Absorption of radiations, caustics, toxic and noxious substances

Employee had a lid come free and had lime hit EE in the face, causing lime in the eye injury.

Struck against stationary object

Employee was exiting EE's car to start EE's shift and step on a rock in the parking lot and rolled EE's ankle, resulting in a sprained ankle.

Over-exertion in lifting objects

Shoveling piles of lime into hopper and strained a chest muscle.

Struck by falling object

While manually assisting a forklift to place a sawhorse, the north end of the sawhorse fell and struck the top of the effected employees right foot resulting in a laceration and fracture.

Employee was removing a sprocket on #3 aerator. The sprocket was heavier than the employee thought and dropped it onto the guard which was on the employee's right foot. The employee also smashed their right thumb between the sprocket and the guard. The employee's right thumb was fractured.

A maintenance employee conducted repairs inside Aerator #3. During repairs, the employee received burns from steam and lime.

Employee slipped on lime putty and landed on EE's right shoulder.

Employee was bent over picking up a drive chain guard for installation when EE felt EE's back strain.

Employee was walking downstairs, missed final step and fell to the floor hitting their left knee. Employee continued to work at full compacity, but medical evaluation determined a fracture of the tibia on 3/31/23. Currently employee is on restricted duty.

Over-exertion in pulling or pushing objects

Employee was assisting mechanical maintenance replace belt on #3 palletizer bag flattener. While feeding the belt through the rollers, the employee felt a pop in the left shoulder. Employee later went to urgent care and was placed on restricted duty, cannot lift more than 3 pounds with left arm or 10 pounds combined arms and no ladder work.

While performing morning stretches, employee felt a pop in right calf muscle while doing the lunge stretch. Employee was taken to urgent care, diagnosed with muscle sprain, instructed to take ibuprofen and ice leg. EE was given restricted duty until follow up on 9/14/2021. On 9/14/2021, leg was still swollen and restricted duty was extended to another follow up on 9/21/2021.

Employee was using an electric pump to fill a sprayer with Mild Acid Detergent. The sprayer overfilled causing the hose to pop out of the sprayer. It sprayed the employee directly in the face. EE then began to flush eyes at the eyewash station. Employee was medically evaluated, treated at emergency room and was released to return to work on 8/16/2021.

Fall down stairs

Employee was walking down the outside lab steps, when EE got tripped up on the third from the bottom step and fell forward to the concrete sidewalk landing on left side. Medical Treatment 3/24/21 - X-Ray Positive multiple fracture of ribs

EE was performing housekeeping activity at the screen plant with a shovel. While the EE was cleaning, there was a sharp pain in the lower back and lower back locked up. OCC Treatment & Restricted Duty

Employee was working on coal elevator and while shoveling strained back. Employee didn't report accident until Monday 3/30. We denied the BWC claim but lost the appeal and was notified on 5/21.

Fall to the walkway or working surface

Employee was changing out 3/4"" bolts on flight for upper dust screw conveyor. EE was using the open end on two ratchet wrenches to tighten the bolt and nut. While tightening towards self with left hand and away from self with the right, the wrench in EE's left hand slipped off the bolt head. Employee lost balance, falling forward onto wrist rolling it over.

Over-exertion in lifting objects

Employee reported to work at 6:30am and after morning crew talk was assigned to remove the slide in the four stone chute feeding the south jet belt. Employee obtained a steel bar used to remove slide and discovered it was stuck. Used the bar to bang on it a few times to loosen it up and attempting to lift and remove again, causing a lower back strain.

Over-exertion in pulling or pushing objects

Employee was closing the east side store room dock door on March 21, 2018 when the door jammed and caused the employee's left arm to jerk. Employee continued working without issue but subsequent medical visits revealed a rotator cuff tear. Surgery was deemed necessary and performed on June 25, 2018.

Working on #1 hydrator cutting steel with grinder. Maintenance operator stood up after completing the work, turned head to the left and felt something fall into left eye. Employee was prescribed eye drops for eye infection.

Employee was pulling scale out of #3 kiln hood when a piece of scale became stuck in the cooler grate. When this happened, employee felt a strain in his lower back.

A mixture of coal and petroleum coke started to spontaneously combust in the # 3 coal bin. This is being reported because the fire lasted more than 30 minutes.

The EE was attempting to remove a grease gun coupled to a grease zerk that had become stuck. The EE pulled on the hose and the coupler whipped back hitting his teeth.

EE was trying to remove lid from a sample missile and while doing so he strained his right wrist. On 5/2/11 EE began P.T. which made this incident reportable.

EE was cutting floor grating with grinding wheel. The wheel hit the edge of grating and kicked, resulting in a cut on the side of the left hand.

Fall from machine

The conditions contributing to his death are still under investigation, there were no direct witnesses to the death of our employee. We know that employee was picking up a bulk load of lime at Graymont. We know that a Graymont employee found him on top of the trailer in a safety harness unresponsive. CPR & AED were administered on site. EMS transported employee to the Hospital.

EE was pulling on a wooden crate in order to align it so a fork truck could pick it up more easily. While pulling on the crate, it pinched EE's left middle and left ring finger tips, fracturing the tuft under the nail.

Employee sprained right wrist while pulling scale from #2 kiln. On 12/29/08, employee went back to doctor due to pain in wrist. On 1/15/09, we received a notice that employee would be getting therapy 3 times per week for 3 weeks. This is the reason the injury became reportable.

The employee was trying to break up a piece of scale from the rotary kiln with a pry bar. When he went to shove the bar into the scale he felt a pain in his right elbow. He is enrolled in physical therapy three times a week for three weeks.

Employee was standing along side of a parked fork truck when a second fork truck backed around the corner hitting the employee.

While poking pocket on #3 Cooler, employee's hand slipped off rod pinching it between pipe & rod cutting right thumb. Taken to ER & they put in 3 stitches.

Employee was shoveling lime in kiln basement and some of the lime dust got into left boot burning top of foot.

Employee was using a jog control to inspect the hydrator when he noticed the lead light he was kneeling on was still in the hydrator and starting to wrap up. He reached down to move the cord and the hydrator pulled his right hand into the path of the side scraper. The skin on the back of his right hand was torn down toward his knuckles and he also broke the tip of his right finger.

Employee was exiting the #1 tube mill and lost his footing causing him to fall off the scaffold platform. When employee fell he broke his left middle finger.

The employee was removing a crusher end cover and when the cover broke loose it swung towards the employee pinching his left hand between the cover and a bucket elevator housing.

Pulling scale #3 Hood Scale Rod slipped off of the piece causing hand to come in contact with the bottom edge of the primary air gate weights cutting finger. Went to emergency room and they put in 3 stitches.

EMPLOYEE WAS WORKING ON #1 KILN STONE INCLINE FEED BELT SKIRTING. AFTER COMPLETING THE JOB HE WALKED DOWN THE SOUTH STAIRWAY. A FEW FEET AWAY FROM THE STAIRS, HE STEPPED ON A STONE WITH HIS RI GHT FOOT AND TWISTED HIS ANKLE. AFTER GOING TO THE DOCTOR, HE WAS TOLD IT WAS FRACTURED.

EMPLOYEE FINISHED SHIFT AND CONSULTED EMERGENCY ROOM PHYSICIAN THESAME DAY BECAUSE EYE WAS IRRITATED. PHYSICIAN REMOVED 2 MORE SPECKS OF DUST AND ADVISED 24 HOURSOF REST.
