DRIVER HAD JUST SCALED OUT HIS LOAD & PULLED AHEAD OF THE SCALE APPROX 117' ALONG THE ROADWAY EAST OF THE PACKHOUSE BLDG. ANOTHER TRUCK FOLLOWED BEHIND HIM ONTO THE SCALE. HE STATED HE SAW DEC EASED PULL HIS RIG OFF TO THE RIGHT SIDE OF THE ROADWAY ON A SLIGHT INCLINE. DECEASED WAS OUT OF HIS TRUCK AT THE REAR LOOKING INTO THE RAISED BED OF UNTARPED MATERIAL. TRAILER WENT OVER.

(Illness) Employee was standing and passed out, fell to the ground, and received a cut on EE's head from the ground. (total time passed out was less than a minute). EE was taken to the hospital, received 3 stitches to close the cut along with IV's. EE was then released, told to stay home one day, permitted by the doctor to drive self home. EE has been working since then.

Left hand caught between tube and chute wall- worker was removing old scrape cleaner- tube was stuck, then broke free, worker did not expect tube to slide forward toward left hand- when it slid tube caught left hand between chute wall and tube- received 7 stitches between left index finger knuckle and middle finger knuckle- no broken bone

Stored coal spontaneous combustion resulting in a fire within the silo. No injuries were sustained. The fire was extinguished within an hour of discovery.

Employee was pulling an 8"" vacuum hose, lost track of footing and stepped back onto the hose they were handling causing an ankle and fibular fracture.

The employee was pulling down on a belt which was connected to a motor pulley and a gear box pulley. The purpose of the task was to rotate the screw conveyor so bolts that were damaged could be replaced. After doing so, the pulley began to turn catching and injuring the employee.

Employee used fingers to switch on electric breaker. Claimed arm pain later in shift. Employer opposed claim based on facts, investigation and absence of mechanism of injury. In June 2022 DHO ruled allowing arm strain. Employer maintains that injury is not work related and is appealing ruling.

Foreman was on the deck of a crane talking to an operator. As EE went to dismount the crane, the foreman tripped and fell to the ground fracturing their heel.

Employee was installing running board on a new pickup truck. The employee was lining up the bracket and the bracket stuck. While trying to free the bracket, the bracket gave way and employee's hand was forced into the running board.

Fall from ladders

Employee was using a ladder to assist an employee to adjust a seal. Employee climbed ladder and as he reached to tie off the fall protection lanyard, the ladder moved causing him to slide. The employee fell to the ground bruising the lower back.

Employee noticed a burning sensation in his left eye but wasn't actually working on anything at the time. The burning got worse as the shift progressed. The employee flushed the irritated eye out with saline and removed two small particles. The eye was still irritated the next shift and employee went to eye doctor who removed another piece of debris.

#5 Kiln lost power due to power outage. On start up after power was restored, a ""pop"" was experienced as the fuel mill was restarted at 10:39 AM. The kiln was shut down immediately following protocol.

Employee was working on rotary kiln, cleaning equipment. Dust or particle of sand got into the employee's left eye.

Employee was shoveling at #4 Kiln feed end stone belt and felt pain in his lower back and the muscle tied up.

The employee was driving a man lift to another area in the plant when she hit a pot hole in the roadway that caused her to be pitched upward. When she came down and landed, she rolled and injured her foot and ankle.

The employee was working at west lime loadout and after the job was finished, she removed her PPE (goggles and safety glasses). In the process of removing the goggles, lime dust got into her left eye.

Struck against a moving object

The employee was operating a loader and while going into a stone pile to load his bucket, the stone abruptly stopped the loader and caused his neck to pop out of place. This was medical only until he went for licensed massage therapy last week (May 18-22, 2015).

A laborer returned to #7 Burning Floor to rake grates. Moments after inserting the rake, EE heard a loud sound and felt a gust of air from the kiln. Upon investigation, it was discovered that this sound was the `rupture disc rupturing in the duct work from the #7 cyclone that was caused by a surge of air through the kiln. There were no injuries associated with this event.

While working at East Lime Loadout, a 2nd shift Vac Truck employee stumbled over his boots and fell onto a handrail. The right side of his chest and stomach made contact with the handrail causing him pain. The employee was evaluated at the ER and given restrictions.

The #5&6 Kiln Burner was on the burning floor cleaning when EE heard a what EE presumed unplanned ignition in the #5 coal mill. The #5 coal mill was running normally prior to the ignition and continued to run after the ignition. The mill was inspected. No major damage was found. The mill was started again and continued normal operation. The cause of the ignition is unknown.

The employee was lifting a garbage bag full of dust bags out of a baghouse when he felt pain in his back. The employee requested medical treatment and was placed on restricted duty at this time.

At approximately 11:30pm an employee reported seeing flames at the #1 Kiln Auxiliary Motor. The fire department was called and the fire was extinguished by 12:15am. No Injuries or Illnesses. The motor was damaged and sent out for repair. The cause of the fire is unknown at this time.

Employee strained neck by turning head to look behind him while backing up in his haul truck. PT, Prescriptions, and Restricted Duty occurred. At the time of the incident the mine operator did not believe the incident to be work related. We are now reporting as the incident has been accepted by Worker's Compensation.

While coming down a set of stairs the employee says she heard a pop in her knee and felt pain. Initially this claim was ruled as non-work related and the employee did not allege a hazard. Since then the claim was accepted by worker's compensation and we are now reporting the claim.

Employee was attempting to slide an aluminum pan underneath a rail car to vacuum out material. While working a crouched position the employee attempted to move the pan by twisting at the torso and felt a pain in lower back. The employee was placed on restricted duty to allow for the lumbar sprain to heal.

Brick layers were re-bricking inside #2 rotary kiln while coworker was adjusting mechanical tensioner supporting the brick, when a single brick slipped and fell approximately 3.5 feet onto the injured employee's left hand causing a fracture to the finger. Return to work with restrictions the following scheduled shift.

Employee was using a spud bar to break up frozen material in the dust bin under lock out conditions. The employee reported shoulder pain shortly after, but continued to work as scheduled. EE continued to work his full duty until the shoulder was surgically repaired, after which he reported for restricted duty on his next scheduled shift.

Vac Trk Opr. was cleaning spillage in N/S Lime Tunnel when overhead 4"" vac hose came off the attachment. Loaded hose knocked off safety goggles and caused lime to impact his left eye requiring medical attention and several restricted days to heal.

At approx. 12:15AM, maintenance shutoff #5 Coal feeder to inspect drive belts. When the #5 Coal Mill suddenly had an internal coal dust ignition. The force blew dust up thru the feed hopper and blew off the lower inlet duct inspection door. Minimal damage with 6 hrs. to clear, inspect system and investigate cause.

Agg Truck Drivr was going to drive R25 Haul Truck, #14-0339. After checking out the truck, while @ the N. Quarry Shanty @ approx. 7:15 AM, he was stepping back out of the cab, he slipped off and fell 4' landing on both feet. In the process he was holding the right grab rail and injured his rt. arm shoulder requiring medical attention.

Utility Equip. Op.,EE walked out of the locker-room into the hallway when he slipped on water from an overhead A/C unit. He fell onto a concrete floor causing an injury to his back and right hand requiring medical attention.

An ignition incident occurred during the relighting of #4 kiln after an electrical storm. The Raymond coal mill was cleaned out of coal dust but when the mill was started, coal dust remaining in the mill ignited inside the kiln. No damage or injury resulted from this incident.

#7 Kiln Leadman was mounting the plant made 40# steel bracket in front of the #7 Kiln firing hood during shutdown to attach Ind. Kiln gun to trim coating. While handling the Tee shaped bracket it suddenly dropped into the receiver and caught his left fifth finger against the housing causing an injury requiring medical repair.

Mason/Labor was walking down #6 Stairway carrying a box of dust suits. A sudden gust of wind caused him to grab the hand railing with his Rt. hand to catch his balance. He gripped the railing catching the small finger on upright support pipe twisting the finger sideways. Injury required medical and limited duty until 2/21/2013

At 1:15PM a fire was reported in the MGO warehouse by plant operators that required both a Fire Dept.response & reporting to MSHA @ 1:28PM. The fire lasted beyond 30 minutes until extinguished @ 3:22PM. Extensive water damage of bagged MGO product from fire Dept.There was not a direct cause of the non-injury fire determined by the Woodville Fire Dept. or the company at this time.

Lime Loader was coming back down the East Lime L/O Decline conveyor walkway and stepped down with his right foot at the Sampler area stairway when his rt. knee suddenly gave out. Continued working and got medical attention later that day and is working restricted duty presently since 1/1/2013.

EE's were installing flap gate to a loadout chute when the pry bar one EE was using slipped causing IEE's finger (right tip index) to be mashed.

Loader was stepping onto the top walkway of a Covered Hopper car. While stepping onto the higher front entry step of the car he tripped and fell forward onto the top of the car on his left side requiring initial first aid. He continued working. A later diagnostic test has resulted in a referral today to an Orthopedic to provide medical treatment for a left shoulder strain.

Repairman working with co-worker were tightening ram cylinder head bolts on #6 preheater using a torque wrench. After handling the wrench and other tools off the ram floor level, EE reported that his mid back felt sore. He went down to the shop and reported this to his supervisor. Later in the breakroom, his back tightened up with pain requiring medical attention.

Equipment Cleaner was brought a large rotary feeder to the truck wash station to clean prior to storage by Repairman. While cleaning the inside of feeder, EE reached inside the housing to advance the vanes to next compartment & pinched rt middle finger under his own motion using his rt hand. Med. treatment w/sutures & restricted duty required until recovery is complete.

Employee was grinding to cut rebar when the grinder snagged causing it to shift, in turn causing the employee's right middle finger to be smashed. He did not report the injury until 01/25/12.

EE was repo. a 35' section of hose on the 5th floor of N. Lime Loadout. At approx. 11:00 while reaching over and pulling the hose closer to the work area he felt a sharp pain in his lower back. A co-worker assisted EE and got help that required medical attention and restricted duty the next day and until recovery is complete.

Repairman was working with 2 co-workers replacing a broken elevator chain pin on top of the elevator. At approx. 3PM he felt a pain in his mid back area while swinging a sledgehammer to drive a new pin into position. Continued working through the remaining week until recurring soreness required medical attention on 1/17/12 and restricted duty temporarily until is complete.

QC Lab Tech was getting #4 Kiln Lime sample @ approx 11 PM & slipped going into NW corner doorway. She later reported her rt knee was sore & continued working throughout her schedule w/o incident. On 1/10/2012 she requested a diagnostic exam to check it out & a strain was diagnosed w/o any restrictions. Later PT was requested began on 1/18/2012 making incident reportable.

After returning from break, the 3 employees proceeded to climb back up the structure. The employees had on all required safety equipment. Once the employees reached about the 15 foot level, the complete rebar structure failed and fell over onto the North side. Jobsite was scheduled to shut down on 12/22/11 for Christmas break and reopened on 01/03/12.

Worker was attached to a 30' tall rebar cage @ the 10'-15' (foot) height. For an undetermined reason the cage collapsed, bringing worker down with it. A rebar struck him over the left eye and across the ribs. The blow resulted in swelling over the left eye & soreness of the chest & ribs. Required medical attention & several days off to recover.

Worker was attached to a 30' tall rebar cage @ the 10'-15'(foot) height. For an undetermined reason the cage collapsed bringing worker down with it. A rebar struck him in the mouth & cut his upper lip requiring stitches. He was released the same day, RTW on the next work day.

Contractor foreman was cutting a 10"" cast iron ductile water pipe using a gas powered saw. At approx. 1:35 the non metallic saw blade suddenly broke and pieces flew out from under protective guard. EE was wearing a secured full face shield. However, pieces flew up under the shield causing a laceration to the right side of his face. Sutures were required. RTW next day.

EE and 2 other repairmen were removing the #1 ESP discharge hopper covers after unit shutdown to cool. Hot dust flushed down from inside the hopper and fell onto EE's lower left leg and safety shoe causing burns requiring medical attention and restricted days to heal.

EE parked the vac. truck and was dismounting from the driver side when he stepped down with his left foot facing the unit when his foot slipped off the access step causing him to swing into the cab floor. EE cont'd. working and reported the event later that day. On 2/3/11 medical attention was required and the EE is now on temp. restricted duty until the rib cage strain heals.

Switchman alleges that while walking over to the center scale track to cut the end rr car away from the train, he tripped and fell causing an injury to his left side. He reported the event to the Co later the next day and continued working until 2/4/11 when it became necessary to seek medical attention. EE left ribs requiring work as Loco Op now until healing is complete.

H.R. Manager was walking away from the office to get her car and slipped and fell on the paved visitor parking area west of the office. When her feet went out beneath her she reached back with her left arm to catch her fall and had an injury to her left wrist. The injury required med. intervention that day and restricted duty thereafter during recovery period.

Labor EE was walking from the foreman's office to the storeroom at approx. 7:10 AM. While walking east of Maintenance shop he caught his left foot on a pallet & fell onto his left side causing an injury. Reported later that morning as 1st Aid only. However, by 11/4/2010, additional pain required medical attention for a Left Ankle injury that has him on restricted duty

PM Repairman was standing on the ground recording his work in front of the #4 Kiln dust pump. While reaching in to touch the electronic recording button on the drive guard, he began to step to his right to go to the next job, and twisted his right knee. This caused a mild strain that required medical attention & several restricted days to heal. RTW full duty 10/4/10.

Electricians were replacing the #6 thermocouple on the Mag plant calciner. At 10:30am ee was pulling to remove the old unit from the housing when it let go suddenly causing him to slip and fall from his step ladder. He initially retuned to work and during a recheck was place on restricted duty on 8/13/10 to assist with healing.

Storeroom Supervisor was walking back into the NW corner door of the storeroom at approx 1:20 PM when he lost his balance and fell onto the pavement in front of the storeroom. He went down onto his right side causing an injury to his right hip and upper right leg femur requiring surgery to repair on June 25, 2010. Off work presently until recovery is complete.

On 6/18/10 at approx 5:15am, normal operating The #5 coal mill experienced a minor explosion. Upon inspection, a pair of plant pliers were found damaged in the reject chute wheel barrow, Pliers entered the mill unknowingly causing an apparent ignition of coal without warning. No damage and no injuries. Never stopped nor caught fire. The event was reported to MSHA @5:27am.

Electrician was walking away from the 5/6 Kiln Bldg. outside the West ground area to join his co worker waiting 25' away in the shop truck. When he stepped down with his left foot in front of the #5 side pocket box, he twisted his ankle requiring medical attention and restricted duty days to complete healing.

A loud noise originated from the area of the coal fan for #5 kiln. Upon inspection, it was determined that 13 out of 42 housing bolts had failed on the fan housing. There were no injuries and no fire as a result of this event. MSHA was notified at 6:52 a.m.

Leadman was closing up the #5 preheater prior to startup for the next shift. EE began to close the 2'x2' cast door on the west of the ram floor and had to reopen it to remove access planks inside. While opening door to his right his right hand middle finger got caught against a fixed ladder causing an open fracture requiring medical attention and temp. restricted duty to heal.

On 12/1/09 EE was attempting to unplug wet screen system. While standing on the screen he reported twisting his rt.knee between the spray bars. Testing diagnosed osteoarthritis until 3/5/10 arthro.exam found and repaired a torn lateral meniscus. Reportable as of 3/5/10 due to medical conf. of torn ligament not just arthritis. Temp. restrict.duty until next sch. shift.

EE was spotting an air compressor for the secondary crusher jobsite on level ground west of bldg. While ee was using the trailer jack stand to raise and lower the hitch off the receiver at the hitch tongue suddenly slid under the receiver as ee was along side & got his left arm caught between the jack & receiver bar causing an injury.

EE was walking away from the shop. At the same time, a pickup was coming from the other side of the shop & from behind EE. The driver was avoiding another EE as he rounded the SE corner of the shop. He suddenly saw EE & began stopping. EE was struck on his rt side & fell down. EMS was called & treatment revealed only contusions - restricted duty.

Repairman was working on new 4 deck lime storage screen with several co-workers. At spprox. 9:45 AM he reached up with his left hand to give a wrench to another repairman, and he suddenly lost his balance and fell forward on the disch. hood rail causing an injury to his rt. arm. RTW next day on restricted duty until healing is complete.

Repairman injury was working inside the north shop preparing a section of pipe for #4 kiln grizzly discharge. While lifting the pipe to reposition on floor stands, he used his right foot to slide the floor stand under the raised end and felt something in his lower back. He continued to work his schedule and required medical on 7/27/09 with restricted duty lower back

Setting up for a job on #6 Kiln Burner Pipe and parked they're truck along the East side of the Bldg. next to #6 Cooler. At approx 2:40am injured was handling a welding lead cord from the back of the truck to another EE. He kicked a pallet out of his way and he stepped down hard, fell and twisted his back and hip. Medical treatment w/restrictions until full return 5/28/09.

EE was standing on #6 driver pier inspecting the trunion thrust roller. While looking up a sudden gust of wind blew dust/debris into his face and a particle went under safety glasses into right eye. Flushed eye several times and continued working. Eye remained irritated and required medical treatment to remove FB and treat abrasion.

Electrician was using his retractable utility knife to cut tape off the SW Diester screen drive motor during a shutdown. While power was locked out, he was holding the knife in his left hand to cut away the tape over the wires when it slipped away from his control with the force resulted in a laceration to the top of his right hand requiring medical attention. RTW same day.

While working on the drive pier on #4 Kiln to adjust the up-hill thruster, metal particle flaked off the kiln. Strong wind conditions blew the particle in behind the safety glasses worn by Repairman. He noticed it that night & continued to work with his crew. The next day his rt eye became worse & he required med. attn to remove the f.b. & Rx drops to promote healing. RTW 9/16/08.

Injury was helping a crew of employees clean out #5 preheater. While using a sledge hammer to strike a steel spud bar held by co worker, Other ee and another ee missed the head of the bar. Instead the handle of the sledge hit the bar and caught his right index finger causing an injury requiring treatment. Splint guard applied to tip fracture and return to regular duties 8/7/8.

While changing a screen on the west Deister screen, the repairman was attempting to move a screen section on the discharge landing. The screen slipped and fell on his foot.

While in the process of trying to unload his trailer in the raised position, a strong gust of wind combined with the wet sticking load hanging up inside the bed, caused trailer and tractor to tip over. Contusions to right arm and ribs.

Labor employee was helping a co-worker cleanout the bed of labor truck east of the truckwash area. While in the bed of the truck, he threw out a 20# rock and twisted back to reach for a broom when his lower back went out causing pain and an injury requiring medical attention with restricted duty during the recovery period.

Repairman was assisting fellow repairman in replacing a conveyor belt at the Stone Plant during the PM shift on 2/29/08. At approx. 7:05 PM while walking along the ground area below the work area he slipped on snow covered ice & fell hard onto his upper back & right shoulder area. This event required medical attention & restricted duty during the recovery period.

While restarting the #5 Raymond coal mill at approx. 10:15 AM, an unplanned ignition occurred from a small amount of coal dust left inside the emply mill. There was no damage to the damper control system and the fan housing. The event wasn't reported until 2/8/08 at which time it was called into MSHA. There were no injuries and no fire as a result of this ignition.

Vac Truck operator, EE was retrieving tools beneath #5 rotary kiln ground area during extreme wind conditions. A sudden gust of windblew dust into her face, and under eye protection. Lime dust got into her right eye. Initiallst aid irrigation got it out and she returned to work. The next day it became worse and required medical treatment and temporary retricted duty to heal.

While changing out manually the 90#, 4'x4' sections lying face down, he suddenly felt pain in his lower back. He continued working and iced his back at home. Whe he returned the next morning he couldn't work and required medical attention w/temp. restrictions.

Undetected hot lime material from #6 kiln cooler system caught the stacked long tunnel conveyor belts on fire. Hand extinguishers could not control the fire. Local fire dept was called @ 12:35 pm and began applying foam suppression after 12:50 pm. Fire was controlled and out by 1:20 pm. MSHA called @ 12:56 pm (SR #1-48113790). There were no injuries involved.

While using a bench vise to secure the new handle in position, EE used another small hammer to drive the wedge pin into the top of the new handle at the head of the chipper. The 2 hammer heads made contact, causing several small pieces of metal to break off and went into top of EE's left hand.

Repairman alleges straining his right shoulder while pulling up a metal trash barrell to dump into a trash hopper in the North Shop. He began the day on 7:00 am and was assigned to clean the shop when this unwittnessed event was reported at approx. 7:50 am. He requested medical attention right away and was diagnosed with a right shoulder sprain and placed on restrictions.

While he reached into the area with his left hand the shaft suddenly slipped off the bar and pinched his left index fingertip.
