An employee was descending the cab access ladder of a haul truck. The light illuminating the ladder timed out and went dark. The employee continued to descend the ladder in the darkness. The employee thought ee was on the bottom rung of the ladder and stepped down, causing ee to fall and strike knee on the bottom rung of the ladder

EMPLOYEE WAS WORKING ON CRUSHER WHEN HE WAS STUNG BY A BEE; HE IS ALLERGIC AND ADMINISTERED EPI PEN HIMSELF WHICH HE CARRIED IN TRUCK; CALLED 911 TO MAKE SURE HE WOULD BE OK.

Employee was walking down steps at the secondary plant, when he got to the bottom of the steps, he slipped and fell and landed on his left hand. Diagnosed with a fractured thumb.
