Employee complained of pain in back after driving haul truck. The injury was not reported to management for some time afterwards and the determination of being related to work took several weeks, during which time the employee lost 3 days making it reportable.

While watering roads, operator ascended hill towards the rock truck dump. Mid-way the engine of the water truck stalled, causing the water truck to roll backwards down the previously watered roadway. Operator lost control and the rear driver's side tires encountered the safety berm causing the truck to roll over onto it's side.
Employee was removing a hydrualic hose from a cylinder on a 330LC excavator, when the clamp came loose. Hydraulic oil, under pressure, hit him in the right hand, causing swelling and pain in his hand. No oil was found under the skin. Employee went to the hospital and his legs became swollen for an unknown reason, causing him to lose time.
Lifting crib blocking material to support front end of rock truck, he sprained a muscle in left shoulder area.

Employee was cleaning/lifting rock from 60 in under jaw belt and shoveling. He felt a pull in left waist area. This was reported to the supervisor but nothing needed to be done. Several months later it was found to be a hernia and surgery was performed causing him to miss work.
Employee dropped his hard hat and bent down to pick it up. When he started upward he bumped his head on the welding truck compartment door causing a laceration. Stitches were required.
Driller was in back cab putting up mast. Frost in ground gave away causing the jack to slip in mud resulting in the drill upsetting on side.

Employee was changing hydraulic cylinder on crusher and finger was pinched between cylinder and crusher.
EMPLOYEE WAS REMOVING BUCKET TEETH FROM BUCKET USING A SLEDGE HAMMER TO DRIVE TOOTH OFF ADAPTER. A PIECE OF THE TOOTH BROKE OFF AND STRUCK EMPLOYEE IN HIS LEFT UPPER CHEST. THE PIECE OF METAL HAD TO BE REMOVED IN AN OUTPATIENT SURGICAL PROCEDURE. NO PERMANENT DAMAGE IS EXPECTED.
Working on a site after a misfire; working with a knife. Knife slipped, cut finger.
Employee was climbing down from loader at end of shift. He slipped on step and landed on ground. He landed on his feet and hands. There were wintery, snowy conditions. He did not think he had hurt himself at the time, but over a couple months the condition did not improve.
Lifting pins to install into loader frame and felt pain in stomach.

WHILE CLEANING AROUND ROCK BOX WHICH JAMMED AFTER THE STACKER BELT RIPPED, HE WAS DIGGING ROCK WITH A SHOVEL. WHEN HE HIT HE ROCK BOX WITH HIS HAND THE RESULT WAS A BROKEN RIGHT PINKY FINGER.
EMPLOYEE WAS REMOVING THE CUTTING EDGE FROM BUCKET. WHILE LOOSENING BOLTS AND NUTS HE HELD BOLT WITH RIGHT HAND AND CUT FINGER ON SHARP BOLT EDGE. HE WAS TAKEN TO HOSPITAL AND RECEIVED THREE S TITCHES.
EMPLOYEE WAS OPERATING A 980G LOADER AND THROUGHOUT THE DAY NOTICED A PAIN IN HIS LOWER BACK. MANAGEMENT WAS NOT NOTIFIED UNTIL THE FOLLOWING WEEK.
FINISHED HARD FACING BUCKET ON THE 11TH, CONTINUED NORMAL WELDING THE WEEK OF THE 14TH, ON THE 14TH WENT HOME FROM WORK HAD FLULIKE SYMPTOMS. HEADACHE, TIGHTNESS IN CHEST, NAUSEA, FEVER, WENT TO WORK THE REST OF THE WEEK WITH SAME SYMPTOMS, CAME HOME AFTER WORK ON THE 19TH FEVER WASEVEN HIGHER, WENT TO E.R. AND WAS ADMITTED. EXCESSIVE NICKEL EXPOSURE,TRIGGERED CHEMICAL PNEUMON

EE WAS CLIMBING STEWPS ON T 24 WHEN HE SLIPPED AND FELL TO THE GROUND AND TWISTED BACK.
ON 3/7/00 EE PULLED AB MUSCLE TRYING TO LIFT A PIECE OF STEEL. NO FURTHER TREATMENT ON ORIGINAL DATE OF INJURY. ON 4/12/00 EE FELT DISCOMFORT AND WENT TO THE DR. THE DR SAID THE HERNIA WAS BUL GING & SURGERY WAS SCHEDULED FOR 4/24/00.
