Electrical fire started on our canon drill. EE expelled fire extinguisher put out fire. The process EE said EE inhaled some of fire extinguisher fumes. Refused any medical attention, said felt fine but company sent EE to hospital for exam anyways. Was checked and doctor determined there was no further treatment necessary and was fine.

While loading bags of ANFO, employee strained back. He thought he was fine and did not report. Entire company was on vacation Thursday and Friday. He was scheduled for vacation on Monday and called us during the day (11/30/15) to tell us that his back was still very sore and he needed to seek treatment.

The employee exited the scale house to retrieve an extension cord. While retrieving it, her feet got entangled with a hose causing her to trip and fall causing and injury to her right foot, ankle & knee.

After blowing out a blast hole, the employee was struck on his hardhat by a piece of rock which fell from the roof. The employee fell to the ground as a result of the strike. He was helped to his feet by a coworker and reported the incident to the Mine Foreman.

Employee was attempting to loosen the eyelet on an excavator tool cylinder with a pipe wrench and felt a sharp pain in his neck and shoulder. The employee could not function as normal afterwards.

EE was attempting to loosen a hydraulic fitting on a Gradeall XL4200. When the fitting was loosened enough line pressure remained that the hydraulic fluid sprayed onto/into his eye. EE was wearing safety glasses which minimized the amount that entered his eye. First aid was performed at the face and Medical attention was sought.

A manual grease gun with a 15" hose failed while the employee was greasing the wheel loader. When the hose failed, grease shot into his left eye.

EE Roof Bolter, cut his left hand. EE was climbing onto the bolter, grabbed the railing, his hand slipped and rubbed across a bolt attached to the hand rail.

During his annual hearing test, employee's test results confirmed a shift in his hearing sensitivity in his left ear. The test results indicated a severe hearing loss at some mid and high level frequencies. The amount of shift required reporting to MSHA. Employee was notified of his test results and the requirement for him to wear hearing protection at work.

Approximately 5 tons of rock material fell onto the 2 main heading approximately 2400' in by of portal #10 blocking truck traffic through this heading. MSHA inspector investigated the accident on the morning of 7/13/10. There were no injuries as a result of this incident.

A roof fall occurred behind a brattice curtian in 2 room off of 26 south. The fall knocked down the brattice curtain and a power cable that powered a booster fan. The fall is approximately 30 feet wide x 80 feet long x 30 inches thick. The fall did not affect our main haul route or the primary and secondary escape routes.

EMPLOYEE REACHED OUT OF STOCK TRUCK TO UNLOCK TAILGATE AND PULLED A MUSCLE IN HIS LEFT SHOULDER. THE EMPLOYEE FELT PAIN BUT DID NOT SEEK MEDICAL ATTENTION UNTIL 3/10/06. ON 3/10/06 THE EMPLOYEE WAS ABSENT FROM WORK AND WAS PAID FOR TIME AWAY FROM WORK.

The employee stepped on a sharp piece of metal and cut his foot. He was taken for medical treatment due to a diabetic condition. He immediately returned to work. During a routine exam in early April, the employee was informed of an infection in his foot. The employee filed a comp claim and payments started on 4/13/06. The employee remains off of work.

SIDE OF HIS HEAD WAS SCRAPED, BUT NO STITCHES REQUIRED, 2) L. MIDDLE FINGER: CUT & NAIL TORN OFF. SURGICAL REPAIR & PT REQUIRED (NOT AMPUTATION), 3) R. FOOT: FRACTURED. EE IGNORED TRAINING TO STAY OUT OF UNSCALED AREAS.

SOUNDING ROOF AFTER SCALING WAS PERFORMED. CONTACTED LOOSE ROCK WITH BAR. PIECE OF ROCK WEIGHING ABOUT 3 1/2 LBS SLID DOWN BAR AND STRUCK EMPLOYEE ON RIGHT WRIST CAUSING LACERATION THAT REQUIR ED 4 STITCHES.
