Co-worker cut approximately 12" long 2"x2" angle iron. Piece fell on the walkway and bounced off the walkway falling approximately 18 ft and then hit the welder in the forehead, which required six stitches.

Contractor was doing our yearly ground check. He was walking up walkway (catwalk) and slipped on ice. Had a small ice storm that morning. He was out of sight with helper when he slipped. He worked about 30 mins after accident happened then left to get injury checked. Mine Foreman was told of the accident by helper after injured left for the Dr.
