF CART. WHEN HE LIFTED THE BUCKET HE FELT LIKE HE PULLED A MUSCLE IN HIS RIGHT SHOULDER.
EMPLOYEE CAME FROM THE KILN CONTROL ROOM TO GET SOMETHING TO EAT. WHEN HE WENT TO THE BREAKROOM, HE SAID THAT HIS HANDS STARTED SHAKING, THEN HE FAINTED & HIT HIS RT. SIDE OF HIS BACK. HE BRUISED HIS BACK.
Klinker came across the handrail and hit employee on top of his right foot.

EE WAS CLIMBING DOWN THE LADDER OF THE 928 CATERPILLAR LOADER. HIS FOOT SLIPPED OFF OF THE STEP, CAUSING HIM TO FALL ONTO THE SHOP'S CONCRETE FLOOR. FROM HIS FALL EE RECEIVED A FRACTURED FOREARM AND A FRACTURED HIP, BOTH ON HIS LEFT SIDE.
Ee was shooting a klinker out of the #2 rotary kiln, he was using an 8 gauge Winchester ring blaster. when he fired a shell into the rotary kiln, something ricocheted back at him, a piece of metallic material penetrated the right side of his neck. The foreign object measured 3mm x 2mm. His doctor is going to remove it when the swelling goes down.
THE EMPLOYEE WAS CHANGING V-BELTS ON A CRUSHER. HE STEPPED OFF OF THE PLATFORM & FELL BACKWARDS, HE WAS PREVENTED FROM FALLING TO THE GROUND BY HIS SAFETY HARNESS. HE STRUCK HIS RT. LEG ON THE PLATFORM DURING THE FALL CAUSING THIGH CONTUSION & INJURED HIS KNEE.
EE WAS WASHING DOWN (TUNNEL). HE KNELT DOWN TO AVOID A PIECE OF METAL. HE HIT HIS KNEE ON A PIECE OF METAL PROTRUDING FROM THE WALL, THEN LOST HIS BALANCE, FALLING BACKWARDS ONTO AN I BEAM, HURTING HIS LOWER BACK.
EE WAS SHOVELING CASTABLE FROM A MORTAR PAN. AS HE WAS SHOVELING HE WAS TWISTING WITH THE CASTABLE MATERIAL PUTTING IT INTO BUCKETS SO IT COULD BE CARRIED INTO THE KILN. AS HE WAS DOING THIS TASK HE FELT HIS BACK POP.
EE WAS GREASING THE SCREEN BEARINGS ON THE 2 DECK SCREEN. HE USED THE HANDRAIL FOR A LADDER. AS HE WAS STEPPING DOWN ONTO THE SCREEN WALKWAY HE TWISTED HIS RIGHT ANKLE.
EMPLOYEE WAS WALKING ACROSS A SLAB TO CHECK ON A PIECE OF EQUIPMENT. AS HE STOPPED, HIS RIGHT FOOT SLIPPED OFF OF THE SLAB AT WHICH TIME HE SPRAINED HIS ANKLE.
EMPLOYEE WAS TURNED AROUND DISMOUNTING THE 980-B CATERPILLAR LOADER. HIS RIGHT FOOR SLIPPED OFF THE SECOND STEP, FORCING ALL HIS WEIGHT TO HIS LEFT KNEE THAT WAS BENT. WHEN HE SLIPPED HE WAS ABLE TO HOLD ONTO THE HANDRAILS TO KEEP FROM FALLING TO THE GROUND.
A MCCARTER ELECTRIC EMPLOYEE (ID #F880) USED THE IMPROPER METER TO TEST PHASE ROTATION ON A LIVE CIRCUIT. THE METER THAT WAS USED WAS NOT DESIGNED TO BE USED WITH LIVE VOLTAGE. THE METER SHORT CIRCUITED CAUSING A FLASH THAT BURNED EE'S EYES.
ELECTRICAL EVALUATION OF 480V CIRCUIT TO DETERMINE PHASE AND ROTATION OF MOTOR #4 KILN ID FAN. EMPLOYEE SELECTED METER INCORRECTLY FOR JOB TASK. METER SHORTED OUT LIVE VOLTAGE 480V 600A RESULT ING IN AN ELECTRICAL FLASH CAUSING BURNS TO FACE, EARS AND HANDS.
EE WAS SITTING IN HIS TRUCK CAB WHILE OTHER WAS LOADING HIS TRAILER WITH A LOADER. SOON AS HE WAS FINISHED LOADING THE MATERIAL. HE BLEW HIS LOADER HORN TO LET EE KNOW HE WAS FINISHED. HE DID NOT RESPOND. OTHER CHECKED ON EE AND FOUND HIM UNRESPONSIVE.

EE WAS PUTTING GREASE INTO THE BEARINGS ON A TAIL PULLEY. AFTER GREASING THE BEARINGS HIS GREASE GUN GOT STUCK ON THE FITTING. HE PULLED ON THE GREASE GUN, IT CAME LOOSE STRIKING HIM IN HIS FR ONT TOOTH.
EE'S WERE TIGHTENING NUTS ON THE LONG THRUST ROLLER BOLTS. HE WAS PULLING UP WITH A PIPE WRENCH . THE PIPE WRENCH SLIPPED OFF OF THE NUT, CAUSING THE WRENCH TO STRIKE HIM AT HIS LEFT TEMPLE, GIVING A LACEERATION 1 INCH.
A PIECE OF BELT WAS HANGING FROM A CONVEYOR BELT, EE USED A UTILITY KNIFE TO CUT THE BELT FREE. AS HE PULLED THE KNIFE TOWARDS HIM, THE KNIFE CUT THROUGH SUDDENLY AND THE EXERTED FORCE CARRI ED THE KNIFE THROUGH TO CAUSE THE CUT.
EE WAS CUTTING A PIECE OF CONVEYOR BELT WITH A UTILITY KNIFE. AS HE WAS PULLING THE KNIFE TOWARD HIM HE ACCIDENTLY CUT HIMSELF. HE HAS A LACERATION ABOVE HIS LEFT ANKLE, 1"" LONG HE REC'D 5 STITCHES & A TETANUS SHOT
EE WAS ATTEMPTING TO BREAK LOOSE A LARGE PIECE OF MOLTEN ROCK WHICH WE CALL KLINKERS, FROM THE BACK OF #5 COOLER. AS HE BROKE THE ROCK LOOSE WITH A PIPE, THE ROCK ROLLED AND PINCHED HIS LEFT H AND BETWEEN THE MOLTEN ROCK AND THE IBEAM, SMASHING HIS LEFT HAND, RING FINGER.

EMPLOYEE WAS GIVEN 1 BAG OF IV SOLUTION AND ALLOWED TO REST. HE CAME BACK TO WORK.
THE ACCIDENT HAPPENED ON #1 KILN, THE EE WAS HELPING OTHER KILN HELPERS BUST MOLTEN ROCK FROM THE DOOR OF THE COOLER SYSTEM. AS HE WAS POSITIONING THE JACK HAMMER TO THE MOLTEN ROCK HE TWISTED & APPARENTLY STRAINED HIS LOWER BACK. HE'S ON RESTRICTED DUTY.
THREE EMPLOYEES WERE CHANGING A BEARING ON A CONVEYOR HEAD PULLEY. WHILE LIFTING ONE SIDE OF THE PULLEY ONE OF THEM FELT A PAIN IN HIS LOWER BACK.

EMPLOYEE WAS CLEANING UIP BOARDS, PLYWOOD AND OTHER MATERIALS FROM HIS LEFT LEG WENT THROUGH THE HOLE AND WENT DOWN ON HIS RIGHT KNEE AND LEG. EMPLOYEE KEPT THINKING IT WAS GOING TO BE OK BUT FINALLY HAD SURGERY ON 9-26-00, NO LOST TIME ON RESTRICTED.
