Struck against stationary object

Lime sat in silo and plugged over the chute. Before starting to load trucks, employee was attempting to unplug discharge chute from Silo 4 by pushing bar into discharge chute. Material gave way, bar pushed into chute and hand hit part of the chute. Contact with chute broke left index finger.

Fall from machine

The driver was pulling a load from the mill location in the afternoon of 10/19/17. EE had an incident with the truck and trailer causing the load to shift. While trying to remove the straps from the load, EE fell off the trailer. EE broke wrist and toe. EE received stitches near eye, as well.

Fall to the walkway or working surface

The flatbed driver slipped and fell near the tire wash out area. He thought that he broke his arm, so we sent him to the hospital to get an x-ray. Due to our work schedule, he will be off Monday, December 8, 2014. He will return to work on Tuesday, December 9, 2014. A woman stopped in her vehicle to help him. He did not get her name.

Employee was helping a co-worker open a ground lime chute when some material squirted out striking him in the face. Company was not aware until 10/5/09 that he sought medical attention.

Employee was shoveling spilled material on 1/26/09, on 1/28/09 he developed a lump on lower left abdomen.

Employee was bag loading RKP material. When the material entered the bag, dust splashed from bottom of bag and entered his eyes.

Unloading hydrate lime from bulk tank trailer into lime storage silo. Silo filled and transfer hose was unhooked before all pressure was drained causing lime to gush out of hose and onto employee resulting in lime getting in eyes even with safety glasses on.

Employee was stacking hydrated lime. While stacking he felt tightness in his mid to lower back. Employee used vacation days starting 12/27/07 to 1/04/08. Lost time did not start until 1/07/08.

Employee was loading a 50lb bag of RKP material in hand tied bags. He finished a bag of material and it began to tip over. He reached after it to prevent it from falling and as he did his he hit his left pinky finger on the cage near the bag.

Employee struck his head on low overhead area while walking up stairs. The employee continued to work for several days. On July 5, 2006, his doctor gave him light duty work restrictions. Graymont does not have light duty. At this point, it became a lost time injury.

Employee was lifting and felt a pain in his groin area. This accident was reported to MSHA in Dec. of 04 and the employee was off for 42 days. This is a reoccurring injury.

Employee was helping close the hood on #4 kiln with the south trolly when the coal pipe hit the top handrail and the handrail broke off and fell about 15ft striking the employee on the right arm.
