Struck against stationary object

Employee was in mill using hand shovel when he scraped his finger against a steel chute. EE received a 3/4 inch cut on his middle finger of his left hand. employee immediately cleaned cut and went back to work. Several days later cut became infected. Went to Dr. on 18 & 19 Jan 06.

Struck by flying object

While in plant working a gust of wind blew a piece of dust into his left eye. He was observed the next day by foreman with watering eye and sent to a doctor who took out dust and prescribed antibiotic eye drops.

EE WAS SETTING A HEAD PULLEY INTO PLACE ON THE LONG 411 CONVEYOR. WHILE TIGHTENING THE DRUM'S BUSHING, THE DRUM SHIFTED, PINCHING EE'S LITTLE FINGER ON RIGHT HAND. EE WRAPPED FINGER AND WENT TO DOCTOR - 6 STITCHES & TETANUS SHOT.

WHEN EE WAS BEING TRAINED FOR HAUL OUT OF FINISHED PRODUCT HE PULLED A LEVER TO OPEN SAND BIN. CAME APART. LOST HIS FOOTING, WENT BACK A COUPLE OF FEET, HIT HIS ELBOW ON THE TAIL GUARD. ELBOW HURT AT END OF SHIFT. WENT TO EMERGENCY ROOM, WAS GIVEN TETANUS SHOT AS PRECAUTION.

EE WAS HELPING REMOVE A GUARD ON HIS OWN CLIMBEDA CHUTED WHILE DOING THIS HE PLACED HIS HAND ON THE SCALPER LAYING IT ON EE'S HAND HE PULLED HISHAND BACK & SLIPPED FALLING TO FLOOR BELOW BREAK ING HIS RIGHT LEG BELOW THE KNEE

WHILE USING A TORCH TO CUT BOLT HE FANED THE TORCH ACROSS HIS KNEE. BURNING AN AREA THE SIZE OF A SLIVER DOLLER 2ND DEG BURN HE WAS SENT TO DR WHO PRESCRIBED ANTIBOITICS AS A PRECAUTION
