A Caterpillar bulldozer caught on fire at an unknown time the morning of June 7, 2018. The dozer was operated in the morning for approx. 1 hour then the operator left the property. The fire was reported to EMS at approximately 10:00AM by a passer-by. Cause of fire is still unknown.

Injured person was prying a cat pad on cont miner with a rock bar when he strained his back. He has strained back muscles that require him to miss time from work.
Injured person slipped and fell. As he landed on bottom his ribs struck his self-contained self-rescuer. He sustained bruised ribs which have required time off work.
EMPLOYEE WAS OPERATING A ROOF BOLTER WHEN A SMALL SECTION OF ROCK FELL & STRUCK THE CANOPY. IT BROKE INTO PIECES & A SMALL PIECE STRUCK EE JUST ABOVE HIS UPPER LIP. HE SUSTAINED A CUT REQUIRING 8 STITCHES.

Injured person was walking downhill on a sloped roadway when he slipped on fell. he instinctly held out his right arm to cushion his fall which jammed his right shoulder when his arm hit the ground. He sustained an injury to the right rotator cuff which is going to require medical treatment.
TWO EMPLOYEES WERE MOVING A SET OF LOCKERS DOWN STEPS. AS THEY GOT NEAR THE BOTTOM THE EMPLOYEE AT THE UPPER END RELEASED HIS GRIP MOMENTARILY TO MOVE A FIRE EXTINGUISHER. THIS ACTION CAUSED THE LOCKERS TO SLIDE, UNEXPECTEDLY PINCHING THE INJURED PERSON'S RIGHT WRIST. HE SUSTAINED A FRACTURE OF THE RIGHT RADIUS WHICH IS CAUSING HIM TO LOSE TIME FROM WORK.
INJURED PERSON WAS PLACING A PIECE OF MINE BELT ON BOTTOM IN FRONT OF A POWER CENTER WHEN HE INADVERTENTLY STEPPED ON A NAIL STICKING UP OUT OF BELT. HE SUSTAINED A PUNCTURE WOUND OF LEFT FOOT WHICH REQUIRED HIM TO LOSE TIME FROM WORK.
Injured person was pushing a roof bolt (resin) into a bolt hole in the roof with a wrench in the chuck of the roof bolter. The bolt started bending and slipped out of wrench striking him on left wrist. He sustained a fractured left wrist.

INJURED PERSON WAS DRILLING A BOLT HOLE WHEN A PIECE OF ROCK FELL FROM THE ROOF AND STRUCK THE BOLTER CANOPY. THE ROCK BOUNCED OFF THE CANOPY AND FELL STRIKING THE INJURED PERSON ON THE BACK OF THE LEFT HAND. THIS FORCED HIS HAND AGAINST THE DRILL HEAD CAUSING TWO CUTS ON HIS LEFT MIDDLE FINGER. HIS TREATMENT CONSISTED OF 10 STITCHES AND BEING PLACED ON RESTRICTED DUTY.

INJURED PERSON WAS LOWERING A HEAD JACK FOR CONTINUOUS MINER INTO PLACE WHEN THE JACK SLIPPED SMASHING HIS LITTLE FINGER. HE SUSTAINED A CONTUSION TO THIS LITTLE FINGER ON HIS RIGHT HAND. THIS REQUIRED STITCHES AND ALSO CAUSED HIM TO BE PLACED ON RESTRICTED DUTY.
INJURED WAS HELPING PUSH A MANTRIP OVER A SLIPPERY SPOT WHEN HE SLIPPED AND FELL, STRIKING LEFT HIP. HE SUSTAINED A LOWER BACK STRAIN WHICH REQUIRED TIME OFF AND REST.
INJURED PERSON WAS LIFTING A BOX OF RESIN FROM THE BACK END OF THE BOLTER DECK WHEN HE SUDDENLY EXPERIENCED SEVERE PAIN IN HIS BACK. HE SUSTAINED A STRAIN WHICH REQUIRED PHYSICAL THERAPY AND T IME OFF WORK.
