Employee was using a 6 ft step ladder and lifting a 50 lb bag of hydrated lime. A fellow employee, driving a skidsteer, was placing a pallet of bagged lime and bumped into the ladder, causing the ee to fall to the floor, striking the right side of his face and right shoulder. The fall resulted and bruising on the right side of employee's face.

PERSONNEL WERE REMOVING A SHAFT SLEEVE FROM A PUMP DRIVE WITH A HAMMER AND CHISEL WHEN A SMALL PIECE OF THE CHISEL BROKE OFF AND STRUCK THE INJURED IN THE NECK.

EE WAS IN A DARK AREA OF THE PLANT, SURVEYING THE AREA FOR PLACEMENT OF A SHUT-OFF VALVE. AN EXTENSION CORD FROM A WORK LIGHT STRUCK THE SIDE OF A DUSTY PIPE, THROWING DUST INTO THE EYES OF TH E INJURED. INJURED HAD TEMPORARILY REMOVED HIS SAFETY GLASSES TO BETTER OBSERVE THE VALVE LOCATION.
