Employee was holding angle iron between two piece of channel for x bracing while another employee welded. He was looking away from the welder but the arc was too close and caused welders flash.

Employee was emptying barrel used for burning trash. a piece of rust flew into his right eye. He didn't relaize it lodged there until his eye became uncomfortable, at which time he reported it to the employer.

Employees were removing drum separator from preparation plant. EE was on a ladder to reset come-a-long. The separator shifted and pinned him against a piece of steel.

Struck by flying object

Employee was grinding a plate for the vibrator in the preparation plant, something flew into his eye. It hurt a little at the time, but the pain went away, so he didn't report an injury. The pain returned a week later, he then reported the injury and went to the doctor. The doctor removed a particle from his right eye.

EMPLOYEE WAS OPERATING TRUCK MOUNTED EXCAVATOR IN THE AREA OF THE CRUSHING PLANT. IT IS SUSPECTED THAT AN AIR OVER HYDRAULIC CONTROL DEVICE FAILED ON TRANSMISSION CAUSING THE VEHICLE TO GO OUT OF CONTROL AND CRASH INTO CRUSHER.
