The injured person was rock dusting by hand in the #4 entry. The scoop operator stopped and gave the person notice that he would be tramming through the area. The scoop operator thought the man was clear and began tramming when the scoop bucket struck the man on the foot.

The man was using a rock bar to install belt structure. The rock bar slipped and struck man on the knee causing him to twist his knee.
The man was installing belt structure when he felt a pain in his low back.
Injured was cleaning up accumulations of coal with a scoop tractor and backed up to turn into a crosscut. The persons hand was positioned outside of cab and while pivoting, the machine caught his hand between the cab and frame of machine. The man amputated his middle, ring and small finger of his hand to the second knuckles.
The man was operating a Hilti-Drill to drill a hole to set a belt head. His finger struck the drill bit, lacerating his finger. The man received stitches in his finger. Report filed for medical treatment only.
The man was striking oil can with a hammer to make an opening when he struck his left hand.

A roof fall occurred approximately 100 feet inby the mine opening in the #1 entry. The fall was approximately 25 feet long x 2 feet thick x 18 feet wide. There were no injuries or property damage.
Injured person was hanging line canvas when a loose hanging wire inadvertently struck him in left eye. He sustained a scratched eye which required him to loose one day from work.
DUE TO LOW CEILING HEIGHTS, INJURED EE HAD 2 BENDS IN 4' RESIN GROUTED ROOFBOLTER. UPON UTILIZING DRILL HEAD UP PRESSURE TO ASSIST BOLT INSTALLATION, ROOF BOLT BEARING SLIPPED OFF DRILL HEAD STRIKING EE'S GLOVED RIGHT HAND.

EMPLOYEE WAS DESCENDING LADDER FROM FRONT END LOADER. HIS LEFT ARM WAS ELEVATED ABOVE HIS BODY ON THE LADDER RAIL WHEN HE EXPERIENCED SUDDEN SHOULDER PAIN. RE-INJURY OF PRE-EXISTING CONDITION SUFFERED ON 01/29/05.
Injured employee was assisting shuttle car operator in manually removing trailing cable for inspection. When shuttle car was energized to reel up cable, injured employee was struck in back of left thigh with cable loop.
A roof fall approx. 50' long x 20' wide and 1' to 4' high occurred in #6 entry of 1 left. The fall material was slate with a slip at the top. There were no injuries and no property damage

EE WAS ASSISTING WITH ADVANCE OF SECTION BELT LINE. HIS BACK WAS TURNED TO THE SCOOP TRACTOR BUCKET. EE WAS IN KNEELING POSITION WHEN ORDER WAS GIVEN TO LOWER SCOOP TRACTOR BUCKET. BUCKET CAUGHT AND INJURED EE ON BOTH HEELS.
EE WAS INSTALLING ROOF BOLT AT CORNER OF NEWLY TURNED XCUT. ROOFBOLTING MACHINE WAS POSITIONED ACCORDING TO ROOF CONTROL PLAN. APPROX 30 LB PIECE OF ROCK FELL OUT BETWEEN EXISTING INSTALLED ROOF BOLTS, STRIKING EE ON HIS LOWER LEFT LUMBAR REGION. EE FINISHED SHIFT.
Injured person was stepping up onto the base of the framework neat the tail of no. 1 surface belt. he slipped which over-extended his reach pulling on his left shoulder he sustained stretched ligaments in left shoulder.

EXACT DATE AND TIME ARE UNKNOWN. FOUND ON WEEKLY EXAM. A ROOF FALL OCCURRED IN #1 ENTRY OF 3RD LEFT BELT AT SURVEY STATION #88. IT MEASURED ABOUT 120' LONG X 5' HIGH X 20' WIDE. THERE WERE NO INJURIES AND NO PROPERTY DAMAGE.
Injured person was operating a shuttle car transporting a pond of coal thru a x cut between no. 6 and 7 entires. an rib of coal rolled out striking him on the left side of his head the rib was approx. 18"" in diameter by 7"" thick. he sustained a cut on left temple that required two stitches.
Injured person was helping make a belt splice at #4 main tail. As he swung the hammer to strike a rivet he missed and struck his left hand with the hammer. He sustained a bruise of left hand requiring him to miss time.
Injured person was tramming a shuttle car when he caught his left elbow between the shuttle car canopy post and coal rib. He sustained a fractured bone in elbow which is causing him to lose time from work.

THE FALL MEASURES APPROXIMATELY 24' WIDE X 24' LONG X 10' HIGH. THERE WERE NO INJURIES AND NO PROPERTY DAMAGE, EXCEPT A 24' METHANE EXTENDABLE PROBE AND PROTECTOR.
A ROOF FALL CONSISTING OF LAMINATED SHALE AND A SMALL LAYER OF SANDSTONE OCCURRED IN #6 ENTRY OF 3 LEFT SECTION MMU #004-0. FALL WAS LOCATED 65' IN BY SPAD #92. IT WAS ABOUT 6' HIGH BY 47' LONG BY 20' WIDE. THERE WERE NO INJURIES AND NO PROPERTY DAMAGE.
INJURED PERSON WAS CARRYING ONE END OF A 20HP ELECTRIC MOTOR WHEN HE SLIPPED. THIS THREW HIM OFF BALANCE CAUSING THE MOTOR TO PULL HIM DOWN UNTIL IT HIT THE FLOOR. HE IS HAVING LOW BACK PAIN & WILL BE HAVING FUTURE DIAGNOSTIC STUDIES TO DETERMINE THE EXACT COURSE.
INJURED PERSON WAS RELEASING A COME-A-LONG WHEN THE HANDLE ACCIDENTLY SNAPPED BENDING THUMB BACK ON RIGHT HAND. HE SUSTAINED A HYPER EXTENDED THUMB WHICH CAUSED HIM TO LOSE TWO DAYS FROM WORK.

THE INJURED PERSON HAD FINISHED BOLTING THE FACE OF #2 ENTRY AND WAS PREPARING TO LIFT THE FOOT JACK WHEN HE WAS STRUCK BY A PIECE OF FALLING DRAW SLATE. THIS SLATE HIT HIM ON THE LEFT SHOULDE R. HE SUSTAINED A CRACKED LEFT SCAPULA. HE WAS LOCATED IN A ROOF BOLTED AREA WHEN ACCIDENT OCCURRED.
INJURED EE WAS CHANGING A SWING JACK ON A SCOOP WHEN THE NEW JACK ACCIDENTLY FELL OFF THE SIDE OF THE SCOOP STRIKING HIS LITTLE FINGER ON LEFT HAND. HE SUSTAINED A FRACTURE OF THIS FINGER. THE FINGER HAS BEEN PLACED IN A CAST.

A ROOF FALL WAS DISCOVERED BETWEEN #1 & #2 ROOM OFF 1ST L;EFT SECTION, TWO XCUTS INBY STATION #405. IT WAS ~50' LONG AND 48"" HIGH.
THE MIHE EXAMINER DISCOVERED THE FALL ON HIS WEEKLY RUN. THE FALL DOES NOT EFFECT VENTILATION OR TRAVEL SO IT WILL NOT BE CLEANED UP. IT HAS BEEN DANGERED OFF WITH WARNING SIGNS.

A ROOF FALL OCCURRED IN THE INTAKE DRIFT, APPROXIMATELY 50' INBY THE HIGHWALL. THE AREA WAS SUPPORTED WITH METAL J CHANNELS ON WOODEN LEGS AND AT LEAST ONE 4' RESIN BOLT IN EACH J CHANNEL.
A ROOF FALL MEASURING APPROX 20'W X 30'L X 4'H OCCURRED IN THE #9 ENTRY OF 2ND LEFT APPROX 30' OUTBY STA 514. THE AREA WAS SUPPORTED WITH 48"" AND 60"" RESIN BOLTS AND ROOF MATS.
A ROOF FALL MEASURING 20' L X 20'W X 4'H, OCCURRED IN THE #4 ENTRY OF 1ST LEFT SECTION AT SURVEY STATION 397. THE FALL WHICH IS IN AN INACTIVE AREA OF THE MINE WAS DISCOVERED BY THE MINE EXAMI ONER ON HIS WEEKLY RUN. THE FALL DOES NOT AFFECT TRAVEL OR VENTILATION AND WILL NOT BE CLEANED UP.
EE WAS WALKING UNDERGROUND. HE STEPPED ON AN UNEVEN SURFACE, SLIPPED TWISTING HIS LEFT KNEE. A SHORT TIME LATER HE KNELT DOWN TO PICK UP SOMETHING, HIS KNEE POPPED AND HE COULD NOT STRAIGHTEN IT.
EE WAS ON THE SURFACE USING A SHOVEL TO CLEAN OFF STACKER BELT, WHILE SHOVELING HE TWISTED HIS BODY CAUSING INJURY TO HIMSELF THAT RESULTED IN A HERNIA. EE CONTINUED TO WORK DOING REQULAR DUTI ES TIL HIS SURGERY WAS SCHEDULED. FIRST DAY OFF WORK WAS JAN 22,01.

WHILE GOING FROM A KNEELING TO STANDING POSITION, EMPLOYEE PLACED HIS HAND ON THE BELT GUARD AND CAUGHT HIS FINGER IN THE MOTOR FAN THUS SEVERING RING FINGER TO 2ND KNUCKLE ONHIS RIGHT HAND. THERE WAS A HOLE IN THE GUARDING WHERE HIS FINGER PROTRUDED THRU ACCIDENTLY.
EMPLOYEE WAS OPERATING THE SCOOP WHEN HE BUMPED HIS KNEE AGAINST THE MINE ROOF. HE SUFFERED ONLY A MINOR ABRASION BUT SEVERE SWELLING RESULTED. HE HAD SURGERY ON THIS KNEE IN SEPT. 1988 AND HA S HAD PROBLEMS EVER SINCE.
