The miner was making their way down a fixed two step ladder attached to a catwalk approximately three feet from the ground after doing routine maintenance. Making last step to the ground the miner stepped/turned the wrong way on foot resulting in an ankle sprain/strain.

Employee was getting out of a 730 Cat haul truck and stepped on a rock twisting his left ankle. He went to Greene Memorial Hospital for x-rays & found he had an ankle sprain.

Employee was jacking up a screw shaft with a come-along. While pulling on the handle he strained his lower lumbar muscle in his back.

EE WAS INSTALLING A V-BELT ON AN ELECTRIC MOTOR & 4 X 10 SCREEN. WHILE PULLING ON THE BELT WITH HIS HANDS (FINGERS UNDER BELT), THE BELT SLIPPED ON, ROLLING THE SCREEN DRIVE SHEAVE AROUND AND CATCHING EE'S LEFT RING FINGER UNDER THE BELT. EE JERKED HIS FINGER FREE, PULLING THE END OF HIS FINGER OFF THE BONE.

HE WAITED ABOUT 10 MIN. TO START BRASING AGAIN. WHEN HE LIT THE TORCH A FLASH FIRE BURNED HIS RT ARM, NECK AND RT SIDE OF FACE.
